Prevalence and predictors of patient-directed discharge in patients with opioid use disorder.

BACKGROUND: Patients with opioid use disorder (pOUD) are at risk of patient-directed discharge (PDD). We hypothesize that multiple risk factors (RF) contribute to this risk. METHODS: PDD prevalence at an urban community hospital was assessed using aggregate data between January 2022-December 2023. Retrospective chart review was conducted of pOUD with PDD (n = 362) over those two years and pOUD discharged conventionally (n = 108) between January 2023-March 2023. Nested multivariable logistic regression models were fit for potential predictors of PDD. RESULTS: In patients without OUD, PDD prev

Inpatient verbal aggression: content, targets and patient characteristics.

Verbally aggressive behaviour on psychiatric wards is more common than physical violence and can have distressing consequences for the staff and patients who are subjected to it. Previous research has tended to examine incidents of verbal aggression in little detail, instead combining different types of aggressive behaviour into a single measure. This study recruited 522 adult psychiatric inpatients from 84 acute wards. Data were collected from nursing and medical records for the first 2 weeks of admission. Incidents of verbal aggression were categorized and associations with patient character

Barriers to effective nurse-patient communication in the emergency department.

BACKGROUND: It has been identified that ineffective nurse-patient communication in the emergency department can lead to frustration, mistrust and inadequate pain management. Therefore, it is important to recognise the potential barriers to effective communication, including nurse and patient-related factors. AIM: To identify what emergency nurses perceive as barriers to nurse-patient communication, and if there is any difference in the perception of communication barriers between nurses' demographic subgroups. METHOD: A total of 199 emergency nurses were recruited using convenience sampling,

[An adapted relational approach to hospitalised adolescents].

The treatment of an adolescent hospitalised in paediatrics often poses difficulties. The relational aspect of the nurse's work in this period of development between childhood and adulthood remains delicate in a context of institutionalisation. What is the interaction between the relational approach and the adolescent's experience of hospitalisation in paediatrics?

'Difficult' Patients: Disciplinary Power and Nursing Practice in Forensic Hospital Settings.

Nurses practicing in forensic mental health hospital settings work with patients involved in the criminal justice system who are also diagnosed with psychiatric illnesses. Nurses work towards collaborative and therapeutic relationships with patients with an eventual goal of recovery and successful discharge to the community. Though the majority of patients in these settings in Canada are diagnosed with psychotic disorders, a smaller proportion may be diagnosed with antisocial personality disorder (ASPD), a patient population often described as 'difficult' or even 'untreatable' by nurses. The impact of an emergency department peer navigator (EDPN) program in improving clinical outcomes and healthcare utilization in an urban setting.

INTRODUCTION: Emergency Department Peer Navigator Programs (EDPN) have been shown to increase the prescribing of medications for opioid use disorder (MOUD) and improve linkage to addiction care. However, what is not known is whether it can improve overall clinical outcomes and healthcare utilization in patients with OUD. METHODS: This is a single-center, IRB approved, retrospective cohort study using patients with OUD enrolled in our peer navigator program from 11/7/19 to 2/16/21. On an annual basis, we determined MOUD clinic follow-up rates and clinical outcomes in those patients who utilize

A-TEAM: An Interprofessional Approach to Mandated Reporting in the Hospital Setting.

Reporting suspected child maltreatment in pediatric settings presents unique challenges. Variation in mandated reporter training may lead to discomfort and emotional dysregulation. Failure to collaborate inter-professionally potentially results in suboptimal care for vulnerable children and families. A-TEAM promotes awareness, transparency, empathy, a nonjudgmental strategy, and management by an interprofessional team when referring patients for child protective services evaluation. A faculty trained in pediatric trauma nursing led the development of A-TEAM. Integrating nursing and social work

Engaging nurses in patient recruitment to research trials in the emergency department.

Research is essential for the implementation of evidence-based practice and can lead to improvements and innovations in care delivery. Although ultimately beneficial for patients and staff, the data collection involved in research often presents challenges, particularly in the emergency department (ED) setting. This article describes the challenges of undertaking a feasibility trial in a city centre ED. The target research population were adult patients who presented to the ED with an issue related to opioid misuse, for example opioid overdose, injection site wounds and/or abscess, or injury s

Interventions for hospitalized medical and surgical patients with opioid use disorder: A systematic review.

