Containment refers to the relationship of the most lateral part of the femoral epiphysis to the lip of the acetabulum. Congruence refers to the relation between articular surfaces, but is not as useful a concept as sphericity. Coverage refers to the relationship of the lateral lip of the acetabulum to the resultant force across the hip joint and in most cases is best estimated by the center-edge angle. Femoral heads can be well contained but poorly covered and vice versa. Appreciation of these three concepts is important, not only in considering treatment of the pediatric hip but also in commu…

From January 1970 to December 1976, seventeen patients with twenty-two congenital hip dislocations were treated by open reduction through a medial approach. The average follow-up was 42.2 months. The patients were evaluated for acetabular development, aseptic necrosis, and intraoperative findings. The incidence of aseptic necrosis was 10 per cent. The acetabular index improved rapidly during the first year after reduction. After the first year the rate of development was decreased, but the acetabular index continued to improve throughout the period of study. The medial approach provides a safe…

Eight patients were seen with idiopathic chondrolysis in nine hips, with from one to eight years' follow-up. The adolescent patients showed characteristic diagnostic patterns--patients in the hip and a limp. The disease occurs most frequently in black girls and in them it tends to be more severe. Threre is progressive loss of articular cartilage, frequently progressing to flexion-adduction contracture and fibrous ankylosis. The pathological process in our patients mainly consisted of chronic synovitis and loss of articular cartilate in the weight-bearing surface of the femoral head. The patien…

Osteoporosis in a painful hip is usually a sign of inflammation or tumor. There exists, however, a self-limited clinical entity that presents with pain in the hip and where the only roentgenological manifestation is osteoporosis. Eight patients with this disease are described: five men and three women, ranging in age from 29 to 50 years. In four patients, the onset of disease was related to sprain, pregnancy, or delivery. Laboratory analyses and the clinical course of the disease excluded inflammation or a neoplasm as the cause. All patients fully recovered with 12 month, with no sequelae. The…

Congenital dislocation or subluxation of the hip (congenital acetabular dysplasia) is a complete or partial displacement of the femoral head out of the acetabulum. The physical signs essential for diagnosis are age related. In newborns the tests for instability are the most sensitive. After the neonatal period, and until the age of walking, tightness of the adductor muscles is the most reliable sign. Early diagnosis is vital for successful treatment of this partially genetically determined condition. Various therapeutic measures, ranging from abduction splinting to open reduction and osteotomy…

The division of osteoarthritis into primary and secondary varieties implies that these are aetiologically distinct entities, the former being due to some intrinsic defect of cartilage and the latter resulting from previous articular damage. This traditional concept is questioned and the hypothesis is advanced that osteoarthritis is always secondary to some underlying abnormality of the joint. A detailed clinical, radiographic and morbid anatomical study of 327 cases of osteoarthritis of the hip is presented. In all but twenty-seven some predisposing abnormality of the joint was diagnosed: 107 …

Twenty hip fractures in 19 children aged 2 1/2 to 18 were analyzed in a ten-year retrospective review at the University of Alabama Hospitals in Birmingham, Ala. There was one epiphyseal, one intertrochanteric, 4 subtrochanteric, and 14 intracapsular fractures. The mechanism of injury was usually severe trauma with 31% of the patients sustaining other associated major injuries. Avascular necrosis occurred in three intracapsular fractures (23%), all seemingly related to inadequate internal fixation. Coxa vara occurred in 30% of the patients; all were treated by closed methods, and delayed union …
