Dynamic contact mechanics of the medial meniscus as a function of radial tear, repair, and partial meniscectomy.

BACKGROUND: The menisci are integral to normal knee function. The purpose of this study was to measure the contact pressures transmitted to the medial tibial plateau under physiological loads as a function of the percentage of the meniscus involved by the radial tear or repair. Our hypotheses were that (1) there is a threshold size of radial tears above which contact mechanics are adversely affected, and (2) partial meniscectomy results in increased contact pressure compared with that found after meniscal repair. METHODS: A knee simulator was used to apply physiological multidirectional dynam…

Neurovascular safety and clinical outcome of outside-in repair of tears of the posterior horn of the medial meniscus.

PURPOSE: The purpose of this study was to evaluate the safety and clinical outcome of a new posterior approach for the known outside-in technique for repair of tears of the posterior horn of the medial meniscus (PHMM). METHODS: First, a cadaveric study was performed on 6 cadaveric knees to assess the safety of a point just lateral to the semitendinosus tendon as an entry point for outside-in repair. Dissection was done to measure the clearance of this point to the nearby popliteal bundle and saphenous nerve. A prospective case series study was then performed to assess the clinical outcome of …

An observational cohort study of the meniscus test to detect intravascular epidural catheters in pregnant women.

BACKGROUND: The meniscus test is a rapid non-pharmacologic method of confirming epidural catheter placement by observing a normal saline meniscus while physically manipulating the catheter. The aim of this study was to assess whether the meniscus test improves diagnostic accuracy of aspiration to detect intravascular or intrathecal placement of epidural catheters in pregnant women. METHODS: In this prospective observational study, parturients at >or= 36 weeks of gestation were recruited. In the sitting position, participants received a multiorifice epidural catheter for elective cesarean deli…

Arthroscopic all-inside repair for a tear of posterior root of the medial meniscus: a technical note.

This technical note describes a new arthroscopic technique to repair a tear of posterior root of the medial meniscus. Cartilage at the insertion area of the posterior horn of the medial meniscus (PHMM) was removed using a curved curette inserted through an anteromedial portal. A metal anchor loaded with two FiberWires (Arthrex, Naples, FL) was placed at the insertion area of the PHMM through a high posteromedial portal. A PDS suture was passed the PHMM by curved suture hook through the anteromedial portal. Two limbs of the PDS were then used to pass two limbs of the FiberWire through the menis…

Medial meniscus interposition in proximal tibial epiphyseal fracture: a case report and review of the literature.

Proximal tibial fractures are rare in children, and most are treated conservatively. We report a case of a minimally displaced proximal tibial epiphyseal injury (Salter-Harris type III) in an 11-year-old child. We initially thought to treat this fracture conservatively, but the presence of massive haemarthrosis and considerable pain made us choose arthroscopy under general anaesthetic. During examination under anesthesia no instability could be detected, but the knee was locked in 5 degrees of flexion. On screening the fracture was not reducible anatomically. At arthroscopy the medial meniscus…

Efficacy of therapeutic ultrasound treatment of a meniscus tear in a severely disabled patient: a case report.

This report focuses on diagnosis and management of a meniscus tear of unknown etiology in a functionally impaired patient. The patient was a 21-year-old man diagnosed with viral meningoencephalitis and postvaccinal central pontine myelinolysis, who experienced respiratory failure and required prolonged mechanical ventilation. He was in a comatose state for approximately 1.5 months. He presented to our facility 3 weeks after coma complaining of left knee pain and limited range of motion (ROM). Diagnostic workup, including magnetic resonance imaging, showed a grade II posterior horn medial menis…

Osteochondritis dissecans of the lateral femoral condyle following total resection of the discoid lateral meniscus.

PURPOSE: The purpose of this study was to describe the clinical presentation of 6 athletically active children with symptomatic osteochondritis dissecans (OCD) of the lateral femoral condyle following total resection for a torn discoid lateral meniscus and to discuss its cause. TYPE OF STUDY: Case series. METHODS: Six patients in whom OCD affecting the lateral femoral condyle developed after total resection of the discoid lateral meniscus participated in a detailed clinical, radiologic, and arthroscopic review. The average age at the time of meniscectomy was 9 years (range, 6 to 12 years). A…

Superior detachment of a glenoid labrum variant resembling an incomplete discoid meniscus in a wheelchair ambulator.

Symptoms of instability and painful clunking in the shoulder were found associated with superior detachment of an incomplete discoid glenoid labrum and biceps origin in a 25-year-old woman. The patient was paraplegic below the T4 level secondary to resection of a spinal cord astrocytoma at age of 17. Symptoms began after 3 years of wheelchair use and repetitive transfer injuries. On arthroscopic examination, only the central surface of the articular glenoid cartilage was visible because of labral coverage. The entire labrum was meniscoid without distinguishable variation. The superior labrum w

[Williams-Campbell syndrome complicated by multiple meniscus signs in an adult case; is it a characteristic finding?].

