Cost-effectiveness of heart failure therapies.

Heart failure (HF) is a leading cause of morbidity and mortality worldwide. Management of HF involves accurate diagnosis and implementation of evidence-based treatment strategies. Costs related to the care of patients with HF have increased substantially over the past 2 decades, partly owing to new medications and diagnostic tests, increased rates of hospitalization, implantation of costly novel devices and, as the disease progresses, consideration for heart transplantation, mechanical circulatory support, and end-of-life care. Not surprisingly, HF places a huge burden on health-care systems, …

Diastolic heart failure: a concise review.

The concept of "diastolic" heart failure grew out of the observation that many patients who have the symptoms and signs of heart failure had an apparently normal left ventricular (LV) ejection fraction. Thus it was assumed that since systolic function was "preserved" the problem must lie in diastole, although it is not clear by whom or when this assumption was made. Nevertheless, many guidelines followed on how to diagnose "diastolic" heart failure backed up by indicators of diastolic dysfunction derived from Doppler echoardiography. Diastolic heart failure is associated with a lower annual mo…

PGC-1 proteins and heart failure.

The transcriptional coactivators PGC-1α and PGC-1β are master regulators of oxidative phosphorylation and fatty acid oxidation gene expression. Pressure overload hypertrophy and heart failure are associated with repressed PGC-1α and PGC-1β gene expression. Maintaining expression of PGC-1α and -β preserves contractile function in response to a pathological increase in workload. Here, we discuss the regulation of PGC-1 proteins under conditions of pressure overload hypertrophy and heart failure.

MicroRNAs: new players in heart failure.

MicroRNAs (miRNAs) are a class of non-coding small RNAs representing one of the most exciting areas of modern medical science. miRNAs modulate a large and complex regulatory network of gene expression of the majority of the protein-coding genes. Currently, evidences suggest that miRNAs play a crucial role in the pathogenesis of heart failure. Some miRNAs as miR-1, miR-133 and miR-208a are highly expressed in the heart and strongly associated with the development of cardiac hypertrophy. Recent data indicate that these miRNAs as well as miR-206 change their expression quickly in response to phys…

Gastrointestinal changes associated to heart failure.

Over the last decade, several studies were conducted on the gastrointestinal changes associated to chronic heart failure. This article presents a literature review on the physiopathology and clinical consequences of pathological digestive changes of heart failure patients. Structural and functional abnormalities of the gastrointestinal tract, such as edema of absorptive mucosa and intestinal bacterial overgrowth, have been leading to serious clinical consequences. Some of these consequences are cardiac cachexia, systemic inflammatory activation and anemia. These conditions, alone or in combina…

Heart failure and carotid body chemoreception.

There is substantial evidence to implicate a role of the carotid body (CB) chemoreflex in sympathetic and breathing dysregulation in several cardio-respiratory diseases, drawing renewed interest in its potential implications for clinical treatment and management. Evidence from both chronic heat failure (CHF) patients and animal models indicates that the CB chemoreflex is enhanced in CHF and contributes to the tonic elevation in sympathetic nerve activity (SNA) and periodic breathing associated with the disease. Although this maladaptive change likely derives from altered function at all levels

Nutritional assessment in heart failure patients.

Heart failure (HF) is a growing epidemic worldwide with a particularly large presence in the United States. Nutritional assessment and supplementation is an area that can be studied to potentially improve the outcomes of these chronically ill patients. There have been many studies reporting the effect of various nutrients on HF patients, often with mixed results. Amino acids such as taurine, which is involved in calcium exchange, has been reported to improve heart function. Coenzyme Q10, a key component in the electron transport chain, is vital for energy production. l-carnitine, an amino acid…

Pleural effusions from congestive heart failure.

In heart failure (HF), pleural effusion results from increased interstitial fluid in the lung due to elevated pulmonary capillary pressure. Rarely, pleural effusions may occur in association with isolated right HF. HF-associated effusions are typically bilateral, but if unilateral, they are more commonly seen on the right side. The fluid typically meets the biochemical characteristics of a transudate, although in 25% of the cases it may fall into the exudative range. Testing for natriuretic peptides, such as NT-proBNP, significantly aids in diagnosing or excluding HF in patients with pleural e…

Renal function in advanced heart failure.

Despite recent advances with neurohormonal antagonists and devices, the prognosis of patients with advanced heart failure (HF) remains grave. Renal dysfunction is a common comorbid condition in HF and is associated with adverse outcomes. Current evidence indicates that intrinsic renal disease and inflammation in HF makes the kidney susceptible to hemodynamic compromise and congestion and contributes to a great extent to the development of renal dysfunction. Relief of congestion requires combination treatment with diuretics, neurohormonal antagonists, and occasionally vasodilators as well as in…

Acute heart failure syndromes: initial management.

Postdischarge mortality and morbidity from acute heart failure syndrome (AHFS) are high, affecting nearly half of all discharged patients within 90 days. Emergency department therapy remains largely empiric, with minimal evidence to support definitive recommendations to guide therapy. However, lessons learned from recent registries and trials suggest an approach to initial management based on clinical profiles, as defined by high, normal, or low blood pressure. Clinicians are provided with a practical and consensus-driven approach to everyday AHFS management.

Targeting histone deacetylases for heart failure.

BACKGROUND: Stresses such as chronic hypertension and myocardial infarction can trigger the heart to undergo a remodeling process characterized by myocyte hypertrophy, myocyte death and fibrosis, often resulting in impaired cardiac function and heart failure. Recent studies suggest key roles for histone deacetylases (HDACs) in the control of pathological cardiac remodeling. OBJECTIVE/METHODS: Here, we review these target validation experiments and highlight non-cardiac functions of HDACs that will need to be addressed during development of HDAC-directed therapies for heart failure. RESULTS/C…

[Vasopressin receptor antagonists and heart failure].

Vasopressin plays a physiological role in regulation of blood pressure, fluid volume, and serum osmolality. In heart failure inadequate release of vasopressin may result in excess fluid retention and hyponatremia. Vasopressin receptor antagonists are a new class of orally active drugs targeted to inhibit one or more of three distinct vasopressin receptors, namely V1a- (-->vasoconstriction), V1b- (-->release of ACTH) und V2-receptors (-->inhibition of free water reabsorption in the kidney). In cardiac decompensation with fluid overload selective V2- (Lixivaptan, satavaptan and tolvaptan) and no…

[Diastolic heart failure: myth or fact?].

Diastolic heart failure is a frequent diagnosis, but now it is more acurate to call this heart failure with preserved ejection fraction. The diagnosis can be made non invasively by the echocardiography and measuring BNP (brain natriuretic peptide). Consensus about treatment are less evidence-based than for heart failure with diminished ejection fraction. It is however certain that control of hypertension and heart rate are important. Fluid overload must also be treated with diuretics.

Heart failure: hemodynamic assessment using echocardiography.

Hemodynamics play a crucial role in diagnosing and managing heart failure (HF) as diagnostic markers and therapeutic targets. In an era of declining physical examination skills and questions about the safety of invasive monitoring, quantitative, objective data provided by echo-Doppler measurements can function as a type of "echo Swan-Ganz catheter" as an important adjustment to traditional methods of hemodynamic assessment. Echocardiographic measures of right- and left-sided filling pressures, pulmonary vascular resistance, and cardiac output are possible in many (although not all) HF patients…
