Progressive multifocal leukoencephalopathy associated with elranatamab therapy in relapsed/refractory multiple myeloma.

IntroductionElranatamab, a bispecific antibody targeting B-cell maturation antigen (BCMA) and CD3, has demonstrated remarkable efficacy in relapsed or refractory multiple myeloma (RRMM). With the growing clinical use of BCMA-directed bispecific antibodies, their safety profile continues to evolve; however, progressive multifocal leukoencephalopathy (PML) has not been systematically described with elranatamab to date.Case ReportA 67-year-old woman with RRMM achieved a complete response following elranatamab therapy. After the seventh treatment cycle, she developed neurological symptoms includin

Intraoperative contrast-enhanced ultrasound features of progressive multifocal leukoencephalopathy: a case report.

We report the intraoperative ultrasound findings in a male patient with history of stage IVb mantle cell lymphoma who developed new-onset personality changes and gait instability. Brain imaging revealed an extensive right frontal white matter lesion with subtle multifocal spreading. Given the absence of contrast enhancement on MRI and considering the high-dose steroid therapy administered, an open biopsy was performed due to suspicion of cerebral lymphoma. During procedure, intraoperative ultrasound with and without microbubble contrast was carried out, demonstrating distinct echotexture featu

Polyomavirus-Specific T Cells in Progressive Multifocal Leukoencephalopathy: A Multicenter Retrospective Analysis.

Progressive multifocal leukoencephalopathy (PML) is a rare brain infection caused by JC polyomavirus (JCPyV) in immunocompromised individuals, for which no effective antiviral therapy exists. Diverse cellular therapy approaches using polyomavirus-specific T cells (PyVST) has emerged as a therapeutic option. We aimed to evaluate the real-world effectiveness of PyVST in patients with PML. We retrospectively analyzed 64 patients from 13 centers who received PyVST alongside standard care. Conditions predisposing to PML included mainly hematologic malignancies (33/64, 51.6%). One-year survival was

Distinguishing Progressive Multifocal Leukoencephalopathy From Cerebral Toxoplasmosis in HIV: A Case Report.

Progressive multifocal leukoencephalopathy and cerebral toxoplasmosis are serious opportunistic infections in patients with advanced HIV infection. Diagnosis is often challenging due to overlapping clinical and radiological features. We present the case of a 40-year-old woman with a three-week history of expressive dysphasia, headache, and right arm weakness. Imaging revealed multiple enhancing brain lesions, initially raising concern for metastatic cancer. An extensive malignancy workup was unrevealing. Lumbar puncture confirmed JC virus in the CSF, and further testing identified previously u

Molecular characterization of JC virus in progressive multifocal leukoencephalopathy cases from India.

Progressive Multifocal Leukoencephalopathy (PML) is a rare, often fatal demyelinating disease of the central nervous system caused by reactivation of the John Cunningham virus (JCV) in immunocompromised individuals. Despite an estimated 2.4 million people living with HIV in India, the reported incidence of PML remains lower than in Western countries, likely due to underdiagnosis, underreporting, and distinct host genetic and viral factors. The rising number of individuals on immunosuppressive therapies, including organ transplant recipients and those with autoimmune disorders, further emphasiz

A report of progressive multifocal leukoencephalopathy during adjuvant abemaciclib for breast cancer.

Drugs inhibiting the cyclin-dependent kinases (CDK) 4 and 6 are widely used for the treatment of luminal breast cancer, in the adjuvant and in the metastatic setting, and adverse events are well-known. Progressive multifocal leukoencephalopathy (PML) is classically linked to immunosuppression. We present a case of progressive multifocal leukoencephalopathy that developed during adjuvant abemaciclib therapy in a seventy-year-old woman diagnosed with locally advanced ductal carcinoma of the breast. Neurological impairment developed after 18 months of initiating abemaciclib and in imaging tests a

Inflammatory progressive multifocal leukoencephalopathy with human T-cell lymphotropic virus-1 coinfection.

A middle-aged man with progressive multifocal leukoencephalopathy (PML) in a human T-cell lymphotropic virus type-1 (HTLV-1) carrier on haemodialysis presented with mild dysarthria and ataxia. Brain MRI revealed asymmetric T2-hyperintense lesions in the cerebral white matter, cerebellum and brainstem. A small amount of JC virus (JCV) genome in cerebrospinal fluid was detected by PCR and cerebellar biopsy demonstrated JCV-DNA presence. Pathological findings showed demyelinating lesions and glial cells with mildly enlarged nuclei, accompanied by T-lymphocytes, neutrophils and plasma cell infiltr

Spinal cord involvement in progressive multifocal leukoencephalopathy and immune reconstitution inflammatory syndrome.

