Role of Pterygomaxillary Junction Morphology in Surgically Assisted Rapid Palatal Expansion: A Finite Element Analysis Study.

OBJECTIVE: In the present study, the morphological characteristic of the pterygomaxillary junction (PMJ) was determined radiologically and its effects on the maxillary expansion pattern were investigated by finite element analysis. METHODS: The 3-dimensional morphology of the PMJ was measured separately for each dimension by examining 240 cone beam computed tomography of maxilla. Seven maxilla models with different three-dimensional pterygomaxillary characteristics were modeled, and the effect of the structural characteristic of the PMJ on the maxillary expansion pattern, and the stresses in

Transverse maxillary distraction with a bone-anchored appliance: dento-periodontal effects and clinical and radiological results.

In 29 adult patients presenting with maxillary deficiency, a bone-anchored palatal distractor (Surgi-Tec NV, Brugge, Belgium) was applied after osteotomy of the anterolateral walls of the maxillary sinuses, midpalatal suture, and, eventually, separation of the pterygomaxillary sutures. Expansion proceeded at a rate of 0.33-0.66 mm per day and the device was retained for 4-6 months for consolidation. Active orthodontic therapy was started after 8-10 weeks. The increment of arch width and the perimeter were evaluated using dental casts. Tooth thermal sensitivity and the periodontal side effects

Relationship Between Expansion Magnitude and Relapse in Multi-Segment Le Fort I Osteotomy: A Retrospective Cohort Study.

BACKGROUND: Maxillary transverse deficiency is commonly addressed through multisegment Le Fort I osteotomies. Whether greater expansion magnitudes result in greater relapse remains uncertain, because the existing literature primarily reports relapse of procedures with dental expansion of <4 mm. PURPOSE: To measure the association between magnitude of expansion and relapse among patients undergoing multisegment Le Fort I osteotomies. STUDY DESIGN/SETTING/SAMPLE: This retrospective cohort study included subjects presenting to a private oral and maxillofacial surgery practice in Santa Barbara,

The use of Champy miniplates for osteosynthesis in craniofacial deformities and trauma.

Champy miniplates have been used in the treatment of craniofacial fractures and in osteotomies for correction of posttraumatic deformities, congenital craniofacial deformities, and secondary bony deformities due to pathologies such as hemangioma and neurofibroma. An additional use has been to stabilize free and vascularized bone grafts. The total number of cases reviewed were 50 (25 acute trauma, 4 vascularized mandible reconstruction, and 21 osteotomies of varied types). There were three infections, two in mandibular fractures that were comminuted and compound into the mouth and one in a comp

Anteroinferior distraction of the atrophic subtotal maxillary alveolus for implant placement: a case report.

Most reports on alveolar distraction have been related to vertical distraction in the mandible and the maxilla. There have been few reports on horizontal or oblique alveolar distraction. A case of an atrophic subtotal maxillary alveolus distracted 10 mm anteriorly and 5 mm vertically, followed by the placement of 9 implants, is presented. A healthy, 55-year-old woman presented with a chief complaint of mobility of all maxillary teeth. All remaining 11 teeth except the maxillary left second molar were diagnosed as being involved with advanced marginal periodontitis, and were considered hopeless

Le Fort I osteotomy for extensive juvenile nasopharyngeal angiofibroma: a retrospective study.

INTRODUCTION: Juvenile nasopharyngeal angiofibroma (JNA) is a rare, nonencapsulated, benign neoplasm typically diagnosed in adolescent boys. Surgery is the usual treatment modality for JNA. The optimal surgical procedure should allow maximal exposure of the tumor for complete excision with minimum morbidity. One possible surgical approach is Le Fort I maxillary osteotomy. The aim of this study was to review our experience with the Le Fort I osteotomy and to investigate the feasibility of this approach for extensive JNA invaded into pterygopalatine and infratemporal fossae. METHODS: We retrosp…

Maxillary Nerve as Landmark for Exposure of the Internal Maxillary Artery in Extracranial-Intracranial Bypass Surgery.

OBJECTIVE: To establish a new method for fast exposure of the internal maxillary artery (IMA) during extracranial-intracranial bypass surgery. METHODS: To explore the positional relationship between the IMA and the maxillary nerve and pterygomaxillary fissure, 11 formalin-fixed cadaveric specimens were dissected. Three bone windows of the middle fossa were created for further analysis. Then the IMA length that could be pulled up above the middle fossa was measured after different degrees of removal of bony structure. The IMA branches under each bone window were also explored in detail. RESUL

Bone remodeling of the sinus floor observed 19 years after third molar transplantation to close a maxillary defect: a case report.

Autologous tooth transplantation is the transplantation of an erupted, partially erupted, or unerupted tooth from one site to another in an individual. This is expected to maintain alveolar bone volume through physiological stimulation of the periodontal ligament (PDL). Tooth transplantation can be used for the closure of oroantral communication. As a simple, useful, and minimally invasive method, it should be considered a surgical option when a donor tooth is available in indicated cases. Herein, the authors report the case of a 20-year-old female patient whose left permanent maxillary first

Outcomes of alveolar segmental 'sandwich' osteotomy with interpositional particulate allograft for severe vertical defects in the anterior maxilla and mandible.

The purpose of this study was to report the outcomes of interpositional osteotomy with mineralized allograft in the treatment of alveolar vertical defects in preparation for implant placement. Thirteen defects (11 maxillary and two mandibular) were treated with osteotomy segments ranging in length from two to five missing teeth. The segments were positioned 5-7 mm coronally, with the gap space filled with allograft and then fixated with titanium hardware. Vertical bone augmentation was analyzed by superimposing pre- and post-surgical cone beam computed tomography images and stratified based on

The 'sausage technique' for the repair of large maxillary sinus membrane perforations.

Maxillary sinus membrane perforation is a common complication of lateral window sinus floor elevation and causes significant therapeutic challenges. A novel intrasinus 'sausage technique' is presented that involves spanning a collagen membrane across the osteotomy window with dual internal-external fixation. A stable protective membrane is used to encapsulate the bone grafts, simultaneously isolating perforations and preserving sinus floor vascularity. Clinical applications of this technique have demonstrated its ability to reliably repair large or complete perforations and improve success rat

Classification of Sinus Membrane Perforations Occurring During Transcrestal Sinus Floor Elevation and Related Treatment.

The predictability of transcrestal sinus floor elevation (tSFE) in elevating the sinus membrane following posterior maxillary ridge resorption has been widely demonstrated. To minimize complications and increase success, a literature search was conducted to validate procedures used for tSFE. A decision tree based upon timing of perforations was then developed to improve membrane-perforation management during the procedure. At each surgical procedure, the clinician is encouraged to use size of the perforation, time during the procedure in which the perforation occurred, and resulting symptoms t
