BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is considered a slowly progressive condition, typically requiring a decade to achieve full radiographic development. However, some individuals exhibit accelerated ossification. This study aimed to characterise the clinical profile of these patients, referred to as Fast Ossifiers (FO).
METHODS: Study nested within the Camargo Cohort, integrating cross-sectional and longitudinal data (baseline (E0), 5 year (E1) and 10 year assessments (E2)). Propensity Score matching was applied. FO was defined as progression of ≥2 grades in Schlapbach…

Diffuse idiopathic skeletal hyperostosis (DISH) is a condition in which minor trauma can cause extremely unstable vertebral fractures. Spinal fractures associated with DISH are prone to instability due to the large moment of lever arm and secondary neurological deterioration; hence, surgical internal fixation is considered necessary. On the other hand, some reports suggest that patients with DISH have a high osteogenic potential. In this report, we describe three patients with DISH. These patients had spinal injuries that resulted in a large gap, for which anterior fixation with bone graft wou…

We investigated the risk of heterotopic ossification (HO) after total hip arthroplasty in 67 patients, 16 of whom had diffuse idiopathic skeletal hyperostosis (DISH). DISH was diagnosed on chest, dorsolumbar and pelvic radiographs. HO was graded according to DeLee et al. (1976). After a 1.4 year follow-up, 21 patients had HO. The risk of HO was 5 times higher in DISH than in non-DISH patients, but lower in patients receiving antivitamin K than in those receiving other anticoagulant therapy (relative risk 0.2). Our findings lead us to recommend that DISH should be diagnosed preoperatively for p…

Study DesignRetrospective Cohort Study.ObjectivesThis study explores using deep learning to predict lumbar spine bone mineral density (BMD) from CT images, aiming to improve osteoporosis and osteopenia detection and facilitate early intervention. While dual-energy X-ray absorptiometry (DEXA) is the gold standard, it often overestimates BMD in skeletal hyperostosis. We sought to develop a CT-based model capable of accurately predicting trabecular BMD and validating its fracture-risk prediction performance.MethodsThis retrospective study analyzed 1840 lumbar vertebrae (L1-L4) from 460 patients w…

OBJECTIVE: To quantitatively analyse the morphological characteristics of osteophytes in DISH and syndesmophytes in AS, and summarize different ossification patterns to help identify the two diseases. Associated factors for new bone formation would be investigated.
METHODS: Fifty patients with DISH and 50 age-, sex-, CT examination site-matched patients with AS were enrolled. Radiographic and clinical data were reviewed. Osteophytes (syndesmophytes) in front of each vertebral body and the corresponding intervertebral disc space were defined as vertebral osteophytes unit (VOU). The volume, ang

Diffuse idiopathic skeletal hyperostosis (DISH), also known as Forestier's disease, is a systemic skeletal disorder characterized by ossification of spinal and extraspinal ligaments. While DISH commonly affects the thoracic and lumbar spine, upper cervical involvement (C1-C3) is rare. During routine dissection and subsequent bone retrieval for the osteology library at Goa Medical College, ossification of the anterior longitudinal ligament (ALL) and the alar ligament was observed in the upper cervical vertebrae. Upper cervical DISH may remain asymptomatic or manifest as dysphagia, airway obstru…

Synovitis-acne-pustulosis-hyperostosis-osteomyelitis (SAPHO) syndrome is characterised by aseptic osteitis and is often complicated by pustular dermatitis, such as palmoplantar pustulosis or acne. Although bone lesions are most found in the anterior thoracic region or spine, femoral lesions are not well documented in the literature. There is no established treatment for this condition, and few reports have described its long-term course. Here, we describe two cases of SAPHO syndrome involving the femur and discuss their long-term follow-up. A 40-year-old man (Case 1) presented with right thigh…

