Support interventions for families facing parental life-threatening illness - A scoping review.

OBJECTIVES: Despite the urgent need for support interventions for families facing parental life-threatening illness, research is limited - particularly in progressive neurological diseases. This scoping review aimed to systematically map existing interventions to inform the development of tailored support in the neurological context. METHODS: A scoping review was conducted, including articles published between 2013 and 2025, identified through searches in PubMed, CINAHL, PsycINFO, and Web of Science, along with manual screening of reference lists. Extracted data were systematically charted an

Trauma in Pregnancy: A Comprehensive Approach to the Mother and Fetus.

Trauma is a significant contributor to maternal and fetal morbidity and death in the United States. The nature of the evaluation of the pregnant trauma patient is intense and multidisciplinary. Although it invites unique opportunities for collaboration with our surgical colleagues, it also increases the risk of misunderstanding and conflict. We present in this Viewpoint a standardized fetal trauma survey that seamlessly integrates with Advanced Trauma Life Support (American College of Surgeons, Chicago, IL) in an effort to create a more reliable and collaborative experience in the trauma room.

The baby MB case: medical decision making in the context of uncertain infant suffering.

The recent MB case involved a dispute between an infant's parents and his medical team about the appropriateness of continued life support. The dispute reflected uncertainty about two key factors that inform medical decision making for seriously ill infants: both the amount of pain MB experiences and the extent of his cognitive capacities are uncertain. Uncertainty of this order makes decision making in accordance with the best-interests principle very problematic. This article addresses two of the problems that cases such as that of MB pose for those charged with making medical decisions for

The influence of physician explanations on patient preferences about future health-care states.

OBJECTIVE: To determine the influence of alternative explanations by physicians of the purpose of a medical intervention (intubation and ventilatory support, IVS) on three types of patient preferences: desire for IVS, the length of time patients would find IVS acceptable, and the minimum probability of a good medical outcome patients would require before assenting to continued support with IVS. METHODS: Structured interviews were conducted with patients followed in a continuity care general medicine clinic at the Department of Veterans Affairs Medical Center, Portland, Oregon. Patients were a

Living With Pride in Later Life: Minority Stress, Stigma, and Well-Being Among LGBTIQ+ Older Adults in Spain.

This study examines psychosocial well-being among LGBTIQ+ older adults in Spain, a population underrepresented in gerontological research. Guided by minority stress and social support frameworks, we surveyed 117 adults aged 50-78 in a community-based cross-sectional study conducted in Madrid and collected socioeconomic and relational indicators (income, housing, living arrangements, relationship status). Standardized measures assessed depression, perceived stress, life satisfaction, social support, loneliness, discrimination, and internalized stigma. Latent Profile Analysis identified four psy

A qualitative study of age-specific care needs in patients with early-onset advanced colorectal cancer.

OBJECTIVE: To investigate whether a questionnaire for Adolescent and Young Adults (AYAs), used to make an inventory of clinical care needs, can also be helpful to identify care needs of patients with early-onset colorectal cancer (eoCRC) (aged up to 50 years). METHODS: To evaluate the content and usability of the questionnaire, semi-structured interviews with eoCRC patients treated with systemic therapy were conducted until data saturation was reached. Characteristics of respondents were reported. Interviews were transcribed verbatim, and analyses were performed using grounded theory. RESULT

The impact of psychological support strategies on clinical outcomes and quality of life in patients with chronic heart disease: a randomized controlled trial.

BACKGROUND: As chronic heart disease (CHD) continues to rise globally, attention is shifting beyond medical interventions to the psychological dimensions of care. This study examined whether structured psychological support, integrated within a multidisciplinary framework, could enhance clinical outcomes, promote healthier lifestyles, and improve quality of life among CHD patients. METHODS: In a randomized controlled trial, 200 patients with CHD were allocated to either an intervention group—receiving psychological support, educational sessions, dietary counseling, and personalized physical ac

Alzheimer Dementia in the Orthopaedic Patient.

Alzheimer disease is a neurologic disorder characterized by the progressive cognitive decline. As the population continues to age, orthopaedic surgeons need to become familiar with surgical and nonsurgical treatment considerations in this complex population. Despite the advances in geriatric and dementia care, surgical and postoperative management of both elective and emergent surgery remain complex and controversial in this patient population. Appropriate perioperative and postoperative management can optimize outcomes, and the management can significantly affect the quality of life of patien

Ethical considerations in paediatric neurology: neuromuscular disease and epilepsy.

