Do the dimensions of the hard palate have a relationship with the volumes of the upper airways and maxillary sinuses? A CBCT study.

BACKGROUND: As the hard palate is a central structure of the skull, and its close relationship with the nasal cavity, oral cavity, and maxillary sinuses, it would be of interest to study if there is a relationship between this bone and other structures of the stomatognathic system. Thus, this study aimed to assess the dimensions of the hard palate and associate them with sex, and skeletal and breathing patterns. Also, to investigate if there is a relationship between these dimensions and the volumes of the upper airways and maxillary sinuses. METHODS: Two hundred and ninety-eight CBCT scans o

[Recurrent painful ophthalmoplegia secondary to polyostotic fibrous dysplasia of the maxillary sinuses with involvement of the superior orbital fissure].

INTRODUCTION: Fibrous dysplasia is a bone disease that is usually accompanied by asymptomatic lesions but which may sometimes display neurological manifestations due to the involvement of the craniofacial bones. CASE REPORT: A 25-year-old female, with a history of migraines, who visited at the age of 18 due to pain in the right retro-ocular and maxillary region, although with characteristics that were unlike those of her usual migraines, and which was associated with ipsilateral ophthalmoparesis. The condition had a self-limiting course and responded well to corticoids, although it was recurr

Sensitivity and specificity of diagnostic tests in acute maxillary sinusitis determined by maximum likelihood in the absence of an external standard.

This study shows how to obtain maximum likelihood estimates of test sensitivities and specificities in case of lack of an external standard, using the Expectation Maximisation (EM) algorithm. This method is used to compare four diagnostic tests in patients suspected of acute maxillary sinusitis. Data were analyzed from published studies. Antral aspiration is the test with the highest diagnostic value. The diagnostic value of a positive clinical examination (according to explicit criteria) and of a positive radiograph or ultrasound are comparable. A negative radiograph is of more diagnostic val…

[Surgical treatment of chronic maxillary sinusitis. Report of a new surgical technic and an evaluation of results of traditional surgery].

During recent years, a new operative procedure for treatment of chronic maxillary sinusitis has been strongly recommended as being more effective and more physiological than the traditional Caldwell-Luc operation. The principle of the method is endoscopically guided endonasal resection of the anterior ethmoid cells and enlargement of the natural ostium of the maxillary sinus in the middle meatus. In contrast, the principle in the Caldwell-Luc operation is opening into the sinus in the canine fossa, providing direct approach to the lumen, and finally creation of a fenester towards the inferior …

Treatment of dental implant-related maxillary sinusitis with functional endoscopic sinus surgery in combination with an intra-oral approach.

The present report describes the case of a patient who underwent maxillary sinusitis right after dental implant installation with sinus lifting. Computed tomography scan revealed a dental implant (#16) was protruded inside the right maxillary sinus and confirmed the obstruction of ostium. A symptom remission was gained with the dual approaches combined by functional endoscopic sinus surgery and an intra-oral approach. Fully recovered function and healing of sinus were identified after 10 months follow-up. We report the case of sinusitis caused by protrusion of implants with sinus floor lift pr…

Endoscopic and Caldwell-Luc approaches in chronic maxillary sinusitis: a comparative histopathologic study on preoperative and postoperative mucosal morphology.

The aim of the present investigation was to study the histopathologic mucosal changes occurring in chronic maxillary sinusitis both preoperative and postoperative to functional endoscopic sinus (FES) surgery and the Caldwell-Luc (C-L) operation. Correlations were also sought between the histopathologic parameters and endoscopic findings, as well as patient symptoms. Sixty sinuses with the FES surgery and 55 sinuses with the C-L procedure were studied. The histologic parameters were graded semiquantitatively and compared preoperatively and postoperatively. The C-L operation reduced almost all p…

Twice-daily dosing of loracarbef 200 mg versus 400 mg in the treatment of patients with acute maxillary sinusitis.

Loracarbef is an oral synthetic beta-lactam antibiotic in the new carbacephem class. We conducted a multicenter, randomized, double-blind, parallel-group study to compare the efficacy and safety of loracarbef 200 mg twice daily (BID) and 400 mg BID when given orally for 10 days to patients 12 years of age and older with acute maxillary sinusitis. Because sinus aspirates for culture are not routinely obtained in the management of acute maxillary sinusitis, antimicrobial therapy usually is selected empirically. This study was designed to provide data simulating the usual clinical practice of tre…

Macroscopic purulence, leukocyte counts, and bacterial morphotypes in relation to culture findings for sinus secretions in acute maxillary sinusitis.

Macroscopic purulence, leukocyte counts, and bacterial morphotypes in Gram-stained smears were investigated in 335 sinus secretions (240 aspirates and 95 injection aspirates) obtained by puncture in 234 young patients with acute maxillary sinusitis. Over 90% of the 147 aspirates macroscopically classified as purulent also contained high numbers of leukocytes (greater than 20 per oil immersion field). A total of 82% of the 147 macroscopically purulent aspirates and 79% of the 156 aspirates containing high numbers of leukocytes yielded presumed sinus pathogens by culture in quantities of greater…

Differential Healing Patterns of Odontogenic Maxillary Sinusitis Following Nonsurgical Endodontic Therapy: A Cone-Beam Computed Tomography-Based Case Series.

