Foslevodopa/Foscarbidopa: A Review in Advanced Parkinson's Disease.

Foslevodopa/foscarbidopa [PRODUODOPA® (EU); VYALEV™ (USA, Canada, Japan)] is a soluble formulation of levodopa and carbidopa prodrugs for 24-h continuous subcutaneous (SC) infusion. It is approved for the treatment of motor fluctuations in patients with advanced Parkinson's disease (PD). Administered via an ambulatory infusion pump, it allows for personalized dosing based on individual needs. In a randomized, double-blind, double-dummy trial, continuous SC infusion of foslevodopa/foscarbidopa provided a significant and clinically meaningful increase in hours of 'on' time without troublesome dy

Patient profile, indications, efficacy and safety of duodenal levodopa infusion in advanced Parkinson's disease.

The studies of duodenal infusion of a levodopa on small groups of parkinsonian patients have reported beneficial effects on motor complications. However, little is known about the patient profile and indications for duodenal levodopa infusion. The purpose of this study is to exhaustively investigate the clinical characteristics of the population and indication, efficacy and tolerability of duodenal levodopa infusion in natural care settings. Of the 102 patients treated with duodenal levodopa infusion since 2003, 91 were enrolled in a multicentre retrospective study. The mean age was 72.7 years

Preliminary in vivo study of an intra-aortic impeller pump driven by an extracorporeal whirling magnet.

To achieve the aim of long-term heart-assist with a simple implantable device, we have been trying to develop a minimal intra-aortic impeller blood pump driven by an extracorporeal magnetic device. The purpose of the current study was to evaluate its feasibility by acute in vivo animal tests. The minimal intra-aortic pump was a cage-supported rotor-impeller, 17 mm in diameter with a total length of 30 mm. The driving magnet, mounted extracorporeally, was 55 mm in diameter and 50 mm in length. Seventeen dogs weighing from 28-34 kg were used in the study. After thoracic incision, heparin (50 U/k

Influence of pigment epithelium-derived factor on outcome after striatal cerebral ischemia in the mouse.

We here suggest that pigment epithelium-derived factor (PEDF) does not have an effect on lesion size, behavioral outcome, cell proliferation, or cell death after striatal ischemia in the mouse. PEDF is a neurotrophic factor with neuroprotective, antiangiogenic, and antipermeability effects. It influences self-renewal of neural stem cells and proliferation of microglia. We investigated whether intraventricular infusion of PEDF reduces infarct size and cell death, ameliorates behavioral outcome, and influences cell proliferation in the one-hour middle cerebral artery occlusion (MCAO) mouse model

Efficacy of metabolic support with glucose-insulin-potassium for left ventricular pump failure after aortocoronary bypass surgery.

Refractory pump failure after cardiopulmonary bypass carries a high mortality. To assess the effectiveness of metabolic support for the heart in patients with refractory heart failure after hypothermic ischemic arrest for aortocoronary bypass surgery, we randomly assigned 22 patients to receive either intravenous glucose (50%), insulin (80 units/I), and potassium (100 meq/l) (GIK) infused at a rate of 1 ml/kg/hr for up to 48 hours or glucose (5%) and NaCl (0.225%) infused at the same rate (control). All patients required inotropic drug support, received intra-aortic balloon pump assistance, an

Regional chemotherapy for hepatic metastases of colorectal carcinoma (continuous intraarterial versus continuous intraarterial/intravenous therapy). Results of a controlled clinical trial.

Sixty-four patients with a biopsy diagnosis of colorectal cancer with liver metastases were treated with 5-fluorodeoxyuridine (FUDR) infusions. In a pilot study, the first 20 patients were given hepatic artery infusions of FUDR by implanted pumps. The remaining 44 patients were then randomized prospectively to compare the effectiveness of continuous hepatic artery and intravenous infusion of FUDR (IA/IV group; 21 patients) with hepatic artery infusion alone (IA group; 23 patients). A continuous 14-day infusion regimen of FUDR was applied each month. The dosage was 0.2 mg/kg/d of FUDR for the I

Treatment of patients with advanced malignant lymphoma using gallium nitrate administered as a seven-day continuous infusion.

Previous trials of gallium nitrate (NSC-15200) showed that bolus administration produced dose-limiting nephrotoxicity without substantial an … e treatment for patients with advanced malignant lymphoma. Outpatient administration using portable infusion pumps is safe and practical. Further evaluation of the drug administered as a constant infusion is indicated in patients with other neoplastic diseases.

Diabetes mellitus.