Background: Concurrent with the opioid overdose crisis there has been an increase in hospitalizations among people with opioid use disorder (OUD), with one in ten hospitalized medical or surgical patients having comorbid opioid-related diagnoses. We sought to conduct a systematic review of hospital-based interventions, their staffing composition, and their impact on outcomes for patients with OUD hospitalized for medical or surgical conditions. Methods: Authors searched PubMed MEDLINE, PsychINFO, and CINAHL from January 2015 through October 2020. The authors screened 463 titles and abstracts f

Depression and its association with adverse childhood experiences in people with substance use disorders and comorbid medical illness recruited during medical hospitalization.

AIMS: People who have experienced adverse childhood experiences (ACEs) are more susceptible to substance use disorder (SUD) and depression. The present study examined depression prevalence in hospitalized patients with SUD and examined the association of individual ACEs with major depression. Depression rates 3 months after discharge were also examined. METHODS: Medical inpatients with SUD were recruited from Temple University Hospital. Depression was assessed using the Patient Health Questionnaire-9 (PHQ-9) at baseline and 3 months post-discharge. Participants were also assessed using an ACE

Addiction consultation services - Linking hospitalized patients to outpatient addiction treatment.

BACKGROUND: Approximately 15% of hospitalized patients have an active substance use disorder (SUD). Starting treatment for SUD, including medications, during acute hospitalizations can engage patients in addiction care. In July 2015, the Boston Medical Center Addiction Consult Service (ACS), began providing inpatient diagnostic, management, and discharge linkage consultations. We describe this implementation. METHODS: The ACS staff recorded SUDs diagnoses and medication recommendations and tracked follow-up data for affiliated outpatient office-based addiction clinics and methadone maintenanc

Separation and psychological trauma: a paradox examined.

This paper explores the separation of children from their parents that took place in British hospitals in the middle years of the 20th century (Bradley 2001). It is widely believed that this child-parent separation was the product of a failure to understand the child's psychological and social needs (Bowlby et al 1939, Brain and Maclay 1968, Branstetter 1966, Editorial 1957). Paediatric nurses have been blamed for this ignorance and the resulting failure of care (Bowlby et al 1939). However, evidence suggests that such accusations are flawed in logic and in fact: paediatric nurses of the time

Clinical and legal characteristics of inmates with bipolar disorder.

Individuals with bipolar disorder are at an increased risk of criminal arrest compared with those in the population at large. The combination of manic symptoms and substance abuse seem to be the primary illness factors for this increased risk, and the public mental health system has faced challenges in treating these patients successfully in community settings. To decrease the risk for arrest in bipolar patients, clinicians can screen and refer patients for substance use disorders, stabilize mania in hospital settings, and take measures to improve outpatient adherence in the postmanic hospital

Self, agency and the surgical collective: detachment.

In this article, I describe the socio-technical organisation of surgical rehabilitation. After having gone through surgical intervention, patients are implicated within various types of medical work aimed at adjusting their bodies to post-operative social and material environments. My argument is that the process of re-establishment of a 'self' is mediated through a re-disposition of agency in the socio-technical ensemble upon which the patient depends immediately after surgery. Drawing on one year of ethnographic fieldwork in a neurosurgical clinic, and theoretical resources from science and

A treatment crisis: cocaine use by clients in methadone maintenance programs.

We surveyed 11 methadone maintenance treatment programs in the Baltimore, Maryland, area to examine cocaine use among their 2414 clients and the methods employed to cope with that use. The percent of clients with at least one urine sample positive for cocaine during the month previous to study was 15.7% (379/2414) and ranged from 5.9% to 33.0% among the 11 programs. We determined the programs' use of monitoring strategies, treatment services, and administrative controls. We discuss the policy implications of our findings for methadone maintenance programs' efforts to address cocaine use with p

Serious assaults by inpatients.

Forty-nine hospital wards in the United States and Canada were surveyed during specified periods for one year to document assaults by inpatients on staff or other patients. Nine (5.7%) of the 158 assaults by adult psychiatric inpatients were sufficiently severe to require medical treatment. Four others occurred on the only children's service studied. Only one (2.6%) of 39 assaults on adult nonpsychiatric units required medical care. Injuries were rarely serious, with only four assaults in any setting leading to bedrest or missed workdays. The overall rate of assault (assaults per bed per year)
