Fine-needle aspiration for diagnosis of tuberculous lymphadenitis in children in Bangui, Central African Republic.

BACKGROUND: Tuberculosis (TB) is a major cause of childhood morbidity and mortality in developing countries. One of the main difficulties is obtaining adequate specimens for bacteriological confirmation of the disease in children. The aim of this study is to evaluate the adequacy of fine-needle aspiration (FNA) for the diagnosis of TB. METHODS: In a prospective study conducted at the paediatric hospital in Bangui in 2007-2009, we used fine-needle aspiration to obtain samples for diagnosis of TB from 131 children aged 0-17 years with persistent lymphadenitis. RESULTS: Fine-needle aspiration pr…

Granulomatous typhlocolitis, lymphangitis, and lymphadenitis in a horse infected with Listeria monocytogenes, Salmonella Typhimurium, and cyathostomes.

A 15-year-old American Quarter horse mare was euthanized because of poor response to therapy for severe diarrhea. Significant gross findings were limited to the large intestines. The walls of the cecum and colon were thickened with widely scattered nodules in the mucosa and submucosa that extended into the enlarged colic lymph nodes. Microscopically, there was severe granulomatous typhlocolitis, lymphangitis, and lymphadenitis, with many intralesional Gram-positive, non-acid-fast coccobacilli and few cyathostomes. Intralesional bacteria were immunohistochemically and polymerase chain reaction …

Dermatopathic lymphadenitis with generalized erythroderma in a patient with epstein-barr virus-associated hemophagocytic lymphohistiocytosis.

Here, we describe a patient with Epstein-Barr virus (EBV)-associated hemophagocytic lymphohistiocytosis (HLH) who simultaneously presented with generalized erythroderma and dermatopathic lymphadenitis (DL). A 63-year-old Korean woman presented at our hospital with fever, hepatosplenomegaly, axillary lymphadenopathy, and generalized erythrodermic eruption. The bone marrow biopsy findings were consistent with the diagnosis of HLH, and EBV DNA was detected using the polymerase chain reaction. Based on serologic tests that indicated a primary EBV infection, the patient was diagnosed with EBV-assoc…

Fatal haemoptysis in a young man with tuberculous mediastinal lymphadenitis. A case report and review of the literature.

A young man presented with a short febrile illness associated with signs of cough, sputum and haemoptysis (>200 ml/24 h) to his local hospital. The chest radiograph revealed a small cavity in the upper zone with patchy consolidation. He was kept in isolation awaiting a bronchoscopy for tuberculosis (TB). He developed sudden, severe haemoptysis and died as a result of exsanguination within a short space of time and could not be resuscitated. Post-mortem examination revealed dehiscence of the aortic wall from necrotising tubercular lymphadenitis. This illustrates the importance of recognising th…

A clinical scoring system as useful as FNAC in the diagnosis of tuberculous lymphadenitis in HIV positive patients.

Among HIV positive patients, Fine needle aspiration cytology (FNAC) or biopsy for diagnosis of tuberculous lymphadenopathy is often avoided due to an unspoken stigma. Earlier, we had developed a clinical scoring scale for the diagnosis of tuberculous lymphadenitis (TBLN), which had 88% sensitivity and detected no false positives. In the present study, we attempted to develop similar scale that could assist in diagnosing TBLN in AIDS. All 42 HIV positive patients of adenitis attending Ramdeo Hospital and Research Centre, Jodhpur between August 2001 and December 2004 were studied. History of pas…

Correlation of fine needle aspiration cytology, smear and culture in tuberculous lymphadenitis: a prospective study.

BACKGROUND AND AIM: Bacteriological studies are necessary to confirm the diagnosis of tuberculous lymphadenitis, as cytological appearances mimic other granulomatous lesions. The objective was to assess the diagnostic role of culture of fine needle aspiration done on clinically suspected cases of tuberculous lymphadenitis and to determine the prevalence of drug resistance in M. tuberculosis isolates. SETTING AND DESIGN: A prospective, double-blind study over a period of one year in a tertiary care hospital. MATERIAL AND METHODS: Fine needle aspiration cytology and culture were done on 250 pa…

Cat scratch disease diagnosed by polymerase chain reaction in a patient with suspected tuberculous lymphadenitis.

We present a patient diagnosed as having tuberculous lymphadenitis after lymph node biopsy and referred for tuberculous therapy. On review, because of recent cat exposure, she was tested for cat scratch disease (CSD), but serological tests for Bartonella henselae were negative. However, the diagnosis of CSD was confirmed, and tuberculosis excluded, by polymerase chain reaction techniques. This case serves as a reminder that CSD may mimic tuberculosis histologically and that sensitive molecular diagnostic tests are available.

Toxic Epidermal Necrolysis-Like Acute Cutaneous Lupus Erythematosus With Histiocytic Necrotizing Lymphadenitis: A Case Report.

Toxic epidermal necrolysis-like acute cutaneous lupus erythematosus (TEN-like ACLE) is an acute, life-threatening manifestation of ACLE. Histiocytic necrotizing lymphadenitis (HNL) is a rare disease of lymph nodes that presents with fever and lymphadenopathy and is generally self-resolving. We report a case of a 48-year-old female who presented to the emergency department (ED) with fever and arthralgias in the setting of 30 lbs weight loss over two months. She had self-discharged and re-presented to the ED three weeks later with widespread, painful skin sloughing. On exam, the patient had scat…

Cervical Tuberculous Lymphadenitis Associated With New-Onset Acanthosis Nigricans Following Antitubercular Therapy: A Case Report.

