Congenital heart disease in infants with Down's syndrome.

Medical records of 118 newborn infants with Down's syndrome were reviewed to document the types of congenital heart disease (CHD) in those having echocardiography. Of 102 infants having echocardiography, 49 (48%) had heart defects; 47 of these had trisomy 21 and 2 had unbalanced translocation karyotypes. Of the 53 (52%) who did not have heart defects, all had trisomy except 1 with a mosaic karyotype and 1 with a translocation karyotype. The most common heart malformation was an atrioventricular canal, followed in frequency by ventricular septal defect, atrial septal defect, patent ductus arter

Sinoatrial malfusion presenting with caudal position of right atrial appendage.

A 5-year-old boy underwent surgery at our institution for closure of an ostium secundum atrial septal defect. Median sternotomy revealed that the right atrial appendage lay at the acute margin of the heart. Atriotomy revealed that the crista terminalis was also displaced clockwise, extending from the left of the superior vena caval orifice anteriorly along the interatrial septum, to the left border of the inferior vena cava; the musculi pectinati radiated forward and to the right. This highly unusual anomaly is likely due to clockwise rotation of the atrial portion in relation to the normally

The superior approach for correction of the supracardiac type of total anomalous pulmonary venous return.

An alternative approach for correction of supracardiac (type I) total anomalous pulmonary venous return is described. A median sternotomy is used. The posterior wall of the left atrium and the common pulmonary venous trunk are exposed through the transverse sinus. A direct anastomosis between these structures, ligation of the systemic-venous connection (vertical vein), and closure of the interatrial septal defect results in a one-stage repair. In our experience with the supracardiac anomaly in 20 patients, we have found that this approach consistently affords better exposure than other techniq

Percutaneous closure of multiple defects of the atrial septum: procedural results and long-term follow-up.

BACKGROUND: The percutaneous closure of single atrial septal defect (ASD) is a valid alternative to surgery. OBJECTIVES: To assess the feasibility of percutaneous treatment of multiple ASDs. METHODS: Between 1998 and 2007, 165 out of 1280 consecutive patients undergoing ASD percutaneous closure at our institution showed multiple defects that were classified in four categories: double atrial septal defects (d-ASD), multifenestrated atrial septal defects (f-ASD), multifenestrated defects with no signs of right heart overload (f-PFO), and complex cases (c-ASD). The following end points were tak

Rare anomalies of atrioventricular connection: hidden or supernumerary valves with imperforate right atrioventricular connection.

We report two hearts with rare anomalies of the atrioventricular connection. Both cases showed muscular partitions within the right ventricle, giving it a double-chambered morphology. In case 1 the floor of the right atrium was muscular, creating the impression of the absent connexion variant of tricuspid atresia. A hypoplastic valve, however, was seen in the right ventricular inlet chamber. Case 2 apparently showed an imperforate Ebstein's anomaly of the tricuspid valve but a supernumerary valve, draining into the left ventricle, was found in a chamber that might correspond to part of the cor

The electrocardiogram manifestations of atrial and ventricular septal defects before and after repair.

Atrial septal defects (ASDs) and ventricular septal defects (VSDs) account for the majority of congenital heart disease and may remain clinically relevant from infancy into adulthood. Echocardiography is the gold standard for diagnosis and detailed anatomical assessment of these defects; however, the electrocardiogram (ECG) may provide additional pathophysiological and prognostic insights. This review summarizes ECG findings in ASD and VSD before and after repair, with respect to the effects of defect type, timing of intervention, and postoperative evolution of electrical disturbances. A struc

Surgical anatomy of the infundibular septum in transposition of the great arteries with ventricular septal defect.

Anatomic variation of the infundibular septum was studied in transposition of the great arteries with ventricular septal defect in 23 hearts and double-outlet right ventricle with anterior position of the aorta in two hearts. Anterior displacement of the infundibular septum (i.e., "false" Taussig-Bing heart) was associated with coarctation or interruption of the aortic arch in 88% of the cases, whereas posterior displacement resulted in subpulmonary narrowing in 100% of the cases. Anterior displacement makes intraventricular rerouting from the left ventricle to the aorta difficult because of a

Anomalous origin of left coronary artery from pulmonary artery (ALCAPA): A case report.

