Intent to Use Preexposure Prophylaxis (PrEP), HIV Risk Behaviors, and Self-Report Neurocognitive Symptoms by High-Risk Drug Users: A Mediation Analysis.

Neurocognitive impairment (NCI) and HIV risk are significant correlates of intent to use preexposure prophylaxis (PrEP). The extent to which similar outcomes could occur when these factors operate together, particularly for people who use drugs (PWUD), remains an important unanswered question. We therefore sought evidence that HIV risk behavior mediated the relationship between NCI and intent to use PrEP in high-risk PWUD in treatment. HIV-uninfected, methadone-maintained people who reported HIV-risk behaviors were enrolled (n = 400). Variables of interest (NCI, HIV risk behaviors, intent to u

African Americans with cancer pain are more likely to receive an analgesic with toxic metabolite despite clinical risks: a mediation analysis study.

PurposeRenal impairment is highly prevalent among patients with cancer, and many patients have undiagnosed chronic kidney disease (CKD) from underlying disease, treatment, or both. African American individuals have disproportionate risk factors (diabetes, hypertension) predisposing them to CKD. We investigated whether African American patients are more likely than white patients to receive morphine with 3- and 6-glucuronide metabolites, which are known to be neurotoxic and accumulate in CKD; whether insurance type mediates the relationship between race and the prescriber's opioid selection; an

Chlamydia pneumoniae-Mediated Inflammation in Atherosclerosis: A Meta-Analysis.

Several studies have attempted to relate the C. pneumoniae-mediated inflammatory state with atherosclerotic cardiovascular diseases, providing inconsistent results. Therefore, we performed a meta-analysis to clarify whether C. pneumoniae may contribute to the pathogenesis of atherosclerosis by enhancing inflammation. 12 case-control, 6 cross-sectional, and 7 prospective studies with a total of 10,176 patients have been included in this meta-analysis. Odds Ratio (OR) with a 95% confidence interval was used to assess the seroprevalence of C. pneumoniae and differences between levels of inflammat

Analysis of depressed cell-mediated immunity in asbestos workers.

To explore the mechanisms of asbestos-related perturbations of the immune system, we evaluated the in vitro cell-mediated immunity of five asymptomatic asbestos workers with hypergammaglobulinemia and decreased T-cell numbers. These results were compared with those in 10 matched controls. Analysis of T-lymphocyte populations revealed decreased absolute numbers of OKT4+ (helper/inducer) T cells in the peripheral blood and phytohemagglutinin (PHA)-stimulated mononuclear cell cultures of the workers. When chrysotile asbestos was added to PHA cultures, expansion of OKT4+ cell populations was dispr

Analysis of the actor-partner interdependence mediation model for self-disclosure between social support and benefit finding among stroke patients and their caregivers.

OBJECTIVE: Stroke is an acute, sudden-onset condition that constitutes an acute stressful event for both patients and caregivers, exerting a significant psychological impact. Based on the actor-partner interdependence mediating model, this study examines the influence of social support on benefit finding among stroke patients and their caregivers and investigates whether self-disclosure mediates this relationship. METHODS: This study is a cross-sectional study. Using convenience sampling, 278 pairs of stroke patients and their caregivers were selected from a Grade III Level A hospital in Lian

eHealth literacy and decisional conflict in patients with coronary heart disease: chain mediation of cyberchondria and psychological inflexibility and a latent profile analysis.

BACKGROUND: Coronary heart disease (CHD) is the leading cause of cardiovascular mortality worldwide, and its management requires patients to make complex, preference-sensitive treatment decisions. Decisional conflict is common in this population and adversely affects treatment adherence and outcomes, yet its association with eHealth literacy and the underlying psychological mechanisms remain unclear. OBJECTIVE: This study examined the association between eHealth literacy and decisional conflict in patients with CHD, tested the chain-mediating roles of cyberchondria and psychological inflexibi

Geographic Access Barrier as a Critical Mediator in Obstetric Fistula Treatment Cascade in Nigeria: Evidence from a Causal Mediation Analysis of 5,496 Cases.

BACKGROUND: Approximately two million women globally currently live with untreated obstetric fistula, predominantly in sub-Saharan Africa and South Asia, with Nigeria accounting for 40% of cases. Despite the recognition of multiple healthcare barriers, the causal mechanisms through which these barriers affect treatment-seeking behavior remain poorly understood. We conducted a causal mediation analysis to examine potential pathways in the fistula treatment cascade. METHODS: We analyzed data from 5,496 women who reported fistula symptoms in the 2024 Nigeria Demographic and Health Survey. Using …

Service satisfaction and perceived social support mediate the effect of internalised stigma on quality of life of people with mental illness: mediation analysis.

When people with mental illnesses internalise stigma, their quality of life (QoL) can be compromised. However, previous studies in sub-Saharan countries, including Ethiopia, have not examined the relationship between internalised stigma and QoL, nor how factors such as service satisfaction and perceived social support might mediate this relationship. In this study using a mediation analysis model we explore the possible mediating role of service satisfaction and perceived social support in the relationship between internalised stigma and QoL among people with mental illness attending psychiatr…

Mediation of Treatment Effects in Problem-Focused Social Skills Training versus Resource Activation for Children with Disruptive Behavior Problems: A Cross-Sectional Analysis.

A previous randomized controlled trial comparing problem-focused computer-assisted skills training (ScouT) with supportive resource activation treatment (STARK) in children (6-12 years) with disruptive behavior disorders (DBD) demonstrated significant pre-post changes in both treatment conditions, with ScouT being slightly superior on some outcomes. Based on the same sample (N = 99), the present study examined cross-sectional indirect associations between treatment condition and outcomes via intermediate variables, which conceptually correspond to mediation. We hypothesized that ScouT would le

Inflammatory Biomarkers as Mediators of the Effect Between High-Dose Corticosteroid Therapy and Mortality in COVID-19-Related ARDS: A Causal Mediation Analysis.

BACKGROUND: Treatment guidelines for acute respiratory distress syndrome (ARDS) recommend the use of high-dose corticosteroids based on their anti-inflammatory effects, yet the efficacy of this therapeutic strategy in patients with COVID-19 related ARDS is inconclusive. To more thoroughly understand the mechanism of action for high-dose corticosteroids in patients with COVID-19-related ARDS, we hypothesized that the reduction of inflammatory markers induced by high-dose corticosteroids would reduce mortality (i.e., inflammation as a mediator). DESIGN AND METHODS: In this retrospective cohort

Problematic mobile phone use, loneliness, and life satisfaction among college students during the COVID-19 lockdown in Shanghai: a cross-sectional mediation analysis.

INTRODUCTION: During Shanghai's COVID-19 lockdown, college students relied heavily on mobile phones for study, communication, and everyday activities, while normal campus routines and face-to-face contact were sharply restricted. Although problematic mobile phone use, loneliness, and life satisfaction have each received considerable research attention, less is known about how they were related under these unusual conditions. This study therefore examined their cross-sectional associations and evaluated whether loneliness statistically accounted for part of the association between problematic m