Progressive multifocal leukoencephalopathy (PML) is an opportunistic infectious demyelinating disease of the central nervous system caused by JC polyomavirus predominantly affecting immunocompromised individuals. Nowadays, HIV, hematological malignancies and iatrogenic immune suppression account for most PML cases. For unknown reasons, spinal cord is classically protected from PML lesions. Here, we report the course of a patient harboring spinal cord lesions in the context of PML with immune reconstitution inflammatory syndrome and review the eight other cases reported in the literature so far

Progressive multifocal leukoencephalopathy in patients treated with chimeric antigen receptor T cells.

Using 2 global postmarketing surveillance databases, Goldman and colleagues report that progressive multifocal leukoencephalopathy (PML), a viral disease associated with profound immunosuppression, occurs in approximately 0.9 cases per 1000 recipients of CD19-directed CAR T-cell therapy. The risk of PML appears higher with CAR T-cell therapy than other cancer therapies, but its precise role cannot be distinguished from antecedent therapies that these patients receive.

Characteristics of Progressive Multifocal Leukoencephalopathy Associated With Sarcoidosis Without Therapeutic Immune Suppression.

IMPORTANCE: Progressive multifocal leukoencephalopathy can occur in the context of systemic sarcoidosis (S-PML) in the absence of therapeutic immune suppression and can initially be mistaken for neurosarcoidosis or other complications of sarcoidosis. Earlier recognition of S-PML could lead to more effective treatment of the disease. OBJECTIVE: To describe characteristics of patients with S-PML. DESIGN, SETTING, AND PARTICIPANTS: For this case series, records from 8 academic medical centers in the United States were reviewed from 2004 to 2022. A systematic review of literature from 1955 to 20

Long-term prognosis and overall mortality in patients with progressive multifocal leukoencephalopathy.

Progressive multifocal leukoencephalopathy (PML) is a rare but fatal opportunistic infection and mainly occurs in patients with immunosuppressive conditions. Despite the increasing number of patients receiving immunosuppressive treatments, studies on PML are still lacking due to its low prevalence and incidence. We retrospectively reviewed patients diagnosed with PML in two tertiary hospitals in South Korea from 1999 to 2021. Total of 47 PML patients were included. Of 27 patients (57.4%) were diagnosed with human immunodeficiency virus (HIV). Median last follow-up modified Rankin Scale (mRS) s

Progressive multifocal leukoencephalopathy analyzed using the Japanese Adverse Drug Event Report database.

BACKGROUND: Progressive multifocal leukoencephalopathy (PML) has been reported as the development of drugs with immunomodulatory properties, such as anticancer, immunosuppressive, and biological agents, has accelerated. To clarify an incidence profile of drug-associated PML in real-world clinical practice, we analyzed reported patients with PML using the Japanese Adverse Drug Event Report (JADER) database. METHODS: We analyzed PML reports extracted from the JADER database based on the preferred term of "progressive multifocal leukoencephalopathy" from between 2004 and 2021. This was a retrosp

Pembrolizumab treatment of inflammatory progressive multifocal leukoencephalopathy: a report of two cases.

Progressive multifocal leukoencephalopathy (PML) is a rare but devastating neurological disease caused by reactivation of the JC virus in susceptible individuals. The illness has classically been associated with the human immunodeficiency virus (HIV) and multiple sclerosis (MS) patients who are treated with natalizumab. It is also associated with haematological malignancies, organ transplantation, autoimmune disease and immunodeficiency. Aside from natalizumab, a range of other immunomodulators including obinutuzumab and rituximab have been associated with PML. The nature of these associations

"Borderline" idiopathic CD4+ T-cell lymphocytopenia presenting with atypical progressive multifocal leukoencephalopathy.

Idiopathic CD4+ lymphocytopenia (ICL) is a rare disorder characterized by low counts of CD4+ cells (<300/mm3) in absence of other known causes of immunosuppression. A few cases of progressive multifocal leukoencephalopathy (PML) were reported in association with ICL with variable outcome. We describe the case of a 40 year-old man diagnosed with PML, which showed a monophasic course. Causes of primary and secondary immunodeficiency were ruled out, only a "borderline" ICL was found. This case highlights that a severe immunodepression could not be an absolute prerequisite in developing PML and al
