Does Angiotensin-Converting Enzyme Inhibitor and β-Blocker Use Reduce the Risk of Primary Liver Cancer? A Case-Control Study Using the U.K. Clinical Practice Research Datalink.

STUDY OBJECTIVE: It has been suggested that use of the antihypertensive drugs angiotensin-converting enzyme (ACE) inhibitors and β-blockers may decrease the risk of primary liver cancer; thus, the objective of this study was to evaluate whether use of ACE inhibitors and/or β-blockers is associated with a lower risk of liver cancer. DESIGN: Nested case-control study. DATA SOURCE: United Kingdom Clinical Practice Research Datalink. PATIENTS: We identified 490 cases with hypertension and a first-time (incident) diagnosis of primary liver cancer between 1988 and 2011. To account for an inductio

Potential of fluid-attenuated inversion recovery (FLAIR) in identification of temporomandibular joint effusion compared with T2-weighted images.

OBJECTIVE: The purpose of this study was to determine the potential of fluid-attenuated inversion recovery (FLAIR) sequence images in the identification of joint effusion (JE) compared with T2-weighted images. STUDY DESIGN: A total of 31 joints (28 patients) with JE were investigated by magnetic resonance imaging (MRI). Regions of interest were placed over JE, cerebrospinal fluid (CSF), and gray matter (GM) on T2-weighted and FLAIR images and their signal intensities compared. The signal intensity ratios (SIRs) of JE and CSF were calculated with GM as the reference point. The Pearson product-

Semi-automatic level-set based segmentation and stenosis quantification of the internal carotid artery in 3D CTA data sets.

We present a new level-set based method to segment and quantify stenosed internal carotid arteries (ICAs) in 3D contrast-enhanced computed tomography angiography (CTA). Within these data sets it is a difficult task to evaluate the degree of stenoses deterministically even for the experienced physician because the actual vessel lumen is hardly distinguishable from calcified plaque and there is no sharp border between lumen and arterial wall. According to our knowledge no commercially available software package allows the detection of the boundary between lumen and plaque components. Therefore i

High-resolution functional vascular assessment with ultrasound.

In order to improve the resolution of contrast-assisted imaging systems, we have created a high-frequency destruction/contrast replenishment imaging system with a spatial resolution of 160 microm x 160 microm. The system utilizes a 1-MHz cylindrically focused transducer for destruction and a 25-MHz spherically focused transducer for pulse/echo imaging. Speckle tracking and a clutter filter are applied across frames to remove the challenging physiologic motion artifacts that are obtained when imaging with a mechanically scanned transducer. Using a new estimation technique, flow constants propor

Response of rainbow trout transcriptome to model chemical contaminants.

We used high-density cDNA microarray in studies of responses of rainbow trout fry at sublethal ranges of beta-naphthoflavone, cadmium, carbon tetrachloride, and pyrene. The differentially expressed genes were grouped by the functional categories of Gene Ontology. Significantly different response to the studied compounds was shown by a number of classes, such as cell cycle, apoptosis, signal transduction, oxidative stress, subcellular and extracellular structures, protein biosynthesis, and modification. Cluster analysis separated responses to the contaminants at low and medium doses, whereas at

Overall and cause-specific hospitalisation and death after COVID-19 hospitalisation in England: A cohort study using linked primary care, secondary care, and death registration data in the OpenSAFELY platform.

BACKGROUND: There is concern about medium to long-term adverse outcomes following acute Coronavirus Disease 2019 (COVID-19), but little relevant evidence exists. We aimed to investigate whether risks of hospital admission and death, overall and by specific cause, are raised following discharge from a COVID-19 hospitalisation. METHODS AND FINDINGS: With the approval of NHS-England, we conducted a cohort study, using linked primary care and hospital data in OpenSAFELY to compare risks of hospital admission and death, overall and by specific cause, between people discharged from COVID-19 hospita

Determining the Electronic Signature of Infection in Electronic Health Record Data.

OBJECTIVES: Recent sepsis studies have defined patients as "infected" using a combination of culture and antibiotic orders rather than billing data. However, the accuracy of these definitions is unclear. We aimed to compare the accuracy of different established criteria for identifying infected patients using detailed chart review. DESIGN: Retrospective observational study. SETTING: Six hospitals from three health systems in Illinois. PATIENTS: Adult admissions with blood culture or antibiotic orders, or Angus International Classification of Diseases infection codes and death were eligible

Emergency Department Visits Prior to Suicide and Homicide: Linking Statewide Surveillance Systems.

