[Pharmacoeconomic evaluation of spinal muscular atrophy therapy in patients with four SMN2 copies diagnosed through newborn screening].

OBJECTIVE: To conduct a comparative assessment of public expenditures associated with the treatment of patients with four copies of the SMN2 gene using presymptomatic treatment strategies versus initiating therapy after symptom onset within the healthcare system of the Russian Federation. MATERIAL AND METHODS: The study presents a mathematical model for cost assessment of initiating disease-modifying treatments including chronic treatments (risdiplam, nusinersen) or one-time gene replacement therapy (onasemnogene abeparvovec) before and after symptom onset in spinal muscular atrophy (SMA). Variations in social care need reporting amongst GP practices in England: a retrospective cohort study in people with multimorbidity.

BACKGROUND: Multimorbidity, the presence of multiple chronic health conditions, presents significant challenges in both health and social care settings. Addressing social care needs, such as assistance with daily activities and support for managing finances, is crucial in care management patients with multimorbidity. However, variability in the documentation and reporting of these needs remains poorly understood. This study aimed to quantify the variations in social care need (SCN) reporting across GP practices in England. METHODS: We conducted a population-based study using electronic health

Views on oral health determinants as described by persons with continuous positive airway pressure-treated obstructive sleep apnoea: a qualitative study.

BACKGROUND: Oral diseases have been associated with cardiovascular diseases, and persons with continuous positive airway pressure [CPAP]-treated obstructive sleep apnoea [OSA] have an increased risk for negative consequences for both oral and general health. CPAP treatment is often life-long and adherence to treatment is essential. Xerostomia is a common side-effect which can lead to treatment abandonment. Oral health is a changeable part of our general health and well-being and exploring the views of oral health determinants from persons with experience of CPAP-treatment is important to preve

Role of male partners in the long-term well-being of women who have experienced severe pre-eclampsia and eclampsia in rural Tanzania: a qualitative study.

Men can be essential sources of support in maternal health, even more so in case of severe acute maternal morbidity (SAMM), affecting 1-2% of childbearing women in low-resource settings. In a qualitative study using semi-structured interviews, we explored the perspectives of nine male partners of women who suffered from (pre-)eclampsia six to seven years earlier in rural Tanzania. Male partners considered their role to be pivotal regarding finances, decision-making in healthcare-seeking and family planning and provided physical and emotional support. After SAMM, households may be affected in t

Access to EU-Authorized Nonreimbursed Anticancer Medicines: An Explorative Survey Among Dutch Medical Oncologists.

PURPOSE: Timelines for market authorization and reimbursement for anticancer medicines differ across European countries. Therefore, medical oncologists may face periods when promising anticancer medicines are not reimbursed, prompting ethical concerns about equitable access, particularly in publicly funded healthcare systems, such as the Netherlands. This study explores Dutch medical oncologists' experiences and moral perspectives regarding access to EU-authorized nonreimbursed anticancer medicines and assesses practice variation. METHODS: A survey targeted 378 Dutch medical oncologists from

A Meta-Review to Guide Military Screening and Treatment of Gambling Problems.

INTRODUCTION: Excessive gambling can cause substantial biopsychosocial problems (e.g., difficulties with finances, relationships, mental, and physical health). For military Service Members, it can also result in security clearance denial or revocation, failure to achieve promotions, and premature career termination. Recent congressional mandates have obligated the U.S. Department of Defense to screen for problematic gambling, the predictive values of which are a function of (i) problem prevalence and (ii) tool sensitivity and specificity. This meta-review (i.e., systematic review of systematic

Social and cultural features of cholera and shigellosis in peri-urban and rural communities of Zanzibar.

BACKGROUND: Responding to the high burden of cholera in developing countries, the WHO now considers vaccination as a supplement to the provision of safe drinking water and improved sanitation in the strategy for cholera control in endemic settings. Cultural concepts of illness affect many aspects of public health. In the first step of a two-step strategy to examine determinants of cholera vaccine acceptance, this study identified social and cultural features of diarrhoeal illness for cholera control in endemic communities. METHODS: A cultural epidemiological study with locally adapted vignett

Outcomes That Matter in Gastrointestinal Cancer Care: A Focus Group Study of Patient-Centred Outcomes Identified by Patients and Care Partners.

BACKGROUND AND OBJECTIVE: Gastrointestinal (GI) cancers pose a substantial burden on patients and care partners, yet outcomes research and clinical tools remain heavily weighted toward clinical endpoints over patient-centred outcomes (PCOs). Without standardised PCOs, risk communication and shared decision-making may revolve around endpoints misaligned with patient priorities. We aimed to identify PCOs meaningful to patients with GI cancer and their care partners. DESIGN, SETTING AND PARTICIPANTS: We conducted a qualitative focus group study with adult patients with a GI cancer diagnosis and

Informing the development of an outcome set and banks of items to measure mobility among individuals with acquired brain injury using natural language processing.

BACKGROUND: The sheer number of measures evaluating mobility and inconsistencies in terminology make it challenging to extract potential core domains and items. Automating a portion of the data synthesis would allow us to cover a much larger volume of studies and databases in a smaller fraction of the time compared to the usual process. Thus, the objective of this study was to identify a comprehensive outcome set and develop preliminary banks of items of mobility among individuals with acquired brain injury (ABI) using Natural Language Processing (NLP). METHODS: An umbrella review of 47 revie

Estimating the fiscal impact of rare diseases using a public economic framework: a case study applied to hereditary transthyretin-mediated (hATTR) amyloidosis.

BACKGROUND: A wide range of rare diseases can have fiscal impacts on government finances that extend beyond expected healthcare costs. Conditions preventing people from achieving national lifetime work averages will influence lifetime taxes paid and increase the likelihood of dependence on public income support. Consequently, interventions that influence projected lifetime work activity, morbidity and mortality can have positive and negative fiscal consequences for government. The aim of this study was to apply a public economic framework to a rare disease that takes into consideration a broad
