Retrospective analysis of translaminar, demographic, and physiologic parameters in relation to papilledema severity.

OBJECTIVE: Some case reports suggest that the translaminar pressure difference is important in cases of papilledema. The purpose of this study was to determine ocular, physiologic, and demographic factors associated with papilledema severity. DESIGN: Retrospective, blinded study. PARTICIPANTS: Patients who had undergone a diagnostic lumbar puncture and had a diagnosis of papilledema in conjunction with idiopathic intracranial hypertension between 2004 and 2012 were included in the study. One-hundred and fifty-one patients were identified in initial screening. Sixty of 151 patients met all in

A phase II study on the duration and stability of the intraocular pressure-lowering effect and tolerability of Tafluprost compared with latanoprost.

PURPOSE: Tafluprost is a novel prostaglandin F(2alpha)-receptor agonist shown to lower intraocular pressure (IOP) in healthy humans and patients with elevated IOP. We investigated the efficacy, safety, and tolerability of tafluprost 0.0015% compared with latanoprost 0.005% in patients with primary open-angle glaucoma, exfoliation glaucoma, or ocular hypertension. METHODS: This was a randomized, double-masked, active-controlled, parallel-group, multinational, and multicenter phase II study. Patients received either tafluprost 0.0015% (n = 19) or latanoprost 0.005% (n = 19), both once daily. Th

Antihypertensive therapy in diabetes mellitus: efficacy, safety, and metabolic impacts in type 1 and type 2 diabetes.

Treating hypertension in diabetic patients with antihypertensive medications should be done with careful consideration because these medications may interfere with macro- and microvascular complications of diabetes, such as myocardial infarction, stroke, and diabetic nephropathy. Some medications also come with cardio- and reno-protective effects along with lowering blood pressure. This narrative review focuses on major antihypertensive drug classes and their efficacy, safety, and metabolic impacts in type 1 and type 2 diabetes mellitus. We found that most classes of drugs, such as ACEIs (Angi

In Vivo Investigation of (2-Hydroxypropyl)-β-cyclodextrin-Based Formulation of Spironolactone in Aqueous Solution for Paediatric Use.

Spironolactone (SPL), a potent anti-aldosterone steroidal drug used to treat several diseases in paediatric patients (e.g., hypertension, primary aldosteronism, Bartter's syndrome, and congestive heart failure), is not available in child-friendly dosage forms, and spironolactone liquids have been reported to be unpalatable. Aiming to enhance SPL solubility in aqueous solution and overcome palatability, herein, the effects of (2-hydroxypropyl)-β-cyclodextrin (HP-β-CyD) were thoroughly investigated on solubilisation in water and on masking the unpleasant taste of SPL in vivo. Although the comple

Efficacy of a Web App-Based Music Intervention During Cataract Surgery: A Randomized Clinical Trial.

IMPORTANCE: Hypertensive events during cataract surgery may induce complications. Information and communication technologies applied to health are popular, but clinical evidence of its usefulness is rare and limited to treating anxiety without specific analysis. Recent publications have described the potential effects of personalized music choices as mobile-based intervention on pain but not on anxiety. OBJECTIVE: To demonstrate the effects of a web app-based music intervention on the incidence of hypertension in participants during cataract surgery performed under local anesthesia. DESIGN,

Optimizing the Transition of Care for Postpartum Preeclampsia: A Scoping Review of Management Strategies and Missed Opportunities.

Hypertensive disorders of pregnancy remain a primary driver of preventable maternal morbidity, specifically during the high-risk transition from hospital discharge to long-term primary care. This scoping review maps the "transition gap" within the fourth trimester to determine why traditional management strategies often fail to protect patients at risk for late-onset postpartum preeclampsia. Following Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines, a systematic search of PubMed, the Cochrane Library, and Google Scholar w

Non-operative Management of a Diaphragmatic Hematoma: A Case Report.

Diaphragmatic hematomas are a rare complication of blunt trauma that are often overlooked due to the presence of high-acuity injuries. However, this injury can result in significant long-term complications if not managed appropriately. We present a 59-year-old female who developed a diaphragmatic crural hematoma following a motor vehicle accident. The patient arrived at the trauma bay unresponsive, hypertensive, requiring bag-valve mask ventilation, and with a Glasgow Coma Scale score of 3/15. On physical examination, the patient was found to have bilateral diminished breath sounds and was pro

Silent Hypoxia From Benzocaine-Induced Methemoglobinemia Following Transesophageal Echocardiogram.

Methemoglobinemia is a rare but potentially life-threatening cause of hypoxia, often presenting with cyanosis and low oxygen saturation unresponsive to supplemental oxygen. We report the case of a 32-year-old woman with a complex medical history including morbid obesity, asthma, type 2 diabetes mellitus, and hypertension, who was admitted with MRSA bacteremia and septic pulmonary emboli. After a nondiagnostic transthoracic echocardiogram, she underwent a transesophageal echocardiogram (TEE), during which topical benzocaine spray was administered for oro-esophageal anesthesia. Pheochromocytoma Multisystem Crisis and Masquerading Disseminated Histoplasmosis in a Neurofibromatosis Type 1 Patient With Bilateral Adrenal Tumors.

Pheochromocytomas are rare catecholamine-secreting neuroendocrine tumors that can occasionally progress to life-threatening disease, including a multisystem crisis. Patients with Neurofibromatosis type 1 (NF1) may develop pheochromocytomas, and the consequent chronic elevation of plasma catecholamine levels could further complicate various cardiovascular and pulmonary manifestations they may have. A 37-year-old African American female with NF1 presented with severe dyspnea, chills, myalgia, vomiting, and abdominal pain. Within several hours of hospital admission, she developed progressive agit
