Scapular Morphology and Posterior Shoulder Stability: Biomechanical Evidence From an Advanced Cadaveric Shoulder Simulator.

BACKGROUND: Static posterior shoulder subluxation (SPSL) is associated with both glenoid retroversion and altered acromial morphology. Although abnormal glenoid anatomy has been considered a crucial etiological factor, the biomechanical role of acromial anatomy remains incompletely understood. HYPOTHESIS: Combined acromial and glenoid malalignment would produce greater posterior humeral head translation than either deformity alone, and targeted corrections could restore posterior stability. STUDY DESIGN: Controlled laboratory study. METHODS: Six fresh-frozen cadaveric shoulders underwent te…

The biomechanics of the supraspinatus-deficient shoulder treated with superior capsular reconstruction vs. reverse total shoulder arthroplasty-experimental study.

AIM OF THE STUDY: Our objective was to compare biomechanical effects of superior capsular reconstruction (SCR) and reverse total shoulder ar … S tear, (3) SCR using dermal allograft, and (4) rTSA without SCR. Deltoid abduction force, superior humeral head translation, and passive range of motion were measured in static tendon loading condition at 0, 30, and 60° of glenohumeral abduction. RESULTS: Both rTSA and SCR restored abduction force to intact levels at all abduction angles. rTSA significantly …

Shoulder hemi arthroplasty radiological and clinical outcomes at more than two years follow-up.

INTRODUCTION: Humeral head replacement is used for glenohumeral osteoarthritis in young or active patients, for conditions sparing glenoid cartilage or when glenoid implantation does not appear feasible. These surgical procedures usually give satisfactory results but there is a risk of glenoid erosion and a possible deterioration of long-term outcomes. HYPOTHESIS: There is a risk of glenoid erosion after humeral head replacement which can be radiographically measured. The importance and progression of this erosion should be evaluated to determine its clinical relevance. PATIENT AND METHODS: …

Aseptic osteonecrosis of the shoulder: Etiologies, diagnosis and medical management.

Aseptic osteonecrosis of the humeral head is a multifactorial entity in which the final common pathway results in disrupted blood supply, increased intraosseous pressure, and bone death. It follows a pathologic and radiographic pattern of progression similar to that seen at the hip. Much of the information regarding the disease has been extrapolated from findings in the femoral head. In addition, many treatment modalities have been adapted from techniques used in the hip. The causative underlying factors seem to be multifactorial and can be traumatic or non-traumatic. Although clinical symptom…

[Results of rotation osteotomy using the Weber method in the treatment of habitual shoulder dislocation].

Weber's rotational osteotomy is a surgical treatment of recurrent shoulder dislocation. While eliminating recurrence of the complaint, it at … icular operative method. The most common accompanying damage was normally the typical defect of the humeral head (incidence 91%). The regular overstretching of the subscapular muscle and the defect of the humeral head are considered by the operational technique introduced by Weber.

[X-ray examination of the shoulder in suspected luxation (author's transl)].

The X-ray examination of an injured part of a skeleton requires, to obtain a 3-D effect, two vertical levels to one another. The explanation of this radiographic minimum demand is seen especially in the posterior dislocation of the shoulder as the a-p-view with the discrete indirect signs of luxation; this is often misinterpreted as being normal by inexperienced people. Indirect X-ray signs are: fixed internal rotation of the Humerus, "Trough-line", positive "Rim-sign", flattening of the medial aspect of the humeral head and "loss of the normal half-moon shadow" caused by the overlap of the hu…

Neglected anterior shoulder dislocation: open remplissage of the Hill-Sachs lesion with the infraspinatus tendon.

Neglected anterior shoulder dislocation is a rare condition; reduction usually requires an open procedure. There is usually a concurrent Hill-Sachs lesion, with the humeral head impaled over the anterior rim of the glenoid. Large, engaging Hill-Sachs lesions can contribute to continued shoulder instability, and therefore require a specific action. Reconstruction of the humeral head with an osteochondral allograft has been advocated, but allografts are not easily available in some countries, such as Egypt. For this reason, I switched to the open infraspinatus remplissage technique. I report the…

Closed reduction for traumatic posterior dislocation of the shoulder using the 'lever principle': two case reports and a review of the literature.

Traumatic posterior dislocation of the shoulder is frequently missed because of its rarity and the absence of characteristic symptoms. Several signs should be emphasised: an overlap of the humeral head and glenoid rim in a true anteroposterior view and the light-bulb sign in the anteroposterior view. To make an accurate and early diagnosis, use of multidirectional radiographs combined with computed tomography is recommended. Closed reduction was successfully performed under general anaesthesia using the DePalma method with slight modification--the lever principle--by pushing the medial side of…

Fracture-dislocation of the shoulder in a 32-month-old child.

A case of fracture-separation and dislocation of the proximal humeral epiphysis, a rare injury, is reported. The patient, a 32-month-old girl suspected of being an abused child, was reported to have fallen from her crib 2 days earlier. She had a painful, swollen, slightly ecchymotic left shoulder, which she held in the neutral position and refused to move. Radiographs showed a healing, nondisplaced radial fracture and fracture-separation of the proximal humeral epiphysis, which was dislocated into an anterior subglenoid location. Surgery under general anesthesia showed the humeral head to lie …

Arterial injuries in the thoracic outlet syndrome.

PURPOSE: This article reviews experience with arterial injury caused by thoracic outlet syndrome. Special emphasis is placed on the influence of athletic or work activities on the axillary-subclavian artery system and the mechanism by which the humeral head compresses the axillary artery and the circumflex humeral arterial branches. METHODS: Retrospective review identified 34 patients (age range 13 to 67 years) treated for upper extremity symptoms or ischemic complications of thoracic outlet syndrome from 1983 to 1993. Evaluation included assessment of occupational and recreational activities…

Managing Bony Defects of the Shoulder Joint that Occur in Association with Dislocation.

BACKGROUND: Defects to the articular surface of the humeral head have been known to be associated with shoulder dislocation since the 19th century. It wasn't until 1934 that the first description of the ubiquitous compression fracture of the posterolateral humeral head that occurs with traumatic anterior instability appeared. From 1940, this defect became referred to as a Hill-Sachs lesion after the investigators who reported the condition. The significance of, and therefore treatment of, these and other such bony defects around the shoulder joint has been hotly debated. METHODS: We reviewed …

Resolution of Glenohumeral Pseudo-Subluxation: An Illustrative Case Report on Relative Clinical Success in Pan-Brachial Plexus Injury.

Brachial plexus injuries (BPIs) are burdensome to patients. These often involve the axillary nerve, which may lead to pseudo-subluxation of the humeral head because of deltoid atony. While advances in nerve reconstruction have improved outcomes, these procedures still have markedly high failure rates, which may lead to persistent pain, dysfunction and poor satisfaction. We present a case of a 51-year-old male with a traumatic BPI in the context of a motorcycle accident who experienced pseudo-subluxation of his humeral head and later underwent surgical reconstruction for his BPI. Despite a lack…

Open Reduction of a Posteriorly Locked Shoulder Dislocation: A Case Example.

Posterior shoulder dislocations are uncommon but pose diagnostic and treatment challenges, particularly when locked due to humeral head impaction or soft tissue interference. These cases often necessitate surgical intervention, especially when locked or associated with recurrent instability or bony injuries. Preoperative assessment is vital to confirm the diagnosis and guide surgical planning. The surgical procedure begins with patient positioning and anesthesia, followed by a chosen approach, such as the deltopectoral route. In this case, exposure involved identifying and tenodesing the bicep…
