Upper limb pediatric fractures in 22 tertiary children's hospitals, China: a multicenter epidemiological investigation and economic factor analysis of 32,832 hospitalized children.

BACKGROUND: Fractures are the most common type of unintentional injury in children, with traumatic upper limb fractures accounting for approximately 80% of all childhood fractures. Many epidemiological investigations of upper limb fractures in children have been conducted, but with the development of society, the patterns of childhood fractures may have changed. This study aimed to analyze the epidemiology and economic cost factors of upper limb fractures in Chinese children. METHODS: We retrospectively reviewed children with upper limb fractures or old upper limb fractures hospitalized betwe

In reverse shoulder arthroplasty, selectively medializing the glenoid in higher-risk patients is associated with reduced risk of acromial stress fracture.

BACKGROUND: Reverse total shoulder arthroplasty has roughly a 4% risk of acromial stress fracture (ASF) and is associated with poor outcomes. Previous finite element analyses indicated that glenoid center of rotation lateralization significantly increased acromial strain; however, clinical studies have reported conflicting data with regards to implant position parameters on the incidence of ASFs. The purpose of this study was to evaluate whether medializing the glenoid component in patients at higher risk of ASF was associated with a lower incidence of ASF. METHODS: A retrospective analysis w

Elbow Arthroplasty: A Concise Literature Update.

Total elbow arthroplasty (TEA) is increasingly used, especially in patients with rheumatoid arthritis (RA) and distal humerus fractures (DHFs). This narrative review of the recent literature published in 2022 reached the following conclusions: 1) Age greater than 80 is not a contraindication for TEA. 2) The estimated 10-year survival reported for linked TEAs is 92%, and unlinked TEAs 84%. 3) For DHFs in the elderly, regarding the flexion/extension arc, TEA gives substantially better results than open reduction and internal fixation (ORIF). The reoperation and elbow stiffness rates are substant

Double-locking precontoured plating system for malunited fractures of the distal end of humerus.

BACKGROUND: Distal humeral malunions are uncommon injuries, often associated with limited elbow motion, pain, instability, weakness, and sometimes ulnar neuritis. The complex anatomy of the elbow joint makes this condition one of the most complex elbow injuries to treat. MATERIALS AND METHODS: Four patients were treated by the same surgeon between 2011 and 2013 using a double-locking precontoured plating system for malunited intra-articular or extra-articular fractures of the distal end of the humerus. RESULTS: At a mean 3 years of follow-up, a significant improvement in the elbow motion and

Total Elbow Arthroplasty with Condylar Preservation for Post-Traumatic Non-union with secondary Arthropathy: A Case Report.

INTRODUCTION: Total elbow arthroplasty (TEA) is a valuable salvage procedure, traditionally reserved for elderly or rheumatoid patients. However, expanding indications have allowed selective application in younger patients with debilitating post-traumatic elbow pathology. CASE REPORT: A 74-year-old female presented with persistent instability and ulnar neuropathy following non-union of a distal humerus fracture and chronically dislocated elbow, complicated by progressive arthropathy. On evaluation, she exhibited valgus-varus laxity, restricted range of motion (ROM), and crepitus. A semi-const

The arthroscopic ulnohumeral arthroplasty: from mini-open to arthroscopic surgery.

In cubarthritis-osteoarthritis of the elbow-surgical procedures may be considered to debride the elbow joint to reduce pain, to increase mobility, and to postpone joint replacement surgery. The ulnohumeral arthroplasty as described by Outerbridge and Kashiwagi was originally introduced to debride both anterior and posterior elbow compartments through a direct posterior mini-open approach. To achieve this, a distal humeral fenestration throughout the humeral fossa is performed. Although with an elbow arthroscopy, a technique that was obviously developed later on, all compartments can be easily

Plastic deformation of the forearm in adults: an analysis of 30 cases.

BACKGROUND: Plastic deformation of the forearm is a rare and frequently missed injury in adults that can result in a significant loss of forearm rotation. The condition is reported mainly in Western countries; however, it is not uncommon in Eastern developing countries. We conducted a retrospective study of 30 cases of forearm deformation to find common factors to increase awareness of the condition in trauma doctors. METHODS: We analyzed 30 cases of forearm plastic deformation in adult patients first diagnosed and treated at the Orthopedic Department of our hospital between January 2000 and

Biomechanics of plate fixation following traditional olecranon osteotomy versus novel proximal ulna osteotomy for visualizing a distal humerus injury.

After a distal humeral injury, olecranon osteotomy (OO) is a traditional way to visualize the distal humerus for performing fracture fixation. In contrast, the current authors previously showed that novel proximal ulna osteotomy (PUO) allows better access to the distal humerus without ligamentous compromise. Therefore, this study biomechanically compared plating repair following OO versus PUO. The left or right ulna from eight matched pairs of human cadaveric elbows were randomly assigned to receive OO or PUO and repaired using pre-contoured titanium plates. Destructive and non-destructive mec

Fractures of the proximal humeral epiphysis.