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a spectrum of heterogeneous diseases commonly recognised by skin and osteoarticular lesions. There have been reports of some surgical cases of the progressive, destructive spondylitis associated with SAPHO syndrome, wherein the destructive spondylitis was considered to have developed due to the progression of spondylitis with SAPHO syndrome as the pathogenic bacteria were not isolated. We herein report a surgical case of destructive cervical spondylitis associated with SAPHO syndrome. A 54-year-old woman with a history…

CONTEXT: Diffuse idiopathic skeletal hyperostosis (DISH) or Forestier's syndrome may reduce vertebral mobility, thus affecting the stability of adjacent vertebral segments and promoting spinal stenosis, vertebral dislocation, and unstable fracture secondary to low-energy trauma.
AIMS: This study aimed to contribute with a case series of three patients affected by DISH undergone surgery with occipitocervical fixation for craniovertebral junction (CVJ) instability since the poor literature about CVJ instability and surgery in patients affected by DISH.
SETTINGS AND DESIGN: This was a multicent…

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) syndrome and lumbar epidural lipomatosis are relatively asymptomatic neurological conditions, with findings often seen incidentally on radiological studies.
OBJECTIVE: The aim of this paper is to present unique findings of concomitant, symptomatic DISH syndrome and lumbar epidural lipomatosis and to discuss the osteopathic diagnosis and treatment implications.
METHODS: Concomitant, symptomatic variants are rare and present challenges to treatment and management, as seen with a 60-year-old African American woman who presented with wo…

The two most common entities among generally rare but under-diagnosed autoinflammatory bone disorders are chronic recurrent multifocal osteomyelitis (CRMO) and synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome. Due to their similarities, many authors consider CRMO to be a subtype of SAPHO syndrome. The aim of this study was to compare clinical, laboratory, and imaging features and outcomes of patients with CRMO and SAPHO. The analysis of the data from 6 children with CRMO (four girls and two boys, age 3.5-14 years) and of 6 children (6 boys, age 13.5-17.5 years) with SAP…

We describe the case of a 63-year-old female who presented with severe inflammatory spondylitis, refractory to various antibiotics. Mycobacterial and fungal osteomyelitis were unlikely. Although asymptomatic, she also had osteomyelitis in the sternocostoclavicular region, and was suspected of having synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome, against which minocycline showed marked efficacy. The presence of severe inflammatory SAPHO, albeit rare, together with the marked efficacy of tetracycline, should be noted.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is characterised by ossification of the spine including the anterior longitudinal ligament. While often still considered a pure radiographic finding, recent evidence suggests relevant impairments in physical functioning.
OBJECTIVE: To evaluate spinal mobility and functional limitations in DISH compared with radiographic (r-) and non-radiographic (nr-) axial spondyloarthritis (axSpA) and healthy controls (HCs).
METHODS: Consecutive patients and HCs were assessed regarding mobility, including the Bath Ankylosing Spondylitis Metrology …

Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by extensive ossification of the spinal ligaments and entheses, resulting in a rigid spine that behaves biomechanically like a long bone. Although spinal fractures and dislocations after minor trauma are well recognized in ankylosed spines, non-traumatic spontaneous instability is rare. We report a rare case of spontaneous L4/L5 dislocation in a young patient with severe obesity and DISH. A 40-year-old man presented with progressive low back pain, bilateral lower extremity weakness, and bladder-bowel dysfunction. He had initially…

INTRODUCTION: To evaluate the differences in anterior spinal bridging and sagittal spinal parameters between patients with diffuse idiopathic skeletal hyperostosis (DISH) and ankylosing spondylitis (AS) using whole-spine computed tomography (CT).
METHODS: This retrospective study included patients with DISH (n=111) and AS (n=27). The number of anterior spinal bridges and sagittal spinal parameters was evaluated. The sagittal vertical axis (SVA) evaluated by whole-spine CT was defined as sup-SVA. Patients were further evaluated by matching their age and sex.
RESULTS: Anterior spinal bridging …
