The theory of the inflammatory ecosystem.

Chronic inflammatory diseases are increasingly recognized as dynamic and spatially complex disorders involving diverse immune and nonimmune components. In this review, we introduce the concept of the inflammatory ecosystem as a framework for understanding how inflammation is organized across distinct anatomical and functional zones, termed inflammazones-ranging from local to systemic. We first examine how immune cells, through activation, differentiation, and mimicry, transition into inflammatory phenotypes. We then explore the transformation of non-immune cells-such as keratinocytes, fibrobla

Beyond Pathogens: Modulating the Commensal Microbiota to Reshape Immunity for Respiratory Disease Intervention.

The respiratory tract microbiota has been recognized as a niche with complex and diverse microbial communities, the composition and function of which in the state of health and disease have become increasingly clear. Relevant literature on respiratory tract microbiota and immune and respiratory disease published in PubMed, SpringerLink and Web of Science up to June 2026 was narratively reviewed. This review aims to elucidate the interactions between the commensal microbiota of the respiratory tract and the host immune system and reveal how these interactions evolve from homeostasis to disease

Clinical and Genetic Aspects of Alopecia Areata: A Cutting Edge Review.

Alopecia areata (AA) is a chronic, non-scarring, immune-mediated skin disease that affects approximately 0.5-2% of the global population. The etiology of AA is complex and involves genetic and environmental factors, with significant advancements in genetic research occurring in recent years. In addition to well-known genes such as PTPN22, CTLA4, and IL2, which have been widely supported as being associated with AA, an increasing number of specific gene-related loci have been discovered through advances in genetic research. For instance, gene analysis of microRNAs can reveal the critical role o

Insights into innate immune signalling in controlling cardiac remodelling.

Canonical innate immune signalling involves complex cascades: multiple germline-encoded pattern recognition receptors rapidly recognize pathogen-associated or damage-associated molecular patterns to induce the production of cytokines, which bind to their corresponding receptors to orchestrate subsequent host defense phases. Inflammation is a healthy response to pathogenic signals, which are typically rapid and specific, and they terminate once the threat has passed. However, excessive activation or suppression of innate immune or inflammatory responses can lead to considerable human suffering,

The immunophysiology of biliary epithelium.

The immune system maintains the homeostasis of the biliary epithelium, where bile and cholangiocytes play an important role in modulating immune responses. Furthermore, cholangiocytes represent the target of the immune system in several hepatobiliary diseases (e.g., primary biliary cirrhosis). This article is organized into two parts. The first section reviews the role of cholangiocytes in the immune defense and the mechanisms by which cholangiocytes regulate immune responses. The expression of adhesion molecules on the cholangiocyte surface allows the interaction with immune cells, thus local

IgG1 protects against renal disease in a mouse model of cryoglobulinaemia.

Immunoglobulins protect against disease to a considerable extent by activating complement and stimulatory immunoglobulin crystallizable fragment receptors (Ig FcRs), and aggregating microbial pathogens. Yet IgG1, the predominant murine serum Ig isotype, cannot activate complement by the classical pathway, binds more avidly to an inhibitory than to stimulatory FcRs, and has limited ability to aggregate pathogens. In these regards, it resembles human IgG4 (ref. 4). We hypothesized that limited ability to activate effector mechanisms might protect against immune complex immunopathology. Here we s

Glycosylation of mouse and human immune cells: insights emerging from N-glycomics analyses.

N-glycans are key players mediating cell-cell communication in the immune system, interacting with glycan-binding proteins. In the present article, we discuss key themes that are emerging from the structural analysis of complex-type N-linked glycans from human and murine immune cell lines, employing high-sensitivity MALDI (matrix-assisted laser desorption ionization)-TOF (time-of-flight) MS technology. Particular focus is given to terminal epitopes, the abundance of multiply branched N-glycans and how glycosylation can affect human health in diseases such as congenital neutropenia and glycogen

Changes in Immune Response during Pig Gestation with a Focus on Cytokines.

Pigs have the highest percentage of embryonic death not associated with specific diseases of all livestock species, at 20-45%. During gestation processes, a series of complex alterations can arise, including embryonic migration and elongation, maternal immunological recognition of pregnancy, and embryonic competition for implantation sites and subsequent nutrition requirements and development. Immune cells and cytokines act as mediators between other molecules in highly complex interactions between various cell types. However, other non-immune cells, such as trophoblast cells, are important in

Ryanodine receptor and immune-related molecules in diabetic cardiomyopathy.

Hyperglycaemia is a major aetiological factor in the development of diabetic cardiomyopathy. Excessive hyperglycaemia increases the levels of reactive carbonyl species (RCS), reactive oxygen species (ROS) and reactive nitrogen species (RNS) in the heart and causes derangements in calcium homeostasis, inflammation and immune-system disorders. Ryanodine receptor 2 (RyR2) plays a key role in excitation-contraction coupling during heart contractions, including rhythmic contraction and relaxation of the heart. Cardiac inflammation has been indicated in part though interleukin 1 (IL-1) signals, supp

Extracellular vesicle-based vaccines: A promising approach for parasitic diseases.

