Using technology to improve access to specialist care in amyotrophic lateral sclerosis: A systematic review.

Our objective was to review the evidence for using technology to improve access to specialist care for patients with amyotrophic lateral sclerosis (ALS) and their carers. Medline, Google Scholar and the Cochrane library were searched for articles describing technology that enabled clinical care of patients with ALS or their carers where the patient/carer and clinician were not in the same location. Two applications were identified: telemedicine to facilitate video conferencing as an alternative to outpatient consultations and telehealth monitoring for patients with respiratory failure. One ran

High-Throughput Hit Screening Cascade to Identify Respiratory Syncytial Virus (RSV) Inhibitors.

Respiratory syncytial virus (RSV) infects 99% of children by age 2 years and is a leading cause of serious lower respiratory tract infection (LRTI) and infant hospitalization in the United Kingdom. Identification of efficacious RSV therapeutics has been hindered by the lack of a robust and appropriate primary assay for high-throughput screening (HTS). Here we report an HTS cascade that identified inhibitors of RSV replication using a robust RSV replicon luminescence-reporter assay for the primary campaign. The performance of the assay was consistent and reliable at scale, with Z' of 0.55 ± 0.0

[The Expulsion of the Jesuits (1767) and its impact on Chilean medicine in colonial times].

In the mid XVIII century medical education was started in Chile with the foundation of the University of San Felipe. However, King Charles the Third expelled the Jesuits from the Spanish Empire in 1767 resulting in clear deterioration of medical development. Jesuits concentrated the cultural elite of the country and had the best professionals and libraries. A restrictive intervention from the Spanish government led to the establishment of a "Protomedicato" whose first director was Dr Jose Antonio Rios who remained in office for 40 years. At the end of the Colonial period, only 4 latin physicia

Incidence, causes, and maternofetal outcomes of obstructed labor in Ethiopia: systematic review and meta-analysis.

BACKGROUND: Obstructed labor is a preventable obstetric complication. However, it is an important cause of maternal mortality and morbidity and of adverse outcomes for newborns in resource-limited countries in which undernutrition is common resulting in a small pelvis in which there is no easy access to functioning health facilities with a capacity to carry out operative deliveries. Therefore, this systematic review and meta-analysis aimed to estimate the incidence, causes, and maternofetal outcomes of obstructed labor among mothers who gave birth in Ethiopia. METHOD: for this review, we used

The effectiveness of intravenous (IV) to oral (PO) antibiotic switch (IVOS) interventions in managing community- and hospital-acquired pneumonia-a systematic review.

BACKGROUND: Prolonged intravenous (IV) antibiotic therapy in hospitalized patients with pneumonia increases healthcare costs and hospital length of stay compared to oral (PO) therapy. Antimicrobial stewardship (AMS) interventions promoting timely IV-to-PO antibiotic conversion may reduce healthcare burden without compromising patient outcomes. Optimal design and implementation of such interventions in different healthcare settings, including respiratory medicine remain unclear. OBJECTIVES: The aim of this study was to evaluate the effectiveness of interventions designed to improve IV-to-PO an

Safety of drug combinations based on psychoactive agents in the treatment of obesity (A reply by the authors).

BACKGROUND: Although the optimization of brain oxygenation is thought to improve the prognosis, the effect of brain tissue oxygen pressure (PbtO2) for patients with severe traumatic brain injury (STBI) remains controversial. Therefore, the present study aimed to determine whether adding PbtO2 to intracranial pressure (ICP) monitoring improves clinical outcomes for patients with STBI. METHODS: PubMed, Embase, Scopus and Cochrane Library were searched for eligible trials from their respective inception through April 10th, 2024. We included clinical trials contrasting the combined monitoring of

Integrating emergency medical services and hospital surge capacity for mass-gathering preparedness: a structured narrative review.

BACKGROUND: Mass gatherings can generate concentrated, time-dependent demand across in-event health services, municipal emergency medical services (EMS), emergency departments, and inpatient services. Evidence relevant to preparedness is heterogeneous and is often reported with inconsistent denominators, limiting direct comparison. METHODS: This structured narrative review synthesizes published evidence on EMS and hospital surge-capacity integration across the four surge domains of staff, stuff/supplies, space, and systems. The review retained the original structured search of MEDLINE/PubMed,
