Bisphosphonate-related osteonecrosis of the jaw around dental implants in the maxilla: report of a case.

OBJECTIVE: We describe a patient who developed bisphosphonate (BP)-related osteonecrosis of the jaw (ONJ) around implants in the upper molar area. PATIENTS AND METHODS: The patient was a 54-year-old woman with ulceration of the gum, bone exposure, and severe spontaneous pain around implants in the upper left molar area. She had received BPs intravenously for 2 years to treat bone metastases of breast cancer. She was diagnosed with BP-related ONJ. Sequestrum including implants was resected, and hyperbaric oxygen therapy was performed. Undecalcified ground sections were prepared from the resect

A 9-14 year follow-up of onlay bone grafting in the atrophic maxilla.

Treatment of the atrophic edentulous maxilla is challenging especially when bone graft procedures are necessary. In this study an onlay bone graft, a saddle or veneer, with or without maxillary sinus floor inlay graft, harvested from the anterior iliac crest, in combination with implants was used in the reconstruction of patients with extreme atrophy in their maxillae. The aim was to investigate treatment outcome, and the impact of gender and smoking, in 44 patients in a prospective, long-term, follow-up study concerning implant survival rate and marginal bone loss adjacent to the surfaces of …

The challenge of implant therapy in the posterior maxilla: providing a rationale for the use of short implants.

Rehabilitating patients with a resorbed maxilla presents several challenges when the desired treatment plan involves the placement of endosseous implants. Correct diagnosis requires knowledge on jaw healing patterns, systemic effects, and the impact of bone quality changes on implant success rates. Appropriate treatment planning requires an in-depth understanding of the materials and methods available to the contemporary implant surgeon. The clinician must be able to persist on evidence-based techniques and adhere to those proven methods. Successful surgical placement requires correct use of t…

Complications associated with segmentation of the maxilla: a retrospective radiographic follow up of 82 patients.

The aim of the present study was to retrospectively examine the clinical and radiographic incidence and frequency of injuries to teeth and their surrounding tissues adjacent to interdental osteotomies in conjunction with segmentation of the maxilla. Vertical interdental osteotomies have been performed in combination with Le Fort I osteotomy for correction of various dento-facial deformities. All our orthognathic patients are followed prospectively in a standardized manner. Eighty-two consecutive patients who underwent dento-facial correction by segmented maxillary osteotomy alone or in combina…

[A bone-destroying tumor of the maxilla. Reparative giant cell granuloma or brown tumor?].

Brown tumors are focal bone lesions caused by an increased osteoclastic activity and fibroblastic proliferation within a primary or more rarely secondary hyperparathyroidism. They are named after their typical brown hemorrhagic stroma with its also typical giant cell formations. We report the case of a 31-year-old pregnant patient with a rapidly growing tumor of her left maxilla whose first symptoms during pregnancy mimicked chronic sinusitis. After swelling of the cheek, diplopia, and recurrent epistaxis appeared, she was referred to our Department for further diagnostics. After CT scan, biop…

[Primary hyperparathyroidism presenting as a brown tumor of the maxilla and hard palate. Apropos of a case].

Significant controversy persists in the classification of giant cell lesions of the maxillofacial skeleton. Multifocal giant cell lesions are strongly suggestive of hyperparathyroidism brown tumors. Maxillary localization is extremely rare. We report a case of primary hyperparathyroidism manifesting in the form of giant cell tumors of the maxilla, the hard palate and long bones. Biochemistry confirmed the diagnosis. Computed tomography localized the parathyroid tumor at the cervicothoracic junction in the retrosternal area. We discuss the clinical features of skeletal involvement in primary hy…

Maxilla First versus Mandible First in 2-Jaw Orthognathic Surgery: Determining Factors and Outcomes.

