Combined Procedure Versus Catheter Ablation Alone for Atrial Fibrillation: Predictors of Long-Term Arrhythmia Recurrence.

BACKGROUND: Early recurrence (ER) of atrial tachyarrhythmia (ATA) has been proven to predict late recurrence (LR) in patients undergoing catheter ablation (CA). However, the long-term ATA recurrence in patients undergoing combined left atrial appendage closure (LAAC) and CA procedures remains unclear. METHODS: This study aimed to investigate the long-term ATA recurrence after the combined procedure. We enrolled patients with atrial fibrillation (AF) who underwent combined procedures from January 2019 to February 2022. A 1:1 propensity score match was used to identify a comparable cohort from …

Beyond the CHA2DS2VASc: Hypercoagulable States in Anticoagulated AF Patients.

Despite the proven efficacy of direct oral anticoagulants (DOACs) in preventing thrombus formation in atrial fibrillation (AF), thrombus formation may rarely occur even with appropriate anticoagulation. We report the case of a 70-year-old woman with a history of AF, initially on warfarin, who underwent percutaneous left atrial appendage closure with a Watchman FLX 27 mm device due to recurrent gastrointestinal bleeding. She subsequently developed multiple unprovoked deep venous thromboses, necessitating the reinitiation of rivaroxaban therapy. She presented to the hospital with complaints of l…

Early Outcomes With a Next-Generation Dual-Seal Left Atrial Appendage Occluder: Results From the VERITAS Study.

BACKGROUND: Percutaneous left atrial appendage occlusion (LAAO) is an alternative stroke prevention strategy in atrial fibrillation patients deemed unsuitable to tolerate long-term oral anticoagulation. Although current LAAO devices have been shown to provide stroke protection, they have been associated with peridevice leak and pericardial effusion. The VERITAS study evaluates the safety and effectiveness of Abbott's next-generation dual-seal LAAO device. OBJECTIVES: The purpose of this study was to assess the VERITAS primary endpoints and key clinical outcomes through 45 days. METHODS: In t

Left Atrial Appendage Exclusion via Right Minithoracotomy Using an Epicardial Clip Device During Minimally Invasive Mitral Valve Surgery.

Background and Objectives: Left atrial appendage (LAA) closure is a Class I recommendation in patients with atrial fibrillation to reduce the risk of cardioembolic stroke. Achieving reliable and complete LAA exclusion during minimally invasive mitral valve surgery via right minithoracotomy remains technically challenging. We report here to our knowledge the largest series of a novel technique for LAA exclusion using an epicardial clip device applied via right minithoracotomy during minimally invasive mitral valve surgery. Materials and Methods: Between June 2023 and May 2026, 40 patients with

Patterns of antithrombotic treatment after left atrial appendage occlusion.

BACKGROUND: Left atrial appendage occlusion (LAAO) is an alternative to oral anticoagulants (OACs) for stroke prevention in atrial fibrillation (AF); however, data on post-LAAO antithrombotic patterns remain limited. OBJECTIVE: This study aimed to characterize post-LAAO antithrombotic patterns in routine clinical practice. METHODS: Patients with AF aged ≥65 years undergoing first-time LAAO were included from Optum claims data (2015-2024). OAC discontinuation was defined as an absence of refill in 60 days. Prolonged OAC use was operationally defined as an OAC refill after an imaging evaluatio

Initial Experience with Standalone Thoracoscopic Left Atrial Appendage Occlusion Using AtriClip Pro2: A Single-Center Retrospective Cohort Study in Latvia.

Background and Objectives: Left atrial appendage (LAA) closure is an established strategy for stroke prevention in selected patients with atrial fibrillation (AF), particularly when long-term oral anticoagulation (OAC) is contraindicated. Thoracoscopic epicardial LAA occlusion using the AtriClip Pro2 device provides a minimally invasive surgical approach; however, evidence for standalone use remains limited. This study aimed to describe the initial feasibility and early clinical outcomes of standalone thoracoscopic LAA occlusion using AtriClip Pro2 in Latvia. Materials and Methods: We performe

[A case report of cardioembolic stroke following a successful Maze procedure and Surgical left atrial appendage occlusion for atrial fibrillation].

A 75-year-old woman presented with dysarthria and sensory deficit of the left upper and lower limbs. Brain MRI findings revealed a cerebral infarction in the right middle cerebral artery territory. One year prior to admission, she underwent a Maze procedure and surgical left atrial appendage closure to treat her atrial fibrillation, which restored her sinus rhythm. Consequently, she discontinued anticoagulant therapy two months before admission. However, transesophageal echocardiography revealed a left atrial thrombus, which was considered the cause of the cerebral infarction. Treatment with o…

Percutaneous Management of Left Atrial Appendage Perforation: Keep Calm and Think Fast.

Left atrial appendage (LAA) perforation is a possible complication not only after release of the closure device, but also during the diagnostic phase due to sheath positioning in the LAA. We present an 83-year-old woman with permanent atrial fibrillation and high thromboembolic and bleeding risk who was admitted for elective percutaneous LAA closure. During angiographic study, she suddenly became hypotensive. Heart perforation with leakage of contrast in the pericardial space was evident and imaging confirmed cardiac tamponade. Rapid release of the closure device and pericardial evacuation all

Robotic left atrial appendage ligation: a safe strategy for a frail octogenarian with recurrent bleeding.

Atrial fibrillation remains a challenging condition to treat, especially in older patients with multiple comorbidities. Anticoagulation may lead to life-threatening bleeding and therefore might not be possible in high-risk patients. We report the case of a successful robotic-assisted left atrial appendage closure in such a patient, which proved to be an effective treatment strategy and provided a rapid recovery.

Practical Applications of Concomitant Pulmonary Vein Isolation and Left Atrial Appendix Closure Device Implantation.

Pulmonary vein isolation (PVI) using cryoballoon causes acute tissue edema of the osteal region of the pulmonary veins and the left atrium. In two cases combining PVI with an implantation of a left atrial appendage closure device led to malsizing of the device, device shouldering, and a paraprosthetic residual flow. (Level of Difficulty: Advanced.).