This case series evaluated the differential healing patterns of maxillary sinus mucosa and periapical bone following nonsurgical root canal treatment for odontogenic maxillary sinusitis to propose a prognostic perspective based on distinct radiographic features. Ten cases of odontogenic maxillary sinusitis managed by primary nonsurgical root canal treatment or retreatment were categorized into 4 radiographic patterns based on periapical and sinus lesions. Clinical symptoms were recorded, and radiographic healing was assessed using cone-beam computed tomography (CBCT) at baseline and during fol…

Unilateral, Rostral Maxillary Sinusitis Resulting from Displacement and Retention of an Alveolar Plug in a 4-Year-Old Horse.

Sinusitis is a common occurrence in horses and often develops secondary to dental disease. Extraction of cheek teeth in horses is associated with variable degrees of complications and although postoperative displacement and retention of alveolar plugs has been identified as one such potential complication, few cases of resulting sinusitis have been reported. This manuscript describes a four-year-old Thoroughbred mare that was presented for chronic unilateral left-sided mucopurulent nasal discharge after extraction of the left maxillary second molar tooth two months earlier. Radiographic and co

Endodontic sealer-induced maxillary sinusitis: intraoral surgical approach and implant placement at a site of failed nasal endoscopic surgeries.

A 28-year-old male patient was referred from an otorhinolaryngologist for managing unilateral chronic maxillary sinusitis (MS). The patient had undergone 2 functional endoscopic sinus surgeries, although the MS was not resolved. Based on his dental history, endodontic treatment had been done on the symptomatic area. A leak of endodontic sealer and peri-apical lesion on tooth #14 was found on cone-beam computed tomographic examination. Extraction of tooth #14 and the modified Caldwell-Luc operation were performed to remove the endodontic sealer material and relevant inflammatory tissue. The sin…

Trans-implant treatment of chronic recurrent maxillary sinusitis via dynamic implants with an internal port: Experience with 31 cases.

This study was performed to test a protocol for trans-implant treatment of chronic recurrent maxillary sinusitis (CRS), via implants with an internal removable sealing screw. The appropriate indications for treatment, optimal number of treatment sessions, and general schedule of treatment and follow-up were assessed. This was a retrospective study of 31 patients with CRS who were managed with trans-implant lavage. Of these patients, 28 (referred for a sinus elevation procedure) underwent a simultaneous implantation procedure and treatment of the sinusitis. Three patients received treatment for…

Diagnostic value of plain radiographs in chronic maxillary sinusitis: a comparison between radiological and endoscopic findings in 75 patients.

Preoperative radiologic and intraoperative endoscopic findings of maxillary and ethmoid sinuses were compared in 75 adult patients, in whom 135 chronically inflamed maxillary sinuses were operated using functional endoscopic sinus surgery (FESS). At sinoscopy, secretion was found in 91% (41/45) of the radiologically opacified antra and in 47% (31/65) of the antra with moderate or marked mucosal swelling in plain radiographs. Antral fluid level in radiographs was a relatively rare (23%) finding and showed secretion as reliably as opacification. Radiographic detection of secretion in maxillary s

Chronic maxillary sinusitis of dental origin and oroantral fistula: The results of combined surgical approach in an Italian university hospital.

Unilateral chronic maxillary sinusitis is a possible complication of odontogenic disease or dental treatment and is mainly due to the development of an oroantral fistula (OAF). The management of chronic maxillary sinusitis of dental origin (CMSDO) requires a combined treatment via endoscopic sinus surgery (ESS) and intraoral surgical treatment of the odontogenic source. The aim of this study is to present the results of our university hospital unit in the treatment and follow-up of a case series of 34 patients treated with a combined surgical approach for CMSDO due to OAF. All patients were tr…

Cone Beam Computed Tomography Evaluation of the Artery in the Lateral Wall of the Maxillary Sinus: Retrospective Analysis of 602 Sinuses.

PURPOSE: This retrospective study evaluates the location of the arteries in the maxillary sinus, particularly in the middle portion, using cone beam computed tomography (CBCT) scans that can detect the lateral arteries with high reliability. METHODS: In this retrospective study, 2 experienced independent examiners evaluated 602 sinuses on CBCT scans. DISCUSSION: No significant correlation was found between the location of arteries and the patient's age, sex, or dentition. In 92.0% (P ≤ 0.001) of the sinuses, at least 1 arterial branch was detectable. CONCLUSION: Based on the CBCT scans, we

Diffusion-Weighted Magnetic Resonance Imaging in Maxillary Sinuses Inflammatory Diseases: Report of Three Cases and Literature Review.

BACKGROUND: Magnetic resonance imaging is considered a preferable imaging examination in the diagnosis of inflammatory maxillary sinus disease and can provide precise sinonasal characterization. Diffusion-weighted magnetic resonance imaging and apparent diffusion coefficient are complementary magnetic resonance imaging tools that can be applied to the differentiation of sinus diseases. In this report, 3 cases of inflammatory maxillary sinus diseases imaging findings considering diffusion-weighted magnetic resonance imaging features were described. Additionally, a literature review considering

Auto-obliteration of maxillary sinuses through osteoneogenesis in children with cystic fibrosis: A possible new way to reduce morbidity.

BACKGROUND: Cystic Fibrosis is an autosomal recessive disorder with a mutation in the cystic fibrosis transmembrane regulator gene, leading to dysregulation of epithelial fluid transport, dehydration of airway surfaces and impaired mucociliary function in the sinuses, lungs, pancreas and other organs. This leads to thickened secretion which blocks the sinus ostia and promotes chronic bacterial overgrowth and destruction of the mucosa. Chronic rhinosinusitis in cystic fibrosis patients leads to a decreased quality of life involving increased hospitalizations, infections, pneumonia, and acute ex