BACKGROUND: The choice between maxilla first or mandible first in the double-splint method for orthognathic surgery remains controversial. There is little comprehensive analysis regarding the factors that affect sequence selection, subsequent accuracy, and stability outcomes. METHODS: This retrospective study was conducted on patients with jaw deformities who underwent orthognathic surgery at our center from 2020 through 2024. Inclusion criteria were preoperative simulation using 3-dimensional software and either maxilla-first or mandible-first procedures. Exclusion criteria were single-jaw s…

Physiotherapy Management for Surgical Defect of Operated Mucormycotic Osteomyelitis of Maxilla: Post COVID-19.

The maxilla is among the jaw bones with a lot of blood flow. Maxillary bone necrosis is uncommon and can be caused by infection, trauma, or unusual metabolic abnormalities. The maxilla is a vital bone that forms the roof of the mouth cavity. Mucormycosis is a prevalent fungus that infects the maxilla, particularly in diabetic and immune-compromised people and post-COVID-19 patients. Osteomyelitis is an inflammatory condition affecting the bone and marrow tissues. It is an opportunistic infection that occurs due to the host's susceptibility to illness due to the complication of other diseases. …

Different Treatment Modalities Using Dental Implants in the Posterior Maxilla: A Finite Element Analysis.

The objective of this study was to compare the biomechanical behavior of peri-implant bone tissue and prosthetic components in two modalities of treatment for posterior region of the maxilla, using short implants or standard-length implants associated with bone graft in the maxillary sinus. Four 3D models of a crown supported by an implant fixed in the posterior maxilla were constructed. The type of implant: short implant (S) or standard-length implant with the presence of sinus graft (L) and type of crown retention: cemented (C) or screwed (S) were the study factors. The models were divided i…

[Clinical application of spiral CT in locating impacted teeth of children,s anterior maxilla].

PURPOSE: To discuss the accuracy of localization and clinical usefulness of spiral CT for anterior maxillary impacted teeth. METHODS: In 89 children with impacted teeth in our department during June 2004 and December 2009, Spiral CT was performed in 30 cases whose impacted teeth can't be localized clearly by conventional occlusal film and panoramic radiogragh. Multip-lanarreformation(MPR) and the surface shaded display(SSD) of the spiral CT were used to observe the number, size, shape, location, its relationship with adjacent tissue, angle between crown and root of the impacted teeth. The region …

Implant versus tooth-implant supported prostheses in the posterior maxilla: a 2-year report.

When implants are used for restoration of a jaw with a residual dentition, the possibility of combining implants with natural abutments may be considered. In a longitudinal comparative study, 26 patients (15 women & 11 men, age 49-84 years) with residual anterior dentitions were treated with two different designs of fixed partial dentures bilaterally in the posterior maxilla. On one side the reconstruction was supported by implants only, while on the contralateral side an implant and a tooth in combination were used. The patients were followed at intervals of 3, 6, 12 and 24 months after loadi…

Intra-osseous squamous cell carcinoma of the maxilla: probably arisen from non-odontogenic epithelium.

This paper reports a case of intra-osseous squamous cell carcinoma (IOSC) of the maxilla. The patient was a 52-year-old male with a chief complaint of swelling of his hard palate. The lesion was initially diagnosed as an incisive canal cyst from the clinical and radiographic findings, but histological examination revealed the lesion to be a poorly differentiated squamous cell carcinoma. The review of the literature yielded only a few previous cases of IOSC of the maxilla, which were probably of odontogenic origin. The present maxillary IOSC is thought to be non-odontogenic in origin.

Surgical Accuracy of Maxilla-First Orthognathic Surgery That Tooth Interference OCCURS After Maxillary Movement.

This study aimed to compare the accuracy of maxillary repositioning using 2 types of CAD/CAM-fabricated splints (the traditional intermediate wafer and a customized maxillary repositioning guide) with a specific focus on dental interferences between the upper and lower arches during maxilla-first orthognathic surgery. Patients who exhibited dental interferences after maxillary movement during surgical simulation were enrolled. The intermediate wafer was designed to virtually open the patient's mandible, thereby avoiding tooth interference, whereas the guide was created independently of dental …
