Forty-one consecutive cases of liver abscesses seen at the National University Hospital, Singapore from 1988 to 1994 were reviewed. Twenty-seven cases (65%) were pyogenic, six (15%) amoebic, two (5%) tuberculous and six (15%) indeterminate. The predominance of pyogenic abscesses is in marked contrast to previous studies from the region a decade ago in which amoebic abscesses were the commonest type. 27 patients who presented with pyogenic liver abscess from 1957 to 1984 are analyzed retrospectively and compared with the literature. Diagnosis and start of therapy were frequently delayed because of the unspecificity of symptoms. Symptoms included fever, abdominal pain, weakness, and loss of weight. In 50% of all patients, the abscess was found by chance at laparotomy or autopsy. 50 patients, with ages ranging from eight to 55 years, suffering from amebic liver abscess (ALA), have been studied. 90% of these patients were males. The abscess was located in the left lobe of the liver in nine cases and involved both lobes in four cases. In 17 patients, the abscess had already ruptured at the time of admission and, in four cases, it was about to rupture. In seven patients the diagnosis of ALA could only be made on laparotomy. Amebic liver abscess was seen in 35 patients over a ten-year period at Cook County and the University of Illinois hospitals, Chicago; 32 had immigrated from an area of endemic amebiasis, but three were natives of Chicago. Thirteen examples of isolated liver nodules were examined in 7 persons, 4 with single nodules and 3 with multiple nodules. Pathologically they present a spectrum from a solid structure that resembles liver parenchyma with varying degrees of disorganization and fibrosis to soft and fatty enough to superficially resemble a lipoma. There has been confusion in the literature over the nomenclature of solitary liver nodules. Several such lesions have recently been reported in patients taking oral contraceptives. Similarities exist between these cases that suggest they may be examples of focal nodular hyperplasia. Here three further cases are presented. The criteria for making the diagnosis and its importance are discussed.
