MECHANICAL LEFT VENTRICULAR UNLOADING IN CARDIOGENIC SHOCK TREATED WITH VENOARTERIAL EXTRACORPOREAL MEMBRANE OXYGENATION: A SYSTEMATIC REVIEW AND META-ANALYSIS.

Objective : To evaluate if mechanical left ventricular unloading could reduce mortality in patients with cardiogenic shock undergoing venoarterial extracorporeal membrane oxygenation (VA-ECMO). Methods : We searched MEDLINE, Embase, and the Cochrane Library for randomized controlled trials and propensity score-matched studies published until December 20, 2023. The primary outcome was mortality at the longest follow-up. We used a Mantel-Haenszel random effects meta-analysis and reported the pooled results with a risk ratio (RR) and 95% confidence interval (CI). The review protocol was registere

Routine off-pump coronary artery bypass grafting is safe and feasible in high-risk patients with left main disease.

BACKGROUND: Coronary artery bypass graft surgery (CABG) remains the method of choice for patients with left main disease (LMD). The precise role of off-pump coronary artery bypass graft surgery (OPCABG) remains unclear in this setting. We report the safety and feasibility of a routine OPCABG approach to patients with LMD. METHODS: From 2002 to 2007, 983 patients underwent myocardial revascularization at our institution. We compared 343 OPCABG patients with LMD (group A) to 640 OPCABG patients without LMD (group B). The relationship between the presence of LMD and outcome in OPCABG procedures

Successful use of extracorporeal membrane oxygenation and drotrecogin alpha in patients with acute life-threatening myocarditis.

Acute fulminant myocarditis may present with cardiogenic shock refractory to inotropics and intra-aortic balloon pumping (IABP). Benefit of extracorporeal membrane oxygenation (ECMO) support has been established. The effectiveness of combination with ECMO or IABP and activated protein C (drotrecogin alpha; Xigris) in treatment has yet to be defined. Four patients presented with congestive heart failure 3-4 days after flu-like symptoms. Chest roentgenograms showed cardiomegaly and bilateral pulmonary infiltrates. Two-dimensional echocardiograms demonstrated severe myocardial dysfunction with le…

[Combined aortic and mitral valve replacement with or without preservation of posterior mitral apparatus].

Seventy patients who underwent aortic and mitral valve replacement were reviewed. 37 patients had the mitral valve replaced with preservation of posterior mitral apparatus (AVR and modified MVR; modified group), and the others without (AVR and conventional MVR; conventional group). Hospital deaths occurred in 4 patients (12.1%) in conventional group and in only 1 patient (2.7%) in modified group. Four out of these five hospital deaths were due to low output syndrome (LOS). The other one died of unknown cause. Severe LOS, necessitating intra-aortic balloon pumping, occurred in 4 (12.1%) in conv…

Future Directions in Temporary Mechanical Circulatory Support for Cardiogenic Shock: Novel Devices and Evolving Therapeutic Paradigms.

Temporary mechanical circulatory support (tMCS) is the mainstay of therapy for advanced cardiogenic shock (CS). Recent trial data have shown the benefit of microaxial flow pumps in selected cases of acute myocardial infarction with CS; however, inconsistencies exist in the historical data and rates of vascular complications remain unacceptably high. Novel form factors, device miniaturization, new mechanisms of pumping, and design features aimed at mitigating complications, have all become focal points of device innovation. Reliability, ability to provide high levels of circulatory support, dur

Heparin-induced thrombocytopenia in patients with mechanical circulatory support: a systematic review and meta-analysis.

BACKGROUND: Mechanical circulatory support (MCS) is increasingly being used for providing hemodynamic support in patients with advanced cardiogenic shock. Despite its benefits, it continues to carry a significantly high risk of multiple complications. The use of anticoagulation is crucial to prevent device-related and patient-related thrombosis in MCS-treated patients. Heparin use is complicated by the risk of heparin-induced thrombocytopenia (HIT). OBJECTIVES: We sought to describe the incidence and outcomes of HIT in patients with MCS. METHODS: We performed a systematic search in Embase an

Time to rethink the prioritization of IABP patients in heart allocation systems worldwide?

The intra-aortic balloon pump (IABP) remains widely used in clinical practice despite increasing evidence questioning its efficacy and safety. Designed as a simple temporary mechanical circulatory support device, IABP might offer benefits such as afterload reduction, cardiac output enhancement, and coronary perfusion improvement. However, large randomized controlled trials such as IABP-SHOCK II and CRISP MI have failed to demonstrate a survival benefit in acute myocardial infarction related cardiogenic shock. The ALTSHOCK2 study recently failed to show a clinical advantage of IABP-therapy in p

Escalating and De-escalating Temporary Mechanical Circulatory Support in Cardiogenic Shock: A Scientific Statement From the American Heart Association.

The use of temporary mechanical circulatory support in cardiogenic shock has increased dramatically despite a lack of randomized controlled trials or evidence guiding clinical decision-making. Recommendations from professional societies on temporary mechanical circulatory support escalation and de-escalation are limited. This scientific statement provides pragmatic suggestions on temporary mechanical circulatory support device selection, escalation, and weaning strategies in patients with common cardiogenic shock causes such as acute decompensated heart failure and acute myocardial infarction.

Mechanical Left Ventricular Unloading in Patients Undergoing Venoarterial Extracorporeal Membrane Oxygenation.

BACKGROUND: Venoarterial extracorporeal membrane oxygenation (VA-ECMO) increases left ventricular (LV) afterload, potentially provoking LV distention and impairing recovery. LV mechanical unloading (MU) with intra-aortic balloon pump (IABP) or percutaneous ventricular assist device (pVAD) can prevent LV distension, potentially at the risk of more complications, and net clinical benefit remains uncertain. OBJECTIVES: This study aims to determine the association between MU and outcomes for patients undergoing VA-ECMO. METHODS: The authors queried the Extracorporeal Life Support Organization re

Percutaneous mechanical devices for supporting the left ventricular failure.

This article reviews the indications, contraindications, functionality, and complications for various percutaneous devices that can be used to support the left ventricular failure. We also reviews the anesthetic effect for these devices. A literature review was performed using PubMed. When the heart experiences end-stage systolic ventricular failure, it is generally unable to restore its practical function with pharmacological therapy alone. Percutaneous ventricular support devices have been introduced and used successfully to support a failing ventricle in a variety of settings. These devices