Displacements of the proximal humeral epiphysis at the epiphyseal plate result from extension injuries. The configuration of the epiphyseal plate and the thickness of the periosteum surrounding the epiphysis make slight to moderate displacements relatively stable injuries, which can be treated successfully with external support. In patients with more severe displacement, flexion, abduction, and slight external rotation of the distal fragment bring the metaphysis into alignment with the proximal fragment, correcting the anterior angulation. The thick periosteal sleeve attached to the physis thr

Identifying surgeon-controlled factors associated with acromial fractures after reverse total shoulder arthroplasty: a case-control approach for individual surgeons.

PURPOSE: Patient characteristics are the major determinants of acromial stress fracture risk after reverse total shoulder arthroplasty; the contribution of surgeon-controlled arthroplasty geometry is less certain. This study presents a method by which an individual surgeon can analyze their own fracture cases by comparing the geometry of each case with matched non-fracture controls from the same practice. MATERIALS AND METHODS: Four cases of acromial stress fracture after primary reverse total shoulder arthroplasty by an individual surgeon were analyzed. Two were Levy Type IIB fractures that

Simultaneous internal fixation and latissimus dorsi pedicle flap coverage: A reliable regimen for open fractures with accompanying sizable soft tissue loss of the upper extremities.

BACKGROUND: The optimal treatment protocol for open fractures with accompanying sizable soft tissue defect of the upper extremities has not been specifically delineated. The authors described the concurrent use of internal fixation and latissimus dorsi (LD) pedicle flap coverage in managing such complex fractures. METHODS: Twenty patients with open fractures accompanied by large soft tissue defect of the upper extremities (8 clavicle fractures and 12 humeral fractures) were treated by fix & LD pedicle flap. The dimension of the defect, time to fix & flap, post-operative complications, time to

Preoperative vascular assessment of patients with a supracondylar humeral fracture and a perfused, pulseless limb.

The 'pink, pulseless hand' is often used to describe the clinical situation in which a child with a supracondylar fracture of the humerus has normal distal perfusion in the absence of a palpable peripheral pulse. The management guidelines are based on the assessment of perfusion, which is difficult to undertake and poorly evaluated objectively. The aim of this study was to review the available literature in order to explore the techniques available for the preoperative clinical assessment of perfusion in these patients and to evaluate the clinical implications. A systematic literature review w

Surgical repair of Salter-Harris type II fracture of the distal humerus: short and long-term outcomes in a case series of three dogs and one cat.

CASE HISTORIES: Three dogs and one cat sustained forelimb trauma and were presented to a university veterinary clinic (Liège, Belgium) and a private veterinary hospital (Beacouzé, France). All four animals were referred for surgery. CLINICAL FINDINGS: Two dogs and the cat were ambulatory on admission but unable to bear weight on the affected limb. One dog was non-ambulatory and lacked voluntary movement and sensation in one forelimb. Salter-Harris type II fractures of the distal humerus were diagnosed by radiography in all cases; avulsion of the brachial plexus and pelvic fractures were also

Complex fracture-dislocations of the proximal ulna and radius in adults: a comprehensive classification.

BACKGROUND: Complex fracture-dislocations of the proximal ulna and radius include multiple anatomic lesions, the management of which is known to be demanding. Although several classifications have been proposed, none appear to be exhaustive, and most of them have neither therapeutic nor prognostic value. The purpose of this study was to design a comprehensive classification that may provide a guide for the operative management of these injuries. MATERIALS AND METHODS: The classification is aimed at identifying definite anatomic lesions, called the "main lesions," the presence of which can aff

Ultrasound-guided peripheral nerve stimulation implantation in the spiral groove. A case report.

Neuropathic pain can be treated with peripheral nerve stimulation (PNS), a technique that has become more widespread following the introduction of ultrasound-guided placement. Superficial radial nerve neuropathy is a common disorder, and implanting a PNS device in the distal humeral has been shown to provide good pain control; however, the nerve block test is negative is some patients, and are therefore not candidates for neurostimulation. We report a case of ultrasound-guided implantation of a peripheral nerve stimulator in the spiral groove to treat superficial radial nerve neuropathy. The t

Necrotizing Soft Tissue Infection Following a Closed Distal Radius Fracture: A Case Report and Literature Review.

Necrotizing soft tissue infections (NSTIs), characterized by extensive soft tissue destruction, are rare but life-threatening. We present a case of a NSTI in a healthy 65-year-old woman following a closed distal radius fracture. The patient presented with severe pain, fever, and lethargy 4 days after her index injury, with physical examination of the right upper limb revealing erythema and swelling to the mid-humeral level and blisters of the fingers and hand. Multiple surgical debridements were required to control the infection, which was caused by Streptococcus pyogenes. This case highlights

Late presented case of distal humerus epiphyseal separation in a newborn.

The incidence of traumatic bone injuries occurring during birth is rare (1 per 1000 live births). Of all long bone fractures, the humerus is one of the commonest bones to be involved. However, distal humeral epiphyseal separation is rare and has seldom been written up in case reports and small case series. It warrants some critical assessment and appropriate attention. This injury is sometimes missed as it is difficult to diagnose at initial presentation. It may be mistaken for dislocation of the elbow, osteomyelitis, septic arthritis or brachial plexus injury, owing to lack of movement of the