The pace of developing technology with respect to many diagnostic tests, as well as available treatments including artificial ventilation, may have progressed at a faster rate than our ethical, humane ability to decide on the optimal choices for our patients. In fact, who should make these choices; physicians or patients and families? Certain ethical aspects of neuromuscular disorders and epilepsy are reviewed. For neuromuscular disease, the example of Duchenne muscular dystrophy (DMD) with regards to genetic testing, relatively early wheelchair placement and individualised invasive ventilatio

An evidence-based approach to breastfeeding neonates at risk for hypoglycemia.

The revised standard of care for breastfeeding infants at risk of developing hypoglycemia during transitioning to extrauterine life was developed using the American Academy of Pediatrics (AAP) 2011 hypoglycemia guidelines, the Academy of Breastfeeding Medicine protocol, and staff input. A pre/postimplementation chart audit indicated support of infant safety by glucose stabilization, breastfeeding within the first hour of life, and breastfeeding frequency without an increase in blood sampling, formula use, or admissions to the special care nursery.

The multidimensional impact of malnutrition in incurable cancer.

PURPOSE OF REVIEW: The physiological consequences of malnutrition are well-established, yet the disruption to psychological well-being, identity and interpersonal relationships is less known and equally significant. This review focuses on recent multidimensional evidence on malnutrition in incurable cancer, particularly the psychological, social and relational effects. RECENT FINDINGS: Malnutrition and cachexia carry physical, psychological and relational consequences. For those with incurable cancer, this can lead to social isolation. Furthermore, it can also cause distress, guilt, anxiety a

A Review on the Management of Symptoms in Patients with Incurable Cancer.

Palliative care aims to alleviate suffering and prioritize the quality of life of patients facing serious and fatal diseases, such as cancer. Cancer patients, especially in advanced stages, often have many difficult-to-control symptoms, such as pain, fatigue, dyspnea, anxiety, and depression, requiring the attention of a multidisciplinary team highly trained in palliative care and end-of-life management. Pain, dyspnea, nausea, and vomiting are the focus of symptomatic assessment in palliative care, but patients experience other equally important symptoms that do not receive as much attention a

How do we talk to the children? Child life consultation to support the children of seriously ill adult inpatients.

BACKGROUND: Families with young children often struggle to talk about and cope with a parent's life-threatening illness and potential death. Adult interdisciplinary palliative medicine teams often feel unprepared to facilitate the open communication with these children that has been shown to reduce anxiety, depression, and other behavioral problems. In pediatric settings, child life specialists routinely provide this support to hospitalized children as well as their siblings and parents. Although these services are the standard of care in pediatrics, no research reports their use in the care o

Fathering a Child With a Progressive Life-Limiting Condition in the Context of Pediatric Palliative Care: A Qualitative Interview Study.

Fathers report feeling sidelined and being treated as secondary parents in healthcare settings. Despite their vital caregiving contributions, fathers remain underrepresented in pediatric palliative care research, particularly outside oncological contexts. This qualitative study, using a phenomenological hermeneutical design, explores the lived experiences of Norwegian fathers raising children with progressive life-limiting conditions. Thirteen fathers of children aged 1-18 years participated in in-depth, semi-structured interviews conducted online or in person. All children had received a diag

Healthcare professionals and relatives in shaping the preferred place of end-of-life care and death for patients with advanced cancer: the WhereToCare qualitative study.

BACKGROUND: Although most people in France wish to die at home, the majority of deaths still occur in hospitals. The choices of end-of-life care for patients with advanced cancer and how these preferences are shaped by interactions with relatives and healthcare professionals, remains poorly understood. This study aimed to explore how patients with advanced cancer develop and adjust these preferences, and to examine the roles of relatives, general practitioners, and oncologists in this process. METHODS: We conducted a single-centre longitudinal qualitative study at a French university hospital

Nurses' and midwives' ethical perspectives on euthanasia legalisation in advanced cancer care.

BackgroundEuthanasia remains a highly debated issue in end-of-life care because it raises ethical concerns surrounding patient autonomy, relief of suffering, professional responsibilities, and the sanctity of life. Nurses and midwives frequently face these ethical challenges in caring for patients with advanced illness, yet evidence on their perspectives within sub-Saharan African cancer care settings remains limited.AimThis study explored nurses' and midwives' ethical perspectives and perceived implications regarding the potential legalisation of euthanasia in advanced cancer care in Ghana.Re
