The vascular history and physical examination.

The value of the vascular examination cannot be over-estimated. Symptoms of vascular disease present in the foot and lower extremity may act … n made in the technology of the vascular laboratory. If symptoms are discovered during the vascular history and physical examination, the complete noninvasive study will provide impressive data to quantitate and specifically establish the diagnosis.

Leukemia in a child with a history of medulloblastoma.

Leukemia of mixed lineage, was diagnosed in a 6.5-year-old boy with a history of medulloblastoma, 38 months after his initial cancer diagnosis. Therapy had included craniospinal radiation and nitrosourea-based chemotherapy. In addition, onset of leukemia was preceded by therapy with recombinant growth hormone for short stature. Although rare, leukemia is a treatment-related complication for patients with past brain tumors whose follow-up should therefore include surveillance with complete blood counts.

The history of lens implantation in Canada.

The first successful intraocular lens implantation in a patient after cataract removal was performed in 1949 by Dr Harold Ridley. Although several ophthalmologists attempted lens implantation with the Ridley implant in the years that followed, the success rate was so poor and the results so inconsistent that the procedure was abandoned until the Binkhorst system was introduced by Dr Kwitko (1966). Lens implantation gained a certain measure of acceptance by 1975; however, major problems still existed, for example, a lack of satisfactory magnification and frequently occurring flat chambers. Significance of cocaine history in schizophrenia.

Fifty-one schizophrenic inpatients were divided into two groups, those with and without history of cocaine use, and compared on historical, demographic, cognitive, and psychopathological measures. Patients with a cocaine history were found to be significantly more depressed, less socialized, and more impaired in conceptual encoding and verbal memory, while less disordered in attention. The two groups did not differ in severity of illness or positive and negative syndromes. There were also no differences in control variables such as age, gender, education, intelligence, premorbid adjustment, ne…

Nosebleeds--the importance of taking a history.

Nosebleeds are exceedingly common, and usually idiopathic. Cautery under local anaesthetic to the vessels of the nasal septum is often used successfully as a treatment for the recurrent condition. However, rarely, a nosebleed may be a symptom of a more serious underlying disorder and a suitable history and examination are required.

The natural history of peanut allergy.

Between 1973 and 1985, 114 children, aged 2 to 14 years, underwent double-blind, placebo-controlled, food challenge (DBPCFC) to peanut. Thirty-two of 46 children with symptoms produced by DBPCFC to peanut were included in this longitudinal evaluation. Contact was made with the 32 subjects 2 to 14 years after their positive DBPCFC to peanut. All 32 subjects had exhibited a positive puncture skin test to peanut at the time of the original evaluation. Sixteen subjects had experienced symptoms caused by accidental peanut ingestion in the year before contact. Eight subjects had reacted to accidenta

The natural history of symptomatic haemorrhoids.

In order to calculate the prognosis for a person who has developed a first episode of second degree haemorrhoids, 186 patients were randomly assigned to either no active treatment (expectant management) (n = 91) or rubber band ligation (n = 98) and reviewed every 6 months. The median follow-up period was 48 months in both groups with a range from 6-48 months in the banding group and from 18-48 months in the expectant management group. Rubber band ligation was performed at most three times with three weekly intervals. There were 6 treatment failures in the banding group in contrast to 31 in the

The history and challenge of Rett syndrome.

Rett syndrome was first described in 1966 by Dr Andreas Rett, who reported in German his findings in 22 patients. Recognition of the syndrome grew slowly until 1983, when a series of 35 patients from several countries was reported in English. By 1987, the number of known cases had grown to over 1,250 worldwide, the International Rett Syndrome Association had been founded, and international conferences on the syndrome were being held regularly. Although a developmental staging system has been devised, many questions remain concerning the course of the disease. Rett syndrome poses a challenge to

The phylogenetic history of immunodeficiency viruses.

Knowledge of the phylogenetic history of the human immunodeficiency viruses (HIV-1 and HIV-2) is important for our understanding of the epidemiology of AIDS, the disease caused by these viruses. Reconstruction of the evolutionary tree is hampered, however, by two problems. One is the high variation in nucleotide sequence between the known HIV isolates which can create formidable difficulties in identifying homologous genomic sites that may be used in a molecular phylogenetic reconstruction. Another impediment has been the lack of unequivocal time calibration points: there is only a sparse 'fos…

The natural history of splenic infarction.

Our experience at the Cleveland Clinic and that in the literature with splenic infarction were reviewed to describe the natural history of splenic infarction and provide guidelines for management. Data for this review included 75 patients identified by clinical studies or at autopsy during a 10-year period and a review of 77 cases reported in the literature. The cause of the infarct varied with age; patients under 40 years old most often had an associated hematologic disorder, while those older than 41 years old most often had an embolic event. Other etiologic factors included splenic vascular…

A cross-cultural history of abortion.

Abortion is a universal phenomenon, occurring throughout recorded history and at all levels of societal organization. Techniques are highly varied, as are the circumstances under which it is practised. The status of and attitudes towards abortion in Western civilization are variable and have, in most cases, been changing. As of 1982, 10% and 18% of the world's population respectively, lived in countries where abortion was totally prohibited or where it was permitted only to save the mother's life. In the USA, various national surveys indicate liberalization of public attitudes towards pregnanc…

The history of pediatric surgery in France.

In 1741, Nicolas Andry, counsellor of King Louis XV, published a book about "orthopedics," inventing this word. The book is interesting as the author refers to beliefs and habits of the time. In 1864, Guersant published Notes About Pediatric Surgery, a real textbook which was translated into English and German and dealt with the importance of children's psychological training, anesthesia, and water or mother's milk after the operation, and also described tracheotomy, draining of cervical adenitis, and lithotrity. The classification of bone affections was still very confused. Tuberculosis and s

The history and medical consequences of rubella.

In 1814, George Maton, first recognized that a mild illness characterized by rash, adenopathy, and little or no fever was a discrete entity. Henry Veale, in 1866, named the disease rubella. The illness attracted little attention until 1942, when Norman Gregg noticed that first-trimester maternal rubella caused serious birth defects. The full spectrum and impact of rubella embryopathy remained unclarified until rubella virus was isolated in tissue culture in 1962 by two independent groups: Parkman, Buescher, and Artenstein; and Neva and Weller. Using the new tools of the virus laboratory, many

[History of the changes in infectious diseases].

The spectrum of infectious diseases is not at all constant, it changes. This statement is relevant for the great epidemics as well as for nosocomial infections and simple infectious processes. The epidemiological situation of plague, lepra, cholera and diphtheria is discussed. As concerns nosocomial infections four periods are separated: the time before Semmelweis and Lister, the period of the introduction of antiseptic/aseptic measurements to the hospitals and the chemotherapy-time (period until 1965) and the time afterwards. The spectrum of nosocomial infections and its changes as observed i