Anomalous origin of left coronary artery from pulmonary artery (ALCAPA) also known as Bland-White-Garland Syndrome is a rare anomaly of coronary arteries comprising of 0.25%-0.5% of all congenital heart defects with a prevalence of 1 in every 300,000 live births. Its clinical significance lies in the possibility of resultant coronary steal phenomenon with a left-to-right shunt causing aberrant left ventricular perfusion which may ultimately lead to myocardial ischemia and infarction in children having the abnormality. ALCAPA may manifest as an isolated defect but in 5% of cases it may be assoc

Genetic aspects of congenital heart disease in heterotaxy syndrome.

Abnormal arrangement of thoracoabdominal organs, situs ambiguous, is also known as heterotaxy syndrome (HTX). It has been frequently linked with congenital heart diseases (CHD), which are commonly reported as atrioventricular septal defects (AVSD), atrial septal defects (ASD), ventricle septal defects (VSD), transposition of the great artery, and pulmonary stenosis or atresia. Two HTX categories are right atrial isomerism (RAI) and left atrial isomerism (LAI), which are distinguished by the organ's sidedness as well as their complexity. The etiology of the syndrome is being studied widely, whe

Low incidence of atrial septal defects in nonmammalian vertebrates.

The atrial septum enables efficient oxygen transport by separating the systemic and pulmonary venous blood returning to the heart. Only in placental mammals will the atrial septum form by the coming-together of the septum primum and the septum secundum. In up to one of four placental mammals, this complex morphogenesis is incomplete and yields patent foramen ovale. The incidence of incomplete atrial septum is unknown for groups with the septum primum only, such as birds and reptiles. We found a low incidence of incomplete atrial septum in 11 species of bird (0% of specimens) and 13 species of

Atypical atrial flutter in a patient with atrial septal defect without previous surgery: the role of septal defect as a part of the arrhythmia substrate.

This case report describes an atypical right atrial septal flutter in a patient with a non-corrected atrial septal defect. The unique feature of this case report is that reentrant tachycardia with a cycle around the atrial septal defect was non-scar related. The slow conduction around this atrial septal defect was probably formed by right atrial dilatation and intra-cardiac haemodynamic alterations.

Intraparenchymal Brain Abscess in an Adult Male With Underlying Ebstein Anomaly and Cor Triatriatum Dexter.

A 21-year-old man presented with new-onset seizures and brain abscess. Echocardiography and cardiac magnetic resonance imaging revealed underlying Ebstein anomaly, secundum atrial septal defect, and cor triatriatum dexter. The elevated right heart pressures shunting through the septal defect and transient bacteremia were the likely mechanisms for his presentation. (Level of Difficulty: Intermediate.).

The correction of congenital heart defects with less invasive approaches.

BACKGROUND: The minimally and less invasive approaches for the surgical treatment of different heart diseases are rapidly increasing because of their cosmetic and recovery-related advantages. Presented here are the results of less invasive surgery in 51 patients with congenital heart defects. METHODS: From June 1996 to January 1999, we used less invasive techniques for the correction of congenital heart defects in 51 patients. In 32 patients, we performed right anterolateral thoracotomy (6-13 cm), and on the other 19 patients, we used the partial inferior sternotomy (4-7 cm). The ascending ao

A snare in time--adding a safety mechanism to the Amplatzer Septal Occluder.

Delivery cable/attachment of the Amplatzer Septal Occluder holds the device at a suboptimal angle relative to the plane of the atrial septum prior to release A small gooseneck snare tightened on the hub can maintain flexible control of the device after release of the delivery cable This technique can improve safety of delivery in difficult cases if you think ahead.

Extracardiac Interatrial Tunnel and Atrial Septal Defect Closed With a Single Atrial Septal Occluder in an Adolescent Girl.

In contrast to the atrial septal defect (ASD) as a congenital interatrial shunt, the extracardiac interatrial tunnel is extremely rare. We report the first percutaneous closure of this entity during closure of a secundum ASD after careful investigation of ischemic risk in a 15-year-old girl. The extracardiac interatrial tunnel could entail interatrial shunt, and, if misidentified as an ASD, could result in lethal procedural complications during surgical or catheter interventions. In our case, the tunnel was also morphologically similar to the cardiac arterial/venous system. Therefore, careful