Diabetes mellitus is classified into two major categories: type I, which is insulin dependent, and type II, which is not. Insulin resistance in type II diabetes may be related to impaired receptor binding in some forms of the disorder. In the past, diabetes in pregnant women resulted in high rates of maternal and infant mortality. During the past 10 years, however, better management of maternal diabetes has led to a significant sharp reduction in maternal and fetal morbidity and mortality. The long-term outcome of insulin-dependent diabetes remains gloomy, probably because adequate control of

Unrecognised drug error and subsequent airway management utilising ultrasound-guided cricothyroidotomy and Rapid-O2® oxygen insufflation.

Tubeless microlaryngoscopy optimises surgical access but typically relies on total intravenous anaesthesia, commonly using propofol and remifentanil infusions. We present a difficult airway case where an unrecognised drug error during programming of an infusion pump resulted in unexpected apnoea. Open airway surgery proceeded with the use of a prophylactic cannula cricothyroidotomy using a Rapid-O2® insufflation device to provide rescue oxygenation. Furthermore, cricothyroid membrane identification failed with digital palpation but was successful with ultrasonography. While the latter is curre

[MgSO4 therapy in severe pre-eclampsia/eclampsia].

Corresponding to the newer literature and based on good own experiences lasting for four decades magnesium sulfate is put out as anticonvulsive drug of first choice in cases of severe preeclampsia and eclampsia. Especially the dosage is explained in connection with the classic schemes of Pritchard and Zuspan, in which a high concentration of magnesium in serum situated in therapeutic range is aspired to without additional sedatives. As standard dose initially 4 g MgSO4, followed by 2 g/h intravenously (in severe cases in the first hours 3 g/h) for continuing serum level applied by infusion pum

Perinatal pulmonary responses to arachidonic acid during normoxia and hypoxia.

Dilator prostaglandins are released from the perinatal lung in response to ventilation and also may be involved in the pressor response to hypoxia. However, arachidonic acid, precursor of bisenoic prostaglandins, causes pulmonary vasoconstriction when infused into the pulmonary circulation of perinatal goats. The effects of hypoxia on the arachidonic acid-induced increase in pulmonary vascular resistance (PVR) were evaluated in ventilated fetal and neonatal lambs using an in situ pump-perfused lower left lobe preparation. These studies indicate that pulmonary vascular effects in newborns due t

The effect of different flushing and locking techniques on catheter occlusion rates in central venous catheters: protocol for a multicentre, randomized controlled, parallel-group, open-label, superiority clinical trial.

BACKGROUND: Maintaining venous access is of great clinical importance. Running a slow continuous infusion to keep the vein open (KVO) is often used in peripheral intravenous catheters (PIVCs). Previous studies have compared the effects of intermittent flushing and continuous infusion via peripherally inserted central catheters (PICCs). In this study, we applied KVO to central venous catheters (CVCs) and compared the occlusion rate of this technique with that of the intermittent flushing technique. METHOD: This is a randomized controlled trial of 14 hospitals in China. A total of 250 patients

Ultradian hydrocortisone replacement alters neuronal processing, emotional ambiguity, affect and fatigue in adrenal insufficiency: The PULSES trial.

BACKGROUND: Primary adrenal insufficiency (PAI) mortality and morbidity remain unacceptably high, possibly arising as glucocorticoid replacement does not replicate natural physiology. A pulsatile subcutaneous pump can closely replicate cortisol's circadian and ultradian rhythm. Polyacrylamide-based hydrogel coatings improve biocompatibility of implanted pump devices.

The introduction of transcutaneous and subcutaneous implants and devices into the human body instigates fouling and foreign body responses (FBRs) that limit their functional lifetimes. Polymer coatings are a promising solution to improve the biocompatibility of such implants, with potential to enhance in vivo device performance and prolong device lifetime. Here we sought to develop novel materials for use as coatings on subcutaneously implanted devices to reduce the FBR and local tissue inflammation in comparison to gold standard materials such as poly(ethylene glycol) and polyzwitterions. We

[Pumps and ports for analgesic therapy.].

At one of the symposia at the 16th congress of the German Society for the Study of Pain in Berlin in 1991 an update on the use of pumps and ports in pain treatment was presented. This article tries to focus on some of the conclusions of this meeding. (1) To avoid neurologial damage only analgesic substances that have been tested in animals and with which we have adequate clinical experience, e.g. opiates, clonidine and baclofen, should be used for spinal anaesthesia. (2) The increasing number of manufactures of pumps, ports, catheters and puncture needles should pay more attention to safety an
