Report on the development of a procedure to prevent placement of feeding tubes into the lungs using end-tidal CO2 measurements.

OBJECTIVE: To determine the accuracy of a technique using capnography to prevent inadvertent placement of small-bore feeding tubes and Salem sump tubes into the lungs. SETTING: Twelve-bed medical intensive care unit (MICU) in a 557-bed teaching hospital. PATIENTS: A total of 25 ventilated adult MICU patients were studied-5 in phase 1 and 20 in phase 2. DESIGN: Phase 1 tested the ability of the end-tidal CO2 (ETCO2) monitor to detect flow (and thus accurately detect CO2) through small-bore feeding tubes. A small-bore feeding tube, with stylet in place, was placed 5 cm through the top of the

Transnasal versus transoral endoscopy for the placement of nasoenteral feeding tubes in critically ill patients.

BACKGROUND: Nasoenteral feeding tube placement with the Seldinger technique using transoral endoscopy is a tedious procedure. This study compared the transoral approach with a new technique that uses a transnasal endoscope without the need for a mouth-to-nose wire transfer. METHODS: Critically ill patients requiring nasoenteral feeding tube placement were randomly assigned to the transoral technique using a standard upper endoscope (n = 80) or the transnasal method using a 5.3 mm fiberscope (n = 80). Procedure time, medication requirement, technical difficulty, patient tolerance, and radiolog

Inadvertent intracranial insertion of a nasogastric tube in a non-trauma patient.

Complications following nasogastric intubation in patients with basal skull fractures are well documented. This report is of a rare cause of inadvertent intracranial placement of a nasogastric (NG) tube in a non-trauma patient. The patient subsequently died. The use of NG tubes, their place in airway management, and lessons to be learned from this case are discussed.

Transnasal insertion of percutaneous endoscopic gastrostomy in a patient with intermaxillary fixation: case report.

We describe successful placement of a percutaneous endoscopic gastrostomy via the transnasal approach in a patient who required intermaxillary fixation for an open mandible fracture, and required enteral nutrition for chronic respiratory failure and traumatic brain injury. This method may be useful in other cases where the transoral route is not available for endoscopic insertion of enterostomy tubes.

Creation of compression gastroenterostomy by means of the oral, percutaneous, or surgical introduction of magnets: feasibility study in swine.

PURPOSE: The use of magnets placed surgically, percutaneously, and orally to create compression gastroenteric anastomoses was evaluated in 11 swine. MATERIALS AND METHODS: Disc-shaped, jacketed rare-earth magnets with cutting edges varying in diameter from 0.250 inch (6.4 mm) to 0.500 inch (12.7 mm) were used in seven swine, and rectangular types were used in three swine. Magnets were implanted surgically in five and introduced by means of standard interventional techniques through a gastrostomy in two and perorally in four animals. Anastomoses (n = 8) were studied grossly and histologically

Retrograde percutaneous gastrostomy: a prospective study in 57 children.

We prospectively studied Retrograde Percutaneous Gastrostomy (RPG) in 57 children (age 3 weeks-17 years, 1.7-48 kg) as the feasibility and complications of this technique have not been reported in a large series of children. We used IV sedation (42), oral sedation (4 neonates), general anaesthesia or local anaesthesia only. The retrograde approach for gastrostomy or gastrojejunostomy catheter placement is described. Catheter placement was unsuccessful in only one child, due to the presence of marked hepatosplenomegaly. Catheters were successfully placed in all the other 56 children. In one of

Experience with the Merck small bowel enema tube.

We describe our experience using the Merck 10 French small bowel enema tube in 50 patients. Small bowel intubation was quick and easy to perform, and the tube was well tolerated by our patients. Kinking of the Merck tube was correctable, but prolonged the examination. The examination had to be abandoned in 6% of cases due to retrograde movement of the tube back into the stomach.

Serous otitis media in amyotrophic lateral sclerosis.

Serous otitis media (SOM) is a common disease, the cause of which is not always clear. In this paper authors report the high incidence of SOM among the patients with amyotrophic lateral sclerosis (ALS). Furthermore, a comparative study about ALS and other diseases shows higher incidence of SOM in ALS. Our study indicates that SOM in ALS seems to be mainly due to muscular disturbance of eustachian tube caused by ALS, although the influence of long term supine position and nasopharyngeal irritation by nasogastric feeding tube cannot be denied in the development of SOM.

Is routine postoperative nasogastric decompression really necessary?

Controversy exists regarding the need for nasogastric tube decompression and the incidence of complications resulting from its use following major intra-abdominal surgery. To determine the value of such tubes, 100 patients were managed after surgery with a nasogastric tube in situ until the passage of flatus per rectum (Group I). In a second group of 100 patients, no nasogastric tube was placed after surgery unless vomiting, gross distention, or overt obstruction occurred (Group II). In Group I, the nasogastric tube remained in place an average of 6 days and five patients required replacement

Abnormal course of nasogastric tube in traumatic rupture of left hemidiaphragm.

In a review of 17 surgically proven cases of acute traumatic diaphragmatic hernia, a nasogastric tube had been inserted preoperatively in eight. In six of these patients the tube followed a characteristic course, indicating the diagnosis of gastric herniation into the left hemithorax. In four cases the passage of the tubing established the diagnosis; in the other two cases the diagnosis was suggested by plain films alone, but subsequent passage of the nasogastric tube provided confirmation.

[Stenotic peptic esophagitis following gastric intubation].

In the light of five personal observations and after review of the literature, we call the attention on the possible occurrence of stenosing peptic oesophagitis after a medical act as frequent as apparently innocuous as is a nasogastric tube. However, if the tube itself may play a role in the apparition of the peptic oesophagitis, the primordial favoring element seems to be an often unknown, preexisting hiatal hernia. Prevention and precocious anti-acid treatment are of indisputable usefulness and efficacity. The surgical act, if it takes place, must not only take care of the stenosis but, mor
