Intestinal ileus as a possible cause of hypobicarbonatemia.

The possible occurrence of metabolic acidosis in patients with intestinal ileus is not well recognized. We describe a patient with acute alcohol-induced pancreatitis and a large transverse colon ileus in which plasma bicarbonate dropped rapidly in the absence of an increase in the plasma anion gap. The urinary anion gap and ammonium excretion were consistent with an appropriate renal response to metabolic acidosis and against the possibility of respiratory alkalosis. The cause of the falling plasma bicarbonate was ascribed to intestinal bicarbonate sequestration owing to the enhancement of chl

Chronic intestinal pseudoobstruction and ophthalmoplegia in a patient with mitochondrial myopathy.

A 38 year old woman having chronic intestinal pseudoobstruction associated with mitochondrial myopathy is reported. The clinical and radiographic features suggested the diagnosis of chronic intestinal pseudoobstruction. Muscular atrophy and ophthalmoplegia led to muscle biopsy, which disclosed accumulation of normal and abnormal mitochondria ('ragged red fibres'), characteristic of mitochondrial myopathy.

Familial visceral myopathy. Evidence of diffuse involvement of intestinal smooth muscle.

We report the histologic and ultrastructural findings on two sisters with familial visceral myopathy who presented with acquired megacolon that necessitated subtotal colectomy. Both patients were mentally retarded and had repeated episodes of constipation and fecal impaction. Each presented near the age of 30 with massive dilatation of the colon and without clinical evidence of small intestinal involvement. Histologic abnormalities primarily involved smooth muscle and included marked nuclear enlargement and irregularity, interstitial fibrosis, and cytoplasmic vacuolation. These changes were mo

Vasculitis in a patient receiving naproxen.

A case is described of a 44-years-old woman who developed cutaneous vasculitis, nephritis and paralytic ileus after three years' treatment with naproxen. All these features were thought to be attributable to a naproxen induced vasculitis and all resolved spontaneously after the drug was stopped.

[Clinical aspects and pathology of acute and chronic pseudoobstruction of the colon].

The pseudoobstruction corresponds to the condition of an extreme colonic dilatation with possible wall perforation without concrete evidence of a real block. Acute, reversible and chronic types are distinguished. On two examples, clinic and pathology (through autopsy) are extensively described and discussed with literature. The highest risk in acute pseudoobstruction is a wall perforation with stercoral peritonitis. This is mostly fatal. When diagnosed in time, trials of decompression are indicated. The acute pseudoobstruction is mostly observed in traumatic and septic conditions, but also wit

[German Society of Gastroenterology, Digestive and Metabolic Diseases (DGVS) position statement on endoscopic decompression in acute Ileus].

In patients with ileus with dilated intestine in imaging studies, endoscopic decompression appears a feasible option. However, its use is often uncritical and without scientific evidence. Before considering endoscopic intervention, CT-imaging should differentiate between mechanical obstruction and paralytic ileus/intestinal pseudo-obstruction. Tumor diagnosis and localisation are essential because the latter determines the choice of the decompression procedure. Coecal dilatation of more than 12 cm indicates an increased risk of perforation. In patients with toxic megacolon, dilation of the tra…

Imaging of megacystis-microcolon-intestinal hypoperistalsis syndrome before, during, and after the neonatal period: a pictorial review.

Megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS), also known as Berdon syndrome, is a rare genetic congenital disorder of impaired smooth muscle contractility, resulting in functional obstruction of the bladder and bowel. Historically associated with a poor prognosis, recent advances in the use of total parenteral nutrition (TPN), intestinal rehabilitation, and multi-visceral transplantation have led to improvements in survival in patients with MMIHS, with patients now living into the second decade of life. The radiologist plays a key role in the initial workup of these patien

Paralytic Ileus in a 45-Year-Old Female with Schizophrenia on Clozapine Therapy.

A 45-year-old woman who comes with complains of recurrent paradoxical loose stools and abdominal distension for 3 days. She had type II diabetes, schizophrenia, and a history of left below-knee amputation. While there was no indication of a mechanical blockage, imaging tests revealed paralytic ileus. A potentially fatal gastrointestinal side effect that is frequently associated with anticholinergic drugs, such as clozapine, is paralytic ileus. Full recovery was achieved with conservative treatment and a reduction in the dosage of clozapine. This instance highlights how important it is to ident

Study of the predictive factors affecting outcomes of patients undergoing transperitoneal laparoscopic donor nephrectomy.

PURPOSE: We aim to study the preoperative and intraoperative factors and compare against specific outcomes in patients undergoing transperitoneal laparoscopic donor nephrectomy and see if we could find what were the predictive factors for these outcomes. METHODS: This is a prospective cohort study done in a single high-volume transplant center. 153 kidney donors were evaluated over a period of 1 year. The preoperative factors such as age, gender, smoking status, obesity, visceral obesity, perinephric fat thickness, number of vessels, anatomic abnormalities, comorbidities, and side of kidney a

Clinical Observation of Treatment Efficacy in Critical Paralytic Ileus Disease with Integrated Traditional Chinese and Western Medicine: A Randomized Controlled Trial.

OBJECTIVE: The study aimed to evaluate the treatment efficacy of the combination of Da-Cheng-Qi Decoction (DCQD) injected into the jejunum and as an enema in patients with critical diseases with paralytic ileus. METHODS: In our double-blind randomized controlled study, 114 critically ill patients with paralytic ileus were divided into 2 groups. The control group received conventional medical treatment, and the DCQD group was treated with integrated traditional Chinese medicine (TCM) and Western medicine. The intra-abdominal pressure (IAP), recovery of gastrointestinal (GI) function, clinical

Megacystis microcolon intestinal hypoperistalsis syndrome.

Megacystis microcolon intestinal hypoperistalsis syndrome (MMIHS) is a multisystemic disorder in which impaired intestinal motor activity causes recurrent symptoms of intestinal obstruction in the absence of mechanical occlusion, associated with bladder distention without distal obstruction of the urinary tract. MMIHS and prune belly syndrome may overlap in most of the clinical features and discrimination of these two entities is important because the prognosis, management and consulting with parents are completely different. MMIHS outcome is very poor and in this article we present two neonat
