OBJECTIVES: To assess quality of life (QOL) outcomes after canal wall up (CWU) and canal wall down (CWD) tympanomastoidectomy in the pediatric population.
METHODS: A retrospective review tabulated pediatric patients undergoing CWU and CWD tympanomastoidectomy for cholesteatoma by 2 senior surgeons at a single tertiary academic referral center between March 2017 and March 2023. Chronic Ear Survey (CES) and cosmetic survey outcomes were collected post-operatively.
RESULTS: A total of 77 ears in 75 patients were identified, with 35 undergoing CWU and 42 undergoing CWD as the most recent (index)…

Myoepithelial carcinoma is a rare disease of the head and neck, with only a handful of cases reported occurring within the external auditory canal, middle ear, and mastoid. The patient discussed is a 67-year-old male with a prior history of a bilateral tympanomastoidectomy for cholesteatoma and a prior history of left parotidectomy and adjuvant radiation for pleomorphic sarcoma. Three years after the parotidectomy, the patient presented with contralateral ear fullness. During an intraoperative examination, a fibrous mass was encountered, which revealed an invasive salivary gland neoplasm, myoe…

Background: Mastoid development, tympanic sinus depth, and residual disease after surgery for congenital cholesteatoma are probably related, but these relationships have not been examined in detail.Aims/objectives: This study aimed to clarify the relationships between the abovementioned factors. Materials and Methods: The subjects were 31 patients with congenital cholesteatoma (stage III or IV in Potsic's staging system) that underwent mastoidectomy. The cross-sectional area of the mastoid air cells was measured as described previously. Tympanic sinus depth was classified into A-C using Marchi…

A chronically discharging cavity is a complication of canal-wall down mastoidectomy procedures. Mastoid obliteration can reduce this otorrhoea, with many techniques and materials for obliteration described. This study assesses the effectiveness of SerenoCem granules in providing successful obliteration, by recording the need for subsequent aural care and patient satisfaction using the Glasgow Benefit Inventory. All ears were made dry by the procedure (three after revision). Aural care visits reduced [down 9.2 (95% CI 4.7-13.8) over 2 years] and quality of life benefited [total postoperative Gl…

PURPOSE OF REVIEW: To describe the technique and discuss the advantages and outcomes of the trans-mastoid approach (TMA) for temporal bone cerebrospinal fluid (CSF) leak.
RECENT FINDINGS: TMA for CSF leak repair is an alternative to middle cranial fossa approach (MCFA) with less morbidity and good outcomes.
SUMMARY: Persistent CSF leak in the temporal bone whether idiopathic, congenital or acquired, is an indication for surgery. TMA is a valid option for surgery, competing with MCFA in selected cases. Surgical technique consists of a standard mastoidectomy, exposure of the osteodural defect,…

Acute mastoiditis (AM) is a relatively rare complication of acute otitis media (AOM). The most common pathogens include Streptococcus pneumo … AM. Both extracranial and intracranial complications of AM may occur. Previously, routine cortical mastoidectomy was recommended for AM treatment, but new data suggest that a more conservative treatment approach can be considered, including intravenous (IV) antibiotics alone or IV antibiotics with myringotomy. [Pediatr Ann. 2016;45(5):e176-e179.].

Perhaps the most devastating complication in otologic surgery is that of inadvertent injury to the facial nerve. A review of 22 patients who had sustained an iatrogenic facial nerve injury was conducted. Although the most common procedure being performed during the injury was mastoidectomy (55%), a surprising number of patients had injury during tympanoplasty (14%) or during removal of exostoses (14%). The most common area of injury to the facial nerve in this series was in the tympanic segment. In 79% of the patients, the facial nerve injury was not detected at the time of surgery. All patien…

The long-term results of surgery for chronic otitis media without cholesteatoma were analyzed in 76 ears of children aged 16 years or less. The mean follow-up period was 6.0 years. Thirty-four ears were subjected to mastoid surgery for chronic otorrhea; at final follow-up examination the disease was found to be cured in all but one patient. Forty-two patients with sequelae to chronic otitis media underwent tympanoplasty without mastoidectomy which was successful in 38 (91%) of these. At follow-up, 84% of the patients had hearing levels of 20 dB or better. Outcome of surgery and hearing results…

