PURPOSE: To assess the false-positive rate of breast cancer surveillance by magnetic resonance imaging (MRI) in BRCA mutation carriers and the impact of an abnormal mammography or breast MRI on the patients' decision for prophylactic mastectomy.
PATIENTS AND METHODS: A total of 196 BRCA mutation carriers were included with a median follow-up of 2 years (range 1-9) with annual mammography and MRI. Preference for prophylactic mastectomy was registered at first surveillance after the mutation carriership was revealed.
RESULTS: In all, 41% (81 of 196) of the women had at least one positive MRI o…

This study aimed at identifying complications, intercurrences and the appearance of edema following breast cancer surgery as well as factors that could predispose to arm lymphedema on the operated side. It is a descriptive and prospective study based on a quantitative approach. The subjects consisted of 17 women submitted to unilateral mastectomy in a University Hospital. Data were collected by means of a weekly post-operative follow-up of each patient for a period of three months. Edema was detected in 11 women, nine of which were in mild level and two in moderate level. It must be emphasized…

Lymphoscintigraphy allows to get intravital contrast of functionally valuable lymph collectors of the extremity. A comparative analysis was made of the state of lymph flow in healthy subjects, in patients with breast cancer and in patients with the completed cycle of treatment of breast cancer having postmastectomy edema and without it. The investigation was performed in 280 extremities. Principal variants of the normal lymph flow (collector, diffuse, nodal) were marked out. Possible compensatory mechanisms ameliorating the draining function of the extremity were followed in patients with bloc…

PURPOSE: To estimate the efficacy of contralateral prophylactic mastectomy in women with a personal and family history of breast cancer.
PATIENTS AND METHODS: We followed the course of 745 women with a first breast cancer and a family history of breast and/or ovarian cancer who underwent contralateral prophylactic mastectomy at the Mayo Clinic between 1960 and 1993. Family history information and cancer follow-up information were obtained from the medical record, a study-specific questionnaire, and telephone follow-up. Life-tables for contralateral breast cancers, which consider age at first …

A new problem has arisen for surgeons now that the pectoralis major muscle is routinely left in place after mastectomy. When the pathologist reports a tumor close to the fascial margin, there has been uncertainty regarding the significance of this finding. In the present study, the histories of 346 women with negative nodes who underwent modified radical mastectomy and had an uninvolved plane under the breast were reviewed. The distance from tumor to fascia was recorded by the pathologist, and the patients were divided into "Close" and "Not Close" groups. The "Close" group (90 patients) had tu…

Fourty-one patients underwent breast reconstruction after mastectomy for cancer and reduction plasty of the second breast, over a 36-month period. These plasties were performed only for reasons of asymmetry, in the absence of any positive preoperative finding. Histologic examination using semiserial sectioning of the whole excised sample detected a high frequency of unsuspected cancer 14/41 (34%). There was a strong predominance of in situ carcinomas 11/41 (27%) over invasive carcinomas 3/41 (7%). These results were compared with those of other bilateral cancer series. When such cancers were d…

Surgery for carcinoma of the breast is becoming less radical even to the extent of breast conservation. Subcutaneous mastectomy and implant offers itself as an alternative, retaining a breast shape and removing almost all breast tissue. Forty-four consecutive patients who had this procedure for breast carcinoma were studied with a follow-up of 5-44 months (mean 25.3 months). Only 4 patients (9 per cent) had local recurrences, i.e. recurrence in the skin flap or scar. This compares favourably with other series and other forms of treatment. Removal of the prosthesis was necessary in 10 patients …

BACKGROUND: The psychological impact of breast reconstruction following mastectomy remains poorly defined. We aimed to evaluate the association between breast reconstruction, timing and type of reconstruction with mental health outcomes.
METHODS: We conducted a retrospective cohort study using the TriNetX Research Network, including 51,422 patients who underwent mastectomy for breast cancer. Patients were stratified into mastectomy only, implant-based reconstruction, or autologous reconstruction cohorts, further subdivided into immediate versus delayed reconstruction. Mental health outcomes w…