Cervical tuberculous lymphadenitis (CTL), also known as scrofula, is an extrapulmonary manifestation of tuberculosis, a disease that is endemic to many developing countries, particularly Sub-Saharan Africa and Asia, but may also be found worldwide in developed countries like the United States. CTL can be difficult to detect and may mimic other similar-appearing conditions, so a high index of suspicion is required to accurately diagnose the condition when a patient presents with one or more neck masses. Incision and drainage and excisional surgery are aggressive options available to treat CTL b…

Infectious mononucleosis lymphoadenitis showing histologic findings indistinguishable from toxoplasma lymphadenitis. A report of three cases.

Lymph node lesions in infectious mononucleosis (IM) show a marked histologic diversity. We report here three cases of IM lymphadenitis with histologic findings indistinguishable from those of toxoplasmic lymphadenitis. The histologic findings of the three cases presented here showed a histologic triad of toxoplasmic lymphadenitis, including (i) numerous lymphoid follicles with hyperplastic germinal centers; (ii) small clusters or single epithelioid histiocytes; and (iii) multiple foci of monocytoid B-cells. Moreover, all three lesions contained isolated or small clusters of epithelioid histioc…

Probable atypical cat scratch disease presenting as isolated posterior pancreatic duodenal lymphadenitis and abdominal pain.

We report a case involving a 15-year-old girl with atypical, clinically unsuspected cat scratch disease (CSD) presenting as isolated posterior pancreatic duodenal lymphadenitis, fever, and abdominal pain. The serological, abdominal ultrasonographic, and CT findings, as well as clinical and epidemiological data, indicate that B. henselae was likely an etiologic agent of CSD in our patient.

Systemic lupus erythematosus with erythema multiforme-like lesions and histiocytic necrotizing lymphadenitis--a case report.

A 9-year-old Chinese girl with systemic lupus erythematosus presented initially with persistent cervical lymphadenopathy without any constitutional symptoms. The histology of the involved lymph node showed histiocytic necrotizing lymphadenitis and the initial clinical diagnosis of Kikuchi-Fujimoto disease was made. The patient subsequently developed fever, oral ulcerations, epistaxis, generalised myalgia and erythema multiforme-like skin lesions over the face and upper arms. Investigations showed anaemia, leucopenia, hypocomplementemia, a positive antinuclear antibody and a positive anti-doubl…

Recurrent histiocytic necrotizing lymphadenitis (Kikuchi's disease) in an human T lymphotropic virus type I carrier.

We describe a case of recurrent histiocytic necrotizing lymphadenitis (HNL) with aseptic meningitis. The patient was a 46-year-old male and a carrier of human T lymphotropic virus type I (HTLV-I). The patient had a past medical history of at least three relapses of HNL. In addition, his sister, who was also an HTLV-I carrier, had recurrent clinical episodes consistent with those of HNL, suggesting familial HNL occurrence. This observation suggests the possibility that HTLV-I infection is relevant to the pathogenesis of HNL.

Elevated nil values in TB-IGRA: a potential diagnostic clue for pediatric histiocytic necrotizing lymphadenitis.

BACKGROUND: Histiocytic necrotizing lymphadenitis (HNL) is a benign condition in children that is often misdiagnosed due to its clinical overlap with tuberculosis, rheumatic diseases, and lymphadenitis. The QuantiFERON-TB Gold In-Tube assay (TB-IGRA) is routinely used to rule out tuberculosis, but its Nil value (TB-Nil), which reflects background IFN-γ release, remains underutilized. Many studies suggest that elevated IFN-γ levels may aid in the differential diagnosis of HNL. In this study, we investigate the role of TB-Nil in diagnosing HNL. METHODS: To investigate the relationship between T…

Paradoxical Cervical Lymphadenitis During Anti-tubercular Therapy in an Immunocompetent Young Female: A Diagnostic Pitfall.

A paradoxical reaction (PR) in tuberculosis (TB) is defined as clinical and/or radiological worsening of pre-existing tuberculous lesions or the appearance of new lesions during appropriate anti-tubercular therapy (ATT), despite initial improvement and in the absence of treatment failure, drug resistance, poor adherence, or superinfection. Although more commonly described in HIV-infected or disseminated TB cases, PR can also occur in immunocompetent individuals and may be misinterpreted as disease progression. We report a case of a 19-year-old immunocompetent female receiving standard first-li

The Silent Sentinel's Breach: Middle Lobe Syndrome Caused by Endobronchial Rupture of Mediastinal Tuberculous Lymphadenitis.

Middle lobe syndrome (MLS) is characterized by recurrent or chronic collapse of the right middle lobe of the lung. While non-vascular inflammatory causes dominate, endobronchial rupture of mediastinal tuberculous lymphadenitis causing obstructive MLS is an under-recognized entity that presents significant diagnostic challenges. In this context, we report the case of a 24-year-old male immigrant with no past medical history who presented with cough and right-sided chest pain. Chest radiography revealed right middle lobe alveolar opacity. He received empirical levofloxacin without radiological f…

Maxillofacial Squamous Cell Carcinoma With Cervical Metastasis and Tuberculous Lymphadenitis: Diagnostic Dilemmas and Clinical Insights.

BACKGROUND: Maxillofacial squamous cell carcinoma (SCC) typically metastasizes to lymph nodes, yet coexisting tuberculous lymphadenitis is extraordinarily rare, posing diagnostic challenges. CASE: A 68-year-old female with right maxillofacial SCC and ipsilateral lymphadenopathy underwent radical resection and selective neck dissection after computed tomography had shown nodes with central necrosis and rim enhancement-features indeterminate for metastasis versus infection. Histopathological examination of the dissected lymph nodes revealed concurrent metastatic SCC foci and tuberculous granulo…
