Supplemental Rebates From Drug Makers To State Medicaid Programs Grew Fivefold From 2017 To 2024.

State Medicaid agencies receive discounted prescription drugs through the Medicaid Drug Rebate Program. States also can negotiate supplemental manufacturer rebates, but evidence on the variation and size of these rebates is limited. Using Centers for Medicare and Medicaid Services data, we found that mean supplemental rebates per beneficiary grew from $19 (2 percent of gross Medicaid drug spending) in fiscal year 2017 to $93 (7 percent) in FY 2024. Rebates varied between states (range: $2-$183 per beneficiary in FY 2024). Mean rebates per beneficiary were $20 higher in thirty-five states that …

Expansion of State Medicaid Policies Related to Telehealth, 2018-2023: A National Legal Mapping Study.

UNLABELLED: Policy Points Telehealth increases access to health care services for patients, including rural and underserved communities, and a 50-state survey of telehealth policies can further the study of the impact of telehealth on patient outcomes. This legal mapping study tracked telehealth policies by state Medicaid programs and found widespread adoption, especially in reimbursement for audio-only telehealth (adopted by 0 to 43 states from 2018 to 2023). State adoption of telehealth policies appeared to be independent of rurality, health professional shortage, Medicaid expansion, broadba…

Primary Care Follow-Up After Mental Health and Substance Use Emergency Department Visits in Medicaid.

IMPORTANCE: Medicaid beneficiaries-particularly those with mental health (MH) conditions, substance use disorder (SUD), and alcohol use disorder (AUD)-frequently use emergency departments (EDs). Timely primary care follow-up can reduce high 30-day ED revisit rates for these conditions; however, primary care follow-up after such ED visits remains understudied. OBJECTIVE: To examine rates of 30-day condition-concordant primary care follow-up for MH, SUD, and AUD after ED visits related to these conditions and identify characteristics associated with condition-concordant primary care follow-up. …

Should I Stay, or Should I Go? Provider Considerations for Leaving Medicaid Managed Care Organizations.

OBJECTIVE: As states transition from fee-for-service to Medicaid managed care, provider participation is critical for continuity of care. This study examines the factors influencing provider decisions to exit or consider exiting Medicaid managed care organizations (MCOs) following North Carolina's transition to managed care, implemented via a federal Section 1115 demonstration waiver. STUDY DESIGN: We conducted semistructured interviews with 41 individuals representing 26 provider organizations between March and July 2022 and with 47 individuals from 36 provider organizations between March an…

Telehealth Disparities in Outpatient Substance Use Disorder (SUD) Treatment among Medicaid Beneficiaries during COVID-19.

BACKGROUND: We investigated racial and ethnic disparities in telehealth counseling among Medicaid-insured patients in outpatient substance use disorder (SUD) treatment clinics and assessed whether the clinic-level proportion of Medicaid-insured patients moderated these disparities. METHODS: Using New York State (NYS) Medicaid and statewide treatment registry data, we analyzed 24,814 admission episodes across 399 outpatient SUD clinics during the first 6 months of COVID-19 (April-September 2020). Our outcome measure was the number of tele-counseling sessions within the first 90 days of treatme…

States With Substantial Increases In Buprenorphine Uptake Did So With Increased Medicaid Prescribing, 2018-24.

Multiple federal policy changes since 2018 have been intended to increase buprenorphine prescribing in response to a persistent treatment gap for opioid use disorder in the US. Anticipated national increases did not occur, but highly variable state-level trends provide important insights. We used IQVIA data to examine all-payer and per payer prescribing across states during the period 2018-24. All-payer prescriptions per 1,000 residents increased by more than 60 percent in ten states while decreasing by more than 10 percent in eight states. States expanding Medicaid during this period increase…

Medicaid Expansion Boosted Specialty Treatment Episodes For Substance Use Disorder In Expansion States, 2010-22.

The Affordable Care Act (ACA) enabled states to expand Medicaid to low-income adults and required expansion programs to cover substance use disorder (SUD) treatment. Extending prior research, we analyzed more recent effects of ACA Medicaid expansions on specialty SUD treatment, using 2010-22 all-payer data on treatment episodes. This period coincides with the worsening national drug overdose epidemic, as well as changes to Medicaid policy through program redesign and under the COVID-19 public health emergency. Using difference-in-differences methods, we found that after expansion, episodes to …

Early Third-Trimester Syphilis Screening Adherence Among Medicaid Recipients in New York City, 2020-2022.

BACKGROUND: In November 2019, New York City (NYC) mandated universal syphilis screening during pregnancy at 28 to 32 weeks' gestation (early third-trimester syphilis screening [ETTSS]) for congenital syphilis prevention. Medicaid is a significant payer for pregnancy-related and sexually transmitted infection care in NYC. We examined adherence to, and disparities in, ETTSS among pregnancies of NYC Medicaid recipients from 2020 to 2022. METHODS: We used Medicaid claims data to identify pregnancies using International Classification of Diseases, Tenth Revision codes related to delivery outcome a…

Lifetime abortion incidence when abortion care is covered by Medicaid: Maryland versus five comparison states.

OBJECTIVE: To estimate the association of Medicaid coverage of abortion care with cumulative lifetime abortion incidence among women insured by Medicaid. DATA SOURCES AND STUDY SETTING: We use 2016-2019 (Pre-Dobbs) data from the Survey of Women studies that represent women aged 18-44 living in six U.S. states. One state, Maryland, has a Medicaid program that has long covered the cost of abortion care. The other five states, Alabama, Delaware, Iowa, Ohio, and South Carolina, have Medicaid programs that do not cover the cost of abortion care. Our sample includes 8972 women residing in the study…

Interdisciplinary perspectives on accessing specialty evidence-based treatment for Medicaid-insured adolescents with eating disorders.

BACKGROUND: Family-based treatment (FBT), the leading intervention for adolescents with anorexia nervosa (AN), is severely understudied in outpatient care settings that serve publicly-insured populations. Many individuals with public insurance are lower-income, racially and ethnically diverse, and experience barriers to accessing evidence-based interventions for eating disorders (EDs). METHODS: Semi-structured interviews were conducted with ten interdisciplinary providers who provide specialty care to youth with EDs in an inpatient and/or outpatient medical setting. Interview questions were f

State-Level Variation in Medicaid Managed Care Enrollment and Specialty Care for Publicly Insured Children.

IMPORTANCE: Medicaid and Children's Health Insurance Program cover almost 50% of children with special health care needs (CSHCN). CSHCN often require specialty services and have been increasingly enrolled in Medicaid managed care (MMC) plans, but there is a dearth of recent national studies on specialty care access among publicly insured children and particularly CSHCN. OBJECTIVE: To provide recent, nationwide evidence on the association of MMC penetration with specialty care access among publicly insured children, with a special focus on CSHCN. DESIGN, SETTING, AND PARTICIPANTS: This cross-…

Health disparities attributed to Medicare-Medicaid dual-eligible status in patients with peripheral arterial disease.

OBJECTIVE: Peripheral arterial disease (PAD) is a prevalent and debilitating disease that can be effectively treated by surgical revascularization. However, Medicare-Medicaid dual-eligible patients have experienced worse long-term outcomes, notably higher rates of amputation and mortality, relative to other insurance groups. In the present study, we investigated how insurance status can perpetuate health disparities in PAD outcomes. METHODS: The National Inpatient Sample was queried from 2000 to 2011 for patients aged ≥18 years with PAD who had undergone surgical revascularization with hospit…
