To determine the incidence of renal functional abnormalities after lead poisoning, we evaluated the parameters of renal tubular function in 134 children and young adults, 8-13 years after chelation therapy for severe lead poisoning. There was no evidence of hypertension or reduced kidney function as assessed by serum creatinine (Cr) concentrations. Thirty-six workers exposed to low or moderate levels of lead at low temperature refining processes were surveyed to examine the route of lead intake. The development of implantable defibrillators with single lead systems has necessitated the need for larger transvenous electrodes. This article describes a modified Seldinger technique that can be used when the cephalic vein is too small to allow passage of these larger leads. When successful, the procedure eliminates the need for direct needle cannulation of the subclavian vein and the accompanying risk of pneumothorax.

Retrograde intussusception is an extremely rare entity. The authors report a case of retrograde jejuno-jejunal intussusception in a 12 year old. The features of interest are (1) the retrograde intussusception mass acting as a lead-point for a second prograde jejuno-jejunal intussusception, (2) the anastomotic suture line of a previous resection-anastomosis acting as the lead-point for the retrograde intussusception, and (3) the presence of a "reverse claw sign" (on a barium meal study) caused by the intussusceptum of the retrograde intussusception.

It has been assumed that poor oral hygiene predisposes to gingivitis with progression to periodontitis and tooth loss. However, a high prevalence of gingivitis occurs in some populations in which severe periodontitis is rare. To assess whether gingivitis is a reliable predictor of periodontitis, we compared the periodontal health of tea pickers and university students aged between 18 and 30 in Indonesia. Using ultrasonography, a caecal duplication cyst was identified as the pathological lead point in a 6 year old boy with acute intussusception. The patient underwent definitive surgery rather than inappropriate treatment by attempted radiological reduction. Various pathological lead points in intussusception can now be defined by ultrasound.

OBJECTIVES: Mortality studies of lead workers have shown excesses of nonmalignant renal disease and cerebrovascular disease. Animal studies and one human study have shown excess kidney cancer. We have updated a mortality study of male lead smelter workers (n = 1990).
METHODS: An analysis was conducted using standard life table techniques. The updated analysis added 11 years of follow-up and 363 new deaths.

Lead point lesions are very rarely suspected during ultrasound (US) investigation of intussusception. We report a case of idiopathic intussusception where US suggested a cystic lead point. At operation there was no structural abnormality but fluid was noted to be trapped within intussuscepted mesentery.