Nipple-sparing mastectomy (NSM) continues to increase in popularity with its oncological safety demonstrated in a cohort of patients with primary malignancy and in those undergoing risk-reducing surgery. Currently, there is a paucity in literature regarding clinical outcomes of nipple areolar complex (NAC) complications in patients undergoing autologous reconstruction following NSM via hemiareolar incisions with lateral extensions. We present eccentric hemiareolar incision with lateral extension, as an evolution of the previous Omega technique that avoids medial extension. A 1-year retrospecti…

BACKGROUND: Over 14,000 UK women undergo mastectomy annually, 70% of whom do not undergo breast reconstruction. These women are often left with suboptimal scars.
AIMS: This study aimed to explore the attitudes of women towards their mastectomy scars, particular the aesthetic outcomes, and to raise awareness of the importance of achieving aesthetic flat closure.
METHODS: Semi-structured interviews with women who had undergone mastectomy without reconstruction were recruited from a single UK teaching hospital. Thematic analysis was performed using the Framework Approach.
RESULTS: Twenty women…

Breast cancer surgical management encompasses a spectrum of options that extend beyond oncologic control and carry substantially different cumulative surgical burdens. Although breast-conserving therapy (BCT) and mastectomy offer equivalent survival outcomes in many clinical scenarios, the downstream implications of these choices, including the number of operations, complication profiles, recovery timelines, and need for revision, are often underrecognized during initial treatment planning. This review aims to provide non-plastic surgeons with a practical framework for understanding the surgic…

BACKGROUND: Skin-reducing mastectomy (SRM) allows implant-based immediate breast reconstruction in women with large, ptotic breasts. This study aimed to evaluate the surgical and patient-reported outcomes (PRO) following SRM.
METHODS: A retrospective analysis was conducted on patients who underwent SRM between January 2011 and December 2014. Surgical complications, oncological outcomes, and PRO (using BREAST-Q) were collected, with a median follow-up of 10 years.
RESULTS: Fifty-five women (95 breasts) underwent SRM and completed the first round of the BREAST-Q questionnaire. 82.3% (n = 45) h…

PURPOSE: Transgender and gender diverse (TGD) individuals undergoing gender-affirming mastectomy (GAM) face unique challenges related to breast cancer risk assessment and screening. This study aimed to identify factors that influence healthcare professionals' (HCPs) integration of cancer risk evaluation, including genetic assessment, within the context of GAM and pre-surgical planning.
METHODS: We conducted semi-structured qualitative interviews with 20 healthcare professionals - including six primary care providers, five cancer genetic counselors, six oncologists, and three plastic surgeons …

Poland syndrome is a rare congenital malformation characterized by unilateral chest wall hypoplasia, ipsilateral hand abnormalities, and associations with several malignancies. The present study reports a case of a woman in her 60s with Poland syndrome who presented with infiltrating ductal carcinoma of the left breast and unilateral chest wall hypoplasia. The patient underwent mastectomy with reconstruction using a transverse rectus abdominis musculocutaneous (TRAM) flap, followed by post-mastectomy radiation therapy (PMRT). PMRT after TRAM flap reconstruction was feasible, with acceptable lu…

BACKGROUND: Parenteral nutrition associated cholestasis (PNAC) is a frequently observed pathology in extremely low birth weight (ELBW) infants. Its pathogenesis is determined by the composition and duration of parenteral nutrition (PN) as well as the tolerance of enteral feeds (EF). "Aggressive" nutrition is increasingly used in ELBW infants to improve postnatal growth. Little is known about the effect of "aggressive" nutrition on the incidence of PNAC. We analyzed the influence of implementing an "aggressive" nutritional regimen on the incidence of PNAC and growth in a cohort of ELBW infants.