The development of vaccines to prevent diseases caused by parasites is urgent. Current treatments are highly toxic and ineffective. In addition, these diseases are more prevalent in vulnerable populations and can be fatal in children and immunocompromised individuals. Vaccines for parasites are a challenge in several aspects, including their complex life cycle and mechanisms of evading the immune response. Extracellular vesicles (EVs) are particles released by cells that carry different biomolecules, thus participating in cell-cell communication. EVs released by parasites play a role in the pa

Behçet's disease as an autoinflammatory disorder.

Autoinflammatory diseases are a group of heritable disorders that are characterized by seemingly unprovoked episodes of inflammation at certain locations and and relative lack of high-titer autoantibodies or antigen-specific T cells. Behçet's disease is an inflammatory disorder of unknown aetiology, and many of its characteristic recurrent manifestations overlap with those of autoinflammatory diseases. Behçet's disease has a complex genetic aetiology, and it is more prevalent in certain geographic regions and/or in particular ethnic groups. Enhanced inflammatory response and over-expression of

Environmental factors and rheumatic diseases.

The pathogenesis and pathophysiology of rheumatic diseases is complex and relies on the interaction of different factors. The common view is that the pathological autoimmunity develops in genetically predisposed individuals upon exposure to an environmental trigger. This highlights the importance of recognizing and deconstructing the effects of environmental agents in rheumatic diseases. Several factors have been identified in the last decades, with detrimental or protective effects, impacting not only on disease onset, but also on its natural history. Cigarette smoking has been identified as

Autoimmune Diseases: Molecular Pathogenesis and Therapeutic Targets.

Autoimmune diseases are a set of disorders in which the immune system attacks one's own tissues, leading to chronic inflammation, tissue damage, and systemic dysfunction. Affecting approximately 10% of the global population, these diseases impose significant health and economic burdens worldwide. The pathogenesis of autoimmune diseases is complex, involving not only genetic predisposition (e.g., human leukocyte antigen variants), environmental triggers (e.g., infections), and a dysregulated immune response but also various interacting components that contribute to the development of diverse cl

The role of metabolic reprogramming in inflammatory bone diseases.

Inflammatory bone diseases are characterized by bone destruction, resulting from an imbalance between bone resorption and formation due to chronic inflammation. The dynamic equilibrium among osteoblasts, osteoclasts, and various immune cells is crucial for disease progression. Metabolic reprogramming is a key factor involved in regulating the differentiation and function of these cells, and recent research has increasingly revealed its role in inflammatory bone diseases. This review focuses on the complex roles of glucose, lipid, and amino acid metabolic reprogramming in cellular processes, co

Bullous pemphigoid-associated nephropathy: report of two cases and review of the literature.

Bullous pemphigoid has previously been reported in association with a variety of renal lesions. Two additional cases are presented in this report in which the nephropathy preceded the onset of the skin disease: one case with membranous glomerulopathy and one case of renal allograft rejection with concurrent membranous pathology. Both patients had positive immunofluorescence of the skin, typical of bullous pemphigoid. Institution of systemic corticosteroid therapy resulted in a satisfactory clinical response and cessation of the blistering process. These cases and a review of the literature sug

[Glomerulopathies associated with parasitoses].

Glomerular diseases linked to parasitic infections are frequent in tropical areas: the nephrotic syndrome of African children is often related to malarial infections and hepato-splenic schistosomiasis may be associated with glomerulonephritis. Histological studies indicate that the same parasite can induce a spectrum of glomerular lesions. Immune complexes play a major role in the pathogenesis of these nephropathies but their precise composition is largely unknown. Experimental studies suggest that some of these complexes could be formed by immunoglobulin interactions related to the perturbati

B Cells at the Core: Immune Mechanisms and Therapeutic Potentials in Periapical Lesions.

INTRODUCTION: Periapical lesions (PLs) are common inflammatory diseases primarily caused by microbial infections within root canals. These infections trigger complex immune responses in periapical tissues, with B lymphocytes playing dual roles: defending against pathogens while also contributing to tissue damage. This highlights the crucial role of B cells in the immunological processes of PLs. METHODS: A comprehensive review of the literature on B cells in PLs was conducted using PubMed, Web of Science, Scopus, and ScienceDirect databases. RESULTS: The review included 120 studies that exami

Membranoproliferative Glomerulonephritis Pattern of Injury.

Membranoproliferative glomerulonephritis (MPGN) is no longer a disease but a pattern of injury in various diseases. Characterized by electron-dense deposits, mesangial proliferation, and duplication of the glomerular basement membrane, MPGN was previously classified by findings seen by electron microscopy. However, recognizing complement dysfunction in relation to cases with the MPGN pattern of injury substantially changed our view of its pathogenesis. A new classification, including immune complex-mediated and complement-mediated MPGN, has become preferable and has been adopted by internation