Tuberculous osteomyelitis of the temporal bone is a rare and dangerous entity that should be included in the differential diagnoses of infectious processes of the base of skull. A 21-year-old man presented with petrous apicitis, extradural and retromandibular abscesses, and paresis of the facial nerve. Immediate middle fossa craniotomy and drainage of the extradural abscess, in combination with a mastoidectomy, incision and drainage of the facial abscess, and antimicrobial therapy for gram-positive cocci, failed to check the destructive nature of the infection. The patient subsequently develop…

The incidence of mastoiditis in pediatric age has consistently increased over the last two decades due to increase of antibiotic-resistant bacteria. Compared to young patients, occurrence of acute otitis media complications such as acute mastoiditis and subperiosteal abscess is relatively low in adults. Various treatments for acute mastoiditis with subperiosteal abscess such as tympanostomy tube insertion, intravenous antibiotics, and postauricular incision and drainage have avoided the morbidity and necessity of mastoid surgery. Recently, many studies have indicated mastoidectomy only in case…

INTRODUCTION: Epidural abscess is the commonest intracranial complication of acute mastoiditis. In some cases this entity may pose a diagnostic problem.
MATERIALS AND METHODS: We report a case of acute mastoiditis followed by an epidural abscess in the middle cranial fossa and a bone fistula to subtemporal fossa. Localization of the abscess was revealed by means of computed tomography and magnetic resonance imaging of the head in early stage of the disease.
RESULTS: The treatment of choice was mastoidectomy with epitymanotomy, myringostomy, revision of zygomatic area to ensure drainage of th…

Acute mastoiditis is an acute inflammation of the mastoid process with bone erosion. It is a complication of acute otitis media, which is rare but with increasing incidence. Distinct characteristics are an erythema and oedematous swelling of the skin of the mastoid process. A fluctuant swelling points to a subperiosteal abscess. Laboratory examination and imaging only support the diagnostics. Therapy involves obligatory systemic antibiotic treatment. At the beginning of the inflammation a paracentesis can be sufficient. A mastoidectomy must be carried out if clear signs of an osseous necrolysi…

A series of 79 otic capsule fistulae occurring in 792 consecutive cases of chronic mastoiditis undergoing mastoidectomy was analyzed. The incidence (10%) is the same as in other large series, the primary difference being the location of the fistula. In all series cited, 90% of fistulae were in the lateral semicircular canal whereas in this series the comparable incidence was 75%. The 20 (25%) fistulae located in areas other than the lateral semicircular canal enabled us to document a method of "reading" the fistula test preoperatively to establish its precise location. Eye responses to the tes…

INTRODUCTION AND IMPORTANCE: External auditory canal cholesteatoma (EACC) is a rare, non-neoplastic lesion representing 0.1%-0.5% of otologi … segment of the facial nerve. Complete removal of the cholesteatoma was achieved via canal-wall-down mastoidectomy, followed by facial nerve decompression and canaloplasty.
CLINICAL DISCUSSION: EACC is a rare lesion of keratinizing epithelium confined to the bony canal, typically presenting with chronic otorrhea and dull otalgia. The diagnosis is primarily clin …

Ear surgeries such as mastoidectomy require a clear surgical field with minimal bleeding for optimal visualization and outcomes. However, bleeding during these microscopic and endoscopic procedures can obscure the surgeon's view and potentially impact surgical precision and efficiency. The study aimed to identify the effect of intravenous tranexamic acid during ear surgery. PubMed/MEDLINE, Cochrane Library, and Embase were systematically searched from inception through August 31, 2024. Two authors also manually and independently searched all relevant studies. We included RCTs with (1) patients…

Introduction: Cochlea implants can cause severe trauma leading to intracochlear apoptosis, fibrosis, and eventually to loss of residual hearing. Mild hypothermia has been shown to reduce toxic or mechanical noxious effects, which can result in inflammation and subsequent hearing loss. This paper evaluates the usability of standard surgical otologic rinsing as cooling medium during cochlea implantation as a potential hearing preservation technique. Material and Methods: Three human temporal bones were prepared following standard mastoidectomy and posterior tympanotomy. Applying a retrocochlear …