A 49-year-old woman visited our hospital for further examination of abnormal lung shadows in September 1984. She had suffered from pneumonia at the age of 3 years and had been treated for bronchial asthma until the age of 15. Many thin-wall cystic shadows were seen on her chest X-ray film in 1983. In July 1984, a mass-like lesion with a meniscus sign appeared in one of the cystic shadows. The number and the size of similar lesions increased with worsening of productive cough. An infiltrative shadow was recognized in the left lower lobe in September 1987 and January 1988, but was resolved by tr…

[A correlative retrospective study of the histological findings and arthrograms in degenerative disease of the meniscus (author's transl)].

The ability to diagnose degenerative changes of the menisci by means of arthrograms was studied retrospectively by correlating the histological and operative findings and arthrographic appearances in 45 cases. It was shown that degenerative changes, as indicated histologically, could be diagnosed by arthrography, but that the arthrogram gave no certain indication of its severity. On the other hand, it appears that fairly severe changes must be present before they become radiologically visible. In negative cases the presence of degenerative changes in bone and cartilage may indicate meniscus de…

The torn or degenerated meniscus and its relationship to degeneration of the weight-bearing areas of the femur and tibia.

Data was collected on 300 cadaver knees dissected for the prime purpose of studying chondromalacia of the patella. Simultaneous observations were also made on the state of the meniscus and the articular cartilage in the tibio-femoral joint. The data did not seem to support the oft-repeated statement that the torn meniscus is the primary cause of unicompartmental osteoarthritis. Investigations were also carried out on 69 additional cadaver knees and 100 consecutive unselected arthroscoped knees in which there was damage either to the meniscus or the adjacent joint surfaces. In 60% of the combin…

Integrated mapping of human meniscus and cartilage eQTLs reveals shared and distinct osteoarthritis genetic drivers.

Osteoarthritis (OA) is a prevalent musculoskeletal disorder and a leading cause of global disability. Although meniscal damage is a major risk factor of OA pathogenesis, genetic regulatory studies have remained largely confined to articular cartilage. Here, we establish the first comprehensive expression quantitative trait locus (eQTL) map integrating whole-genome sequencing and bulk transcriptomics from human meniscus (n=112) and cartilage (n=113). Supported by single-nucleus multiomics (cartilage: 56,549 nuclei; meniscus: 34,343 nuclei), we uncovered highly tissue-specific genetic risk archi…

Marathon Running Transiently Causes Medial Meniscus Extrusion: Recovery Time Course and Factors Associated With Extrusion.

Meniscal recovery time following marathon running remains unknown, despite the need for practical recommendations on post-race return to training. We aimed (1) to characterize the time course of medial meniscus extrusion (MME) after a full marathon and (2) to identify baseline factors associated with the magnitude of the acute increase in MME. One hundred recreational runners (200 knees; mean age 47.2 years; 77 men, 23 women) from five marathons underwent weight-bearing, fully extended-knee portable ultrasonography at baseline and immediately post-race, and 90 runners (180 knees) were reassess…

Increased medial meniscus extrusion on ultrasonography in highly competitive rugby players: A cross-sectional study.

Medial meniscal extrusion (MME) has been shown to be associated with the onset and progression of knee osteoarthritis (KOA); however, evidence focusing on young athletes without a history of knee injury remains limited. This study aimed to determine whether MME is already increased in young competitive rugby players compared with controls. The participants comprised 94 male collegiate rugby players without a history of knee trauma or surgery (165 knees) and 42 male university students serving as controls (81 knees). MME was assessed bilaterally using ultrasonography (US) in three knee position

Meniscus Allograft Interpositional Arthroplasty With Elastic Intramedullary Bioabsorbable Pin Fixation for Proximal Interphalangeal Joint Contracture.

Advanced proximal interphalangeal (PIP) joint contracture presents a surgical challenge, particularly in patients who require preservation of joint motion. Arthrodesis provides reliable pain relief, but eliminates motion, whereas implant arthroplasty may be associated with mechanical complications and implant failure. We report a case of severe degenerative PIP joint contracture treated with meniscus allograft interpositional arthroplasty combined with intramedullary bioabsorbable pin fixation. Preoperatively, the joint was fixed at 60° of flexion. At 1-year follow-up, active range of motion i…

Hydrophilic polydopamine (hPDA) fueled bioglue enhances tissue adhesion and promotes healing of avascular meniscus tears.

Avascular meniscus tears exhibit minimal intrinsic healing and frequently progress to joint degeneration because of limited vascularity, poor biological repair capacity, and inadequate restoration of tissue-level mechanical continuity. Here, we report a hydrophilic polydopamine (hPDA) fueled bioglue platform that overcomes the aqueous insolubility limitations of conventional polydopamine (PDA) and enables functional repair of avascular meniscus injuries. Water soluble hPDA was synthesized through controlled depolymerization and recrystallization, yielding monomeric and oligomeric species rich …
