Social contributions as risk factors for readmissions after lung transplantation: Clinical and financial implications.

INTRODUCTION: Lung transplant is associated with a 60%-80% 1-year post-transplant readmission rate. Social contributors represent potentially modifiable risk factors for readmission. We compared the clinical and financial of implications of readmissions associated with and without social factors. METHODS: Retrospective single-center study of lung transplant patients surviving to discharge between 2/2/2013 and 4/11/2023. Two reviewers categorized 1-year readmissions into two groups: social (eg, housing instability or rejection due to medication non-compliance) and non-social (eg, pancreatitis)

Predicting Unplanned ICU Admission in Trauma: Early Warning Signs on the Floor.

BackgroundUnplanned ICU admissions (UA-ICU) is a TQIP metric associated with increased morbidity, mortality, and resource utilization. Despite identification of certain predictive factors in prior publications, gaps remain in identifying patients at-risk, particularly once in-hospital and outside the emergency room setting. Our study investigates novel predictors and emphasizes outcomes in UA-ICU for trauma.MethodsRetrospective cohort study. Single ACS-verified Level 1 Trauma Center (2019-2023). N = 140 UA-ICU after initial non-ICU hospital ward admission and 140 controls never admitted to ICU

Efficacy of plasmapheresis in neutropenic patients suffering from cytokine storm because of severe COVID-19 infection.

BACKGROUND: With the emergence of the coronavirus disease 2019 (COVID-19) and inability of healthcare systems to control the disease, various therapeutic theories with controversial responses have been proposed. Plasmapheresis was administered as a medication. However, the knowledge of its efficacy and indications is inadequate. This study evaluated the use of plasmapheresis in critically ill patients with cancer. METHODS: This randomized clinical trial was conducted on 86 patients with malignancies, including a control group (N=41) and an intervention group (N=45) with severe COVID-19 during

Individuals hospitalized with acute mania have increased exposure to antimicrobial medications.

OBJECTIVES: We have preciously documented that many individuals with acute mania have immune activation. However, the sources of immune activation have not been identified. We investigated whether individuals hospitalized with acute mania have evidence of bacterial infections as determined by the prescription of systemic antimicrobial agents. METHODS: We assessed the recent prescription of systemic antimicrobial medications and the site of presumed bacterial infection in 234 individuals hospitalized for acute mania in either an inpatient unit or a day hospital. We also assessed individuals ho

Using Life-Saving Interventions to Determine Optimal Vital Sign Ranges among Adults Encountered by Emergency Medical Services.

BACKGROUND: Vital signs are an essential component of the prehospital assessment of patients encountered in an emergency response system and during mass-casualty disaster events. Limited data exist to define meaningful vital sign ranges to predict need for advanced care. STUDY OBJECTIVES: The aim of this study was to identify vital sign ranges that were maximally predictive of requiring a life-saving intervention (LSI) among adults cared for by Emergency Medical Services (EMS). METHODS: A retrospective study of adult prehospital encounters that resulted in hospital transport by an Advanced L

Improving 24-Hour Medication Reconciliation Through Audit and Feedback: A Multicycle Quality Improvement Study at a Scottish District General Hospital.

Introduction Medication reconciliation (MR) is a key patient safety process during hospital admission. MR ensures an accurate list of a patient's medications is compiled on hospital admission, enabling safe downstream prescribing and reducing adverse events. This responsibility falls primarily to the admitting clinician in paper-based systems with limited pharmacy input at the point of admission. This study aimed to assess local compliance with the national benchmarks and evaluate whether a structured audit-feedback intervention could improve MR documentation and the recording of reasons for w

Poor adherence to antiretroviral therapy among adult people living with HIV initiated during the COVID-19 epidemic waves - observations at the University Teaching Hospital in Lusaka, Zambia.

BACKGROUND: Coronavirus disease 2019 (COVID-19)-related disruptions in healthcare services and clinical outcomes have been predicted and documented. However, little is known about how antiretroviral therapy (ART) adherence disruptions caused by the COVID-19 pandemic have manifested amidst the 'Undetectable = Untransmittable' campaign initiative. Using a patient's viral load as a proxy for medication adherence, our study aimed to determine the adherence to ART on first-line medications among adult people living with human immunodeficiency virus (PLWHIV) at the University Teaching Hospital in Lu

Development and external validation of a dynamic nomogram for delayed cerebral ischaemia after aneurysmal subarachnoid hemorrhage: a study protocol for a multicentre retrospective cohort study.

