[Thoracoscopic treatment of secondary pneumothorax to costal osteochondroma].

Costal osteochondromas are benign, uncommon and usually asymptomatic tumors, but may be associated with severe intrathoracic complications such as pneumothorax, hemothorax, nerve or vascular injury and fractures. We report a 12 year old patient, who presented a right spontaneous pneumothorax, which did not improve with standard treatment. Thoracic CT showed a costal exostosis of 2.5 cm in length. Excision was performed by videothoracoscopy (VATS), with good postoperative course. We consider the thoracoscopic approach as the most appropriate initial way in this type of tumors.

Optimizing control of the pleural space.

Pleural space problems can create formidable treatment dilemmas for thoracic surgeons. Most arise as iatrogenic sequelae of lung resections, although some occur as late consequences of infection, hemothorax, or systemic inflammatory disease. Regardless of etiology, a central theme in the development and perpetuation of chronic pleural space problems is that there has been a loss of parietal-visceral pleural apposition, and this has allowed for development of an obligate space. A variety of surgical strategies aimed at reestablishment of pleural apposition and obliteration of pleural space are …

Ruptured thymoma causing mediastinal hemorrhage resected via partial sternotomy.

A case of a ruptured thymoma causing mediastinal hemorrhage and hemothorax that was electively resected by a partial sternotomy approach is presented. This case and others previously reported illustrate that a sudden onset of dyspnea and chest pain accompanied by acute mediastinal widening on chest roentgenogram in a previously healthy patient should suggest the diagnosis of a ruptured thymoma. An upper part sternotomy approach may be as safe and effective as a less invasive surgical procedure in resection of noninvasive thymomas, even if dense tumor adhesion exists.

The successful surgical repair of a traumatic transection of the descending thoracic aorta: report of a case.

An 18-year-old man involved in a traffic accident presented with a left-sided hemothorax. After undergoing left chest tube drainage, he showed temporary stable hemodynamics for 7 h. Later, an emergency left thoracotomy was performed because of abrupt hemorrhagic shock. The intima and media of the descending thoracic aorta was completely transected, and a tube graft was successfully interposed with a simple aortic clamp in 47 min. The patient was eventually discharged from the hospital with no sequelae, such as paraplegia.

[Value of pulmophonography in the diagnosis of pleural complications in closed thoracic injuries].

Mathematical analysis of the qualitative and quantitative signs of pulmophonograms taken in 103 patients with closed chest trauma complicated by hemothorax, pneumothorax, and traumatic exudative pleurisy and a changed method of pulmophonographic examination showed that the diagnosis of these complications can be established. The exact diagnosis was made in 87.4% of cases. The authors discuss the differential diagnosis of pneumo- and hydrothorax recognized by this method and various pathological conditions of the lungs. Areas of hypoventilation were present in the lungs after removal of fluid o…

Spontaneous Hemopneumothorax: An Uncommon Cause of Acute Hemodynamic Collapse.

Spontaneous hemopneumothorax (SHP) is a rare but potentially life-threatening condition that may present with hemodynamic instability or even hypovolemic shock without an apparent cause. Given its nonspecific presentation, diagnosis is often overlooked or delayed. There are no standardized guidelines for SHP management, although the literature tends to favor early surgical intervention. Patients with hemodynamic instability, massive bleeding, persistent air leak, retained hemothorax, or trapped lung should undergo surgery. Minimally invasive surgery may be feasible in unstable patients who res…

Controlling intrathoracic hemorrhage on ECMO: help from Factor VIIa and Virchow.

OBJECTIVE: Thoracentesis with chest tube placement is often needed to decompress a clinically significant pneumothorax or pleural effusion. The risks of such a procedure may be considered too great to perform on a systemically anticoagulated patient supported by extracorporeal membrane oxygenation (ECMO). RESULTS: An 8-year-old child with respiratory failure due to necrotizing pneumonia and autoimmune vasculitis, on veno-venous ECMO, developed a severe tension pneumothorax that required emergent decompression with a chest tube. Post-procedure, the patient developed a hemothorax that was appro…

Outcomes of video-assisted thoracoscopic decortication.

BACKGROUND: Video-assisted thoracoscopic surgical decortication (VATSD) is widely used for treatment of early empyema and hemothorax, but conversion to open thoracotomy for decortication (OD) is more frequent in the setting of complex, chronic empyema. This study compared indications for and outcomes associated with VATSD and OD. METHODS: The outcomes of 420 consecutive patients undergoing VATSD or OD for benign conditions from 1996 to 2006 were reviewed and compared with respect to baseline characteristics, preoperative management, and operative and postoperative course. Patients were analyz…

Management of Blunt Chest Trauma.

Common mechanisms of blunt thoracic injury include motor vehicle collisions and falls. Chest wall injuries include rib fractures and sternal fractures; treatment involves supportive care, multimodal analgesia, and pulmonary toilet. Pneumothorax, hemothorax, and pulmonary contusions are also common and may be managed expectantly or with tube thoracostomy as indicated. Surgical treatment may be considered in select cases. Less common injury patterns include blunt trauma to the tracheobronchial tree, esophagus, diaphragm, heart, or aorta. Operative intervention is more often required to address t…