BACKGROUND: Emergency departments (EDs) serve a wide range of patients who present at risk of impending suicide and homicide. AIMS: Two statewide surveillance systems were probabilistically linked to understand who utilizes EDs and then dies violently within 6 weeks. METHOD: Each identified case was matched with four randomly selected controls on sex, race, date of birth, resident zip code, and date of ED visit vs. date of death. Matched-pair odds ratios were estimated by conditional logistic regression to assess differences between cases and controls on reported diagnoses and expected payme

PEPTY: a knowledge-based program for assisting medical reasoning in peptic diseases.

PEPTY is a program developed with the aim of providing a diagnostic and therapeutic assistance in managing peptic diseases. Its theoretical basis is an accurate analysis of current concepts in peptic disease diagnosis and treatment. This was done by reviewing recent literature and consulting skilled gastroenterologists. The decision tree includes three sections dealing with diagnostic, therapeutic and monitoring problems. The diagnostic section starts by evaluating clinical data from patient history and physical examination; the diagnostic hypotheses given at this level are refined and eventua

A Google Earth-based database management for schistosomiasis control in Zanzibar.

Schistosomiasis remains a serious health problem in Africa. Although a strong, coordinated agenda for research on this disease has been in place for the last 50 years in Zanzibar, data storage, retrieval of survey data and management remain problem areas. We investigated the use of Google Earth (GE) in conjunction with a hand-held, global positioning system as a pilot project for managing schistosomiasis control. In this way, risk areas can be surveyed and followed up by visualizing both the distribution of human infections and that of the intermediate snail host together with environmental in

Efficient Trichromatic Ultralong Persistent Luminescence Activated by Body Temperature Toward Convenient Information Encryption and Storage.

Long-persistent luminescence (LPL) materials, celebrated for their remarkable ability to capture and gradually release light over extended durations, have catalyzed advances in domains such as optical data storage and bioimaging. However, the synthesis of efficient, multicolor ultralong LPL with precise modulation of duration remains a formidable challenge. Here, we introduce an effective strategy for inducing trichromatic ultralong LPL in Cs2NaScCl6:Sb3+ through meticulous defect engineering. By judiciously adjusting the dosage of hydrochloric acid during synthesis, we systematically regulate

Quantum teleportation between remote atomic-ensemble quantum memories.

Quantum teleportation and quantum memory are two crucial elements for large-scale quantum networks. With the help of prior distributed entanglement as a "quantum channel," quantum teleportation provides an intriguing means to faithfully transfer quantum states among distant locations without actual transmission of the physical carriers [Bennett CH, et al. (1993) Phys Rev Lett 70(13):1895-1899]. Quantum memory enables controlled storage and retrieval of fast-flying photonic quantum bits with stationary matter systems, which is essential to achieve the scalability required for large-scale quantu

The role of short-term memory impairment in nonword repetition, real word repetition, and nonword decoding: A case study.

In a companion study, adults with dyslexia and adults with a probable history of childhood apraxia of speech showed evidence of difficulty with processing sequential information during nonword repetition, multisyllabic real word repetition and nonword decoding. Results suggested that some errors arose in visual encoding during nonword reading, all levels of processing but especially short-term memory storage/retrieval during nonword repetition, and motor planning and programming during complex real word repetition. To further investigate the role of short-term memory, a participant with short-

Decoding of auditory cortex signals with a LAMSTAR neural network.

OBJECTIVES: Each neuron has a specific set of stimuli, which it preferentially responds to (the receptive field of the neuron). For implantable cortical prosthetic devices specific points of the cortex (or groups of neurons) have to be stimulated to create perceptions of sensory stimulus with specific attributes (such as frequency, temporal characteristics, etc). Such applications would need real time decoding of signals. Previously mathematical techniques, such as computing the receptive field (using electrophysiology data) and artificial neural networks (Kohonen network or SOM and back propa

Newborn screening for Pompe disease: synthesis of the evidence and development of screening recommendations.

BACKGROUND: Pompe disease is a lysosomal storage disorder that leads to the accumulation of glycogen and subsequently to muscle weakness, organ damage, and death. Pompe disease is detectable through newborn screening, and treatment has become available recently. OBJECTIVE: Our goal was to review systematically all available evidence regarding screening for infantile Pompe disease to help policy makers determine whether Pompe disease should be added to their state's newborn screening battery. METHODS: We searched online databases, including Medline, clinicaltrials.gov, and the Computer Retrie

Minor head injury: pathophysiological or iatrogenic sequelae?

This study addresses the possibility that cognitive sequelae--albeit of a transient or minor character--can be associated with mild head injury. Twenty men (aged 16-30 years of age), whose post-traumatic amnesia did not exceed eight hours, were examined within 48 hours of their accident and again one month later. This unselected sample had no previous history of head injury. A control group of 20 men of similar socioeconomic background, was selected from medical wards (where they had been admitted for orthopaedic treatment or a minor operation). They were also retested one month after the firs
