Acute intestinal obstruction comprises a vast clinical entity characterized by the items of the obstruction syndrome. It encompasses numerous etiologies, and in Western countries adhesions are currently the main cause. The work up of this urgent surgical condition must above all be clinical, even though new techniques will probably improve their diagnosis and treatment (CT scan, stent). These new technologies still have to be documented and validated. Likewise, the laparoscopic approach of acute intestinal obstruction has to be prudent. The overall mortality of this urgent condition is still h

There were 468 patients (58% females and 42% males) operated for mechanical bowel obstruction over the period of 13 years, i.e. between 1987 and 1999 included into this study. In 82.3% of these patients the obstruction involved the small intestine; in this group 5.1% had multi-level obstruction related to massive carcinomatous dissemination. The remaining 17.7% of the patients had colonic obstruction. The most common cause of small bowel obstruction was intestinal strangulation (N = 352). Two thirds of those patients had strangulated hernias, and one-third--obstruction due to adhesions. In the

BACKGROUND: Intestinal obstruction is one of the leading causes of admission to emergency wards around the world, and its etiology has changed over the past century.
AIM: The goal of this study was to ascertain the causes of intestinal obstruction at our Institution and compare the results with other reports of Mexican, U.S., and Canadian hospitals.
METHODS: Retrospective review of a cohort of patients with intestinal obstruction operated on between 1985 and 2000. Demographic data and operative findings were obtained.
RESULTS: Our cohort included 452 patients, 55.3% were women; mean age for

Intestinal malabsorption in the child usually manifests by chronic diarrhoea and (or) deficiency of growth and weight gain. Given the wide diversity of causes, diagnosis is primarily based on history-taking and clinical examination, which subsequently directs investigations. Stool examination was long the reference to confirm malabsorption and to determine its mechanisms: impaired digestion (external pancreatic insufficiency), malabsorption (intolerance to cow's milk proteins or to gluten), fermentation (intolerance to primary or secondary sugars). Presently, however, the diagnosis is usually

The past few years have witnessed a considerable shift in the clinical status of intestinal transplantation. A great deal of experience has been gained at the most active centers, and results comparable with those reported at a similar stage in the development of other solid-organ graft programs are now being achieved by these highly proficient transplant teams. Rejection and its inevitable associate, sepsis, remain ubiquitous, and new immunosuppressant regimes are urgently needed; some may already be on the near horizon. The recent success of isolated intestinal grafts, together with the mort

Delusions of parasitosis, though uncommon, are an important cause of distress for affected patients and frequently of frustration for their physicians. They occur primarily in middle-aged or older women, who have the delusional belief that they are infested with parasites. Although the vast majority of cases involve dermatologic manifestations, some patients may have delusions of intestinal infection, as illustrated by this case.

This article reviews both clinical and scientific advances in surgery of the small intestine that have been reported in the last year. The management of both pediatric and adult intussusception is considered. Multiple studies on the evolving role of ileal pouch-anal anastomosis are assessed. The treatment and epidemiology of a wide variety of intestinal neoplasms are reviewed. Advances in small bowel transplantation are also reported. The cause of small bowel obstruction is considered as well as new strategies to prevent adhesion formation. Finally, a number of diverse topics relating to intes

This article provides an overview of advances in understanding the cellular and molecular mechanisms and regulation of intestinal absorption processes of vitamins. The vitamins covered are the water-soluble vitamins folic acid, cobalamin (vitamin B12), biotin, pantothenic acid, and thiamine (vitamin B1) and the lipid-soluble vitamin A. For folate, significant advances have been made in regard to i) digestion of dietary folate polyglutamates to folate monoglutamates by the cloning of the responsible enzyme; ii) identification of the cDNA responsible for the intestinal folate transporter; iii) d

Zinc is an abundant trace element in the human body that is essential for growth and development and immune function. It is important for the formation of biomembranes and zinc finger motifs found in DNA transcription factors and has catalytic function in metalloenzymes. The intestine is the site of zinc absorption and the major route of zinc excretion. Dietary inadequacy or conditions that decrease zinc absorption or increase its losses from the gastrointestinal tract, urine, or skin may quickly cause zinc deficiency due to the limited availability of rapidly exchangeable zinc pools in the bo

Acromegalic subjects show increased frequency of neoplastic lesions in the colon and rectum with respect to the general population. Recent prospective studies using colonoscopy have shown a 3 time higher prevalence of intestinal polyps and up to 4 time increased presence of colorectal cancer in acromegaly, independently of sex, age, duration of disease and clinical status of the patients. The polyps are distributed throughout the extension of the large bowel and are often multiple, showing at least two different histologic types: hyperplastic and adenomatous. Sometimes they are associated with

Endoscopy is not yet a practical procedure for examining the small intestine. A variety of radiological techniques are available for investigating suspected small intestinal disorders. Plain radiographs are the initial procedure in patients who present acutely, and barium studies are mostly used in the patients who present less acutely. Enteroclysis (small bowel enema) is proving to be an accurate barium technique. Computed tomography (CT) is useful in certain cases, particularly in established intestinal obstruction, while angiography and radionuclide studies can be extremely useful in suspec

Intestinal neuronal dysplasia (IND) is a disorder of the enteric nervous system that was first described by Meier-Ruge in 1971. IND may be associated with Hirschsprung's disease or may occur as an isolated disorder. The incidence of isolated IND varies from 0.3% to 40% of suction rectal biopsies. The uncertainty regarding the incidence of IND has resulted from considerable confusion concerning essential diagnostic criteria. The diagnostic difficulties reflect the wide variability in the literature in terms of diagnostic criteria, biopsy procedures, staining techniques, and patient age. The rec

Small bowel tumors (SBT) are rare neoplasms and represent less than 10% of all gastrointestinal tumors. The majority of them are benign and discovered at the time of autopsy. However of those who present symptoms the majority belong to the group of malignant tumors and require of treatment. The most common histological variety are the adenocarcinoma and the carcinoid tumors. Abdominal pain, intestinal obstruction or gastrointestinal bleeding are the most common clinical complaints. Endoscopy or contrast X-ray examination are the most common forms of diagnosis and surgery remain the best way of

The incidence of lipomas of the colon is difficult to evaluate due to lack of symptoms. They are often discovered by chance during endoscopic or radiological examination and sometimes display only obstructive symptoms. A barium enema may be useful for the preoperative diagnosis of lipoma of the colon. The difficulty of obtaining a clear preoperative diagnosis influences the type of surgical treatment undertaken. Thus, a resection is often preferred to the simple removal of the lipoma by colotomy or endoscopy.

UNLABELLED: Intestinal permeability can be assessed non-invasively using the lactulose-rhamnose (L-R) test, which is a reliable measure of small intestinal integrity.
AIMS: To determine risk factors for abnormal intestinal permeability in kwashiorkor, and to measure changes in L-R ratios with inpatient rehabilitation.
DESIGN: A case-control study of 149 kwashiorkor cases and 45 hospital controls. The L-R test was adapted to study kwashiorkor in Malawi, with testing at weekly intervals during nutritional rehabilitation. Urine sugars were measured by thin layer chromatography in London.