INTRODUCTION: Delayed cerebral ischaemia (DCI) caused by aneurysmal subarachnoid haemorrhage (aSAH) is the most frequent complication and typically contributes to poor neurological outcome or deterioration of patients' condition. Therefore, early accurate and effective prediction of DCI is urgently needed. This study aims to construct a dynamic nomogram for precisely calculating the risk of DCI in patients with aSAH. Internal validation of this tool is conducted using the training cohort, and independent external validation is completed by using other medical centre datasets. METHODS AND ANAL

Therapeutic Targets for the Multi-system Pathophysiology of Heart Failure: Exercise Training.

Adult chronic heart failure (HF) is a terminal syndrome. While the HF phenotype is inhomogeneous across the ejection fraction spectrum, exercise intolerance remains a cardinal feature of all HF patients. Impairment of a single organ system cannot independently account for exercise intolerance in HF. Thus, the multi-system integrative pathophysiology of HF leads to challenges in identifying an effective medical therapy aimed at targeting a single mechanism to improve exercise tolerance. This unresolved medical care approach raises a number of points for discussion in this field as it is well-re

Frequency of laboratory measurement and hyperkalaemia in hospitalised patients using serum potassium concentration increasing drugs.

PURPOSE: Although, drug-drug interactions (DDIs) between potassium-increasing drugs (PIDs) are known risk factors for developing hyperkalaemia, not much is known about their risk and management strategies during hospitalisation. This study examines the frequency of serum potassium measurements and hyperkalaemia in hospitalised patients, based on the use of one or more PIDs, and the determinants thereof. METHODS: Adult patients hospitalised in the University Medical Centre Utrecht between 2006 and 2008 were included in this cross-sectional study. The frequency of serum potassium measurements a

Closing the Osteoporosis Care Gap: System-Level Fracture Liaison Service Implementation and a Patient Education Intervention in an Open Health System.

Background Osteoporosis impacts millions of individuals in the United States and is linked to significant morbidity and mortality after fragility fractures. Despite the availability of effective treatments, a majority of patients remain untreated. Fracture liaison services (FLS) have been shown to improve post-fracture management, particularly in integrated healthcare systems, yet evidence from open systems remains limited. This study evaluated the impact of FLS implementation at the Loma Linda University Health (LLUH), an open healthcare system, and examined whether a patient-level educationa

Perspective: Autonomic care systems for hospitalized patients.

With advancements of medical technology and improved diagnostic and treatment options, children with severe birth defects who would otherwise have no chance of surviving post birth survive to go home every day. The average lifespan in the United States has increased substantially over the last century. These successes and many other medical breakthroughs in managing complex illnesses, particularly in frail, elderly patients, have resulted in an increasing percentage of patients with comorbidities. This, coupled with a policy change by Medicare (i.e., Medicare will no longer reimburse hospitals

The prevalence and outcomes of in-hospital acute myocardial infarction in the Department of Veterans Affairs Health System.

BACKGROUND: Most studies of the epidemiology and treatment of acute myocardial infarction (AMI) have focused on patients who experienced onset of their symptoms in the community and then presented to the hospital. There are, however, patients whose symptoms of AMI begin after hospitalization for other medical conditions. The purposes of this study were to determine the prevalence of in-hospital AMI in the Veterans Health Administration (VHA) and to compare baseline characteristics, treatments, and outcomes according to whether individuals presented with AMI or had an in-hospital AMI. METHODS:

Hospital Guardianship: A Quality Needs Assessment of "Unbefriended" Patients Who Lack Decisional Capacity.

BACKGROUND: The "unbefriended" patient does not demonstrate capacity to make their own medical decisions, does not have an advance directive, and lacks a surrogate decision maker. For these patients without a designated health care proxy, hospitals may need to petition for public guardianship, a notoriously arduous process with undefined impact on hospital resources. OBJECTIVE: The objective of this study was to describe the characteristics, system needs, and financial impact of unrepresented inpatients in an academic, tertiary care, urban medical center. METHODS: The Northwestern Memorial H

Developing a decision rule to optimise clinical pharmacist resources for medication reconciliation in the emergency department.

BACKGROUND: The process of obtaining a complete medication history for patients admitted to the hospital from the ED at hospital admission, without discrepancies, is error prone and time consuming. OBJECTIVES: The goal of this study was the development of a clinical decision rule (CDR) with a high positive predictive value in detecting ED patients admitted to hospital at risk of at least one discrepancy during regular medication history acquisition, along with favourable feasibility considering time and budget constraints. METHODS: Data were based on a previous prospective study conducted at
