We report on a patient who developed winged scapula on the contralateral side to the approach for a cerebellopontine tumour excision, an unreported complication following this procedure. We think this was due to damage of the long thoracic nerve in the dependant side, while in the park bench position.

Aim of this study was to determine sensitivity and specificity of the mastoid vibration test in patients who had suffered an attack of vestibular neuritis. Results were compared with the caloric test and two bedside tests of vestibular function (head shaking test and head thrust test). Results are reported in 28 patients who had a residual vestibular deficit 6 months after acute neuritis and in 25 healthy subjects. Mastoid vibration nystagmus was evoked in 21 patients but not in controls. In these patients, mastoid vibration test had a sensitivity of 75% and specificity of 100%. Since one pati…

Of more than 200 patients who underwent high-resolution computed tomography (CT) of the middle ear, the vast majority had tubotympanic disease in one of its forms: middle ear effusion, tympanosclerosis, granulation tissue, tympanic membrane retractions, or acquired cholesteatoma. The CT appearance of each of these conditions is discussed and illustrated. Emphasis is placed on the differential diagnosis of tubotympanic disease by determining dependent from nondependent soft-tissue opacity using two CT projections.

OBJECTIVES: To evaluate the association between prior community-based antibiotic treatment for acute otitis media (AOM) and the incidence and severity of complications in children hospitalized with acute mastoiditis (AM), noting that this design does not address whether prior antibiotic treatment prevents AM development.
METHODS: This retrospective cohort study analyzed medical records of 522 children (age < 15 years) diagnosed with AM at a tertiary medical center between 2015 and 2025. Children were categorized by prior antibiotic exposure: those receiving antibiotics for > 48 h before hospi…

BACKGROUND: We report a case of osteosarcoma of the mastoid process.
METHODS: The tumor was identified after the patient presented with progressive hearing loss 5 years after radiotherapy treatment for parotid mucoepidermoid carcinoma. A CT scan revealed a mass invading the middle ear and mastoid process. The diagnosis of osteosarcoma was confirmed by biopsy.
RESULTS: The mass was surgically excised, and the patient was treated with postoperative cisplatin and doxorubicin. The patient also developed a postauricular abscess that was treated with incision and drainage and antibiotics. This abs…

Already in 2021 it was reported, that the COVID-19 pandemic influences the incidence of acute mastoiditis in children. The goal of this multicenter cross-regional study was to collect and analyze a substantial pre-, peri- and postpandemic data set.Absolute case frequencies were inquired from six segments: epidemiology, medical history taking, spectrum of pathogens, course of disease, therapy, and imaging. Nine ENT clinics in Germany reported about a 6 year time frame from 2018 to 2024.251 acute mastoiditis emergency cases were seen. There occured 69 pre-pandemic, 31 pandemic, and 151 post-pand…

Leukocyte adhesion deficiency type-1 is frequently associated with otorhinolaryngological manifestations, most notably a high prevalence of otitis media. Although multiple case reports have documented an increased prevalence of acute otitis media and mastoiditis in patients with LAD-1, no clinical or experimental studies have fully elucidated the pathophysiological mechanisms underlying this association. Despite guideline recommendations for urgent antibiotic therapy in immunocompromised patients with acute otitis media, a standardized treatment protocol for its complications remains lacking. …

Myoepithelial carcinoma is a rare disease of the head and neck, with only a handful of cases reported occurring within the external auditory canal, middle ear, and mastoid. The patient discussed is a 67-year-old male with a prior history of a bilateral tympanomastoidectomy for cholesteatoma and a prior history of left parotidectomy and adjuvant radiation for pleomorphic sarcoma. Three years after the parotidectomy, the patient presented with contralateral ear fullness. During an intraoperative examination, a fibrous mass was encountered, which revealed an invasive salivary gland neoplasm, myoe…

INTRODUCTION: The benign nature of Bell's palsy has led to a lack of a standardized work-up, and dangerous underlying mimics are at risk of being missed.
CASE REPORT: An 84-year-old female with a history of vertigo presented to the emergency department with a left-sided facial droop consistent with Bell's palsy. After further work-up, the patient was diagnosed with bilateral necrotizing mastoiditis.
CONCLUSION: Unilateral facial weakness involving the forehead and palpebral fissures is often diagnosed as idiopathic Bell's palsy. Various pathologies can present with unilateral facial weakness…

BACKGROUND: Mastoiditis is an otological emergency, and cross-sectional imaging has a role in the diagnosis of complications and surgical planning. Advances in imaging technology are becoming increasingly sophisticated and, by the same token, the ability to accurately interpret findings is essential.
METHODS: This paper reviews common and rare complications of mastoiditis using case-led examples. A radiologist-derived systematic checklist is proposed, to assist the ENT surgeon with interpreting cross-sectional imaging in emergency mastoiditis cases when the opinion of a head and neck radiolog…

Bezold's abscess is a very rare extracranial complication of acute mastoiditis. Lateral sinus thrombosis is another intracranial complication of acute mastoiditis that can occur, but there are only few reports of concomitant ispilateral Bezold's abscess and lateral sinus thrombosis with favorable outcome. We diagnosed and treated successfully a 14-year-old girl suffering from Streptococcus pneumoniae acute mastoiditis complicated with Bezold's abscess and lateral sinus thrombosis. Surgical treatment included myringotomy, cortical mastoidectomy, and Bezold's abscess incision and drainage. Durin…
