Sleep and Mental Health Issues in Current and Former Athletes: A Mini Review.

Sleep and mental health are important aspects of human health that work concurrently. However, sleep and mental health disorders are often overlooked and undiagnosed in sport due to the negative stigma associated with them. Evidence suggests that athletes are disproportionately affected by mental health issues and sleep problems. Internal and external pressures contribute to psychological distress. Variable competition times, travel and stress are detrimental to sleep quality. Retirement from sport can deteriorate sleep and psychological wellbeing, particularly for those who retired involuntar

Recovery-oriented mental health training interventions: An integrative review.

BACKGROUND: Whilst the concept of personal recovery in the mental health field, supporting people with mental illness in finding purpose and meaning in their lives, is gaining international recognition, many healthcare providers remain more familiar with clinical recovery, which focuses on medication and symptom management. In this review, we focused on personal recovery-oriented training interventions designed to improve recovery-related knowledge, attitudes, and competency among healthcare professionals and peer support workers caring for people with mental illness within mental health and s

Measuring fatigue four years after stroke: a bi-factor analysis of the Norwegian Chalder Fatigue Questionnaire.

BACKGROUND: Post-stroke fatigue (PSF) is a persistent and debilitating condition. Assessing its characteristics and severity accurately is important, but the suitability of existing fatigue instruments for use in stroke populations is uncertain. The Chalder Fatigue Questionnaire (CFQ) is a widely used generic measure capturing both mental and physical fatigue, but the dimensionality of the 11-item CFQ is debated. The objective of this study was to examine the best-fitting model for the CFQ and evaluate whether the addition of latent specific factors for the physical and mental items provided m

Sociodemographic Disparities in Mental Health Care Settings Among Adults.

OBJECTIVE: Untreated mental illness has significant consequences for both society and individuals, yet sociodemographic disparities in mental health care persist. Given the barriers to mental health care that rural and racially marginalized populations face, understanding mental health treatment settings across these populations may help catalyze future actions tailored to these settings. This study aimed to analyze rural-urban and racial-ethnic disparities across mental health care settings. METHODS: This cross-sectional study examined 92,639 adult respondents to the National Survey on Drug

Governing Dementia: A Historical Investigation of the Power of States and Professionals in the Conceptualization of Dementia in China.

This study intends to understand how Chinese states and healthcare professionals interact with each other in adopting biomedical concepts within the context of globalization of mental health. The conceptualization of dementia as a stigmatized mental disorder in China serves as a salient case to examine interactions between states and professionals as well as the interrelationships between different healthcare professionals in producing knowledge. By engaging the biopolitical approach, this project explores the historically-contingent conceptualizations of dementia, namely dementia as a vague a

Recruitment and retention of homeless individuals with mental illness in a housing first intervention study.

BACKGROUND: Homeless individuals with mental illness are challenging to recruit and retain in longitudinal research studies. The present study uses information from the Vancouver site of a Canadian multi-city longitudinal randomized controlled trial on housing first interventions for homeless individuals. We were able to recruit 500 participants and retain large number of homeless individuals with mental illness; 92% of the participants completed the 6-month follow up interview, 84% the 24-month follow up, while 80% completed all follow-up visits of the study. PURPOSE: In this article, we des

Assisted suicide in Germany: a survey on requests for and current practices of assisted suicide.

BACKGROUND: Following the constitutional court ruling in 2020, which legalized assistance in suicide in Germany under certain conditions, the practice of assisted suicide in Germany has been developing rather dynamically regarding the number of assisted suicides as well as the number of institutions offering suicide assistance. Challenges include a competent assessment of decisional capacity and the choice of adequate drugs. The aim of this study was to provide information about characteristics of people requesting assistance with suicide and those who died by assisted suicide as well as infor

Establishment and clinical impacts of decision-support system for older patients with aortic valve stenosis: A retrospective observational study.

OBJECTIVES: Treatment decisions for severe aortic stenosis (AS) are complex, since there are two active and comparable options: transcatheter aortic valve implantation and surgical aortic valve replacement. The disease predominantly affects older individuals, who are frequently comorbid with from cognitive impairment. This study aimed to establish a screening-triggered system to assess the decision-making capacity of patients with AS, support their decision-making, and facilitate referrals to specialists when necessary. METHODS: This is a retrospective pre-post observational study. Relevant h

American Geriatrics Society position statement: Making medical treatment decisions for unrepresented older adults.

This paper is an official position statement of the American Geriatrics Society (AGS) and updates the 2017 AGS position statement, Making Medical Treatment Decisions for Unbefriended Older Adults. In this updated position statement, the term "unbefriended" is replaced by "unrepresented" as a term that is more value-neutral, more accurately describes the circumstance in which a person without medical decision-making capacity does not have recognized surrogate representation, and better aligns with increasingly preferred terminology as reflected in recent medical literature. We define unrepresen

Brain-Computer Interfaces, Completely Locked-In State in Neurodegenerative Diseases, and End-of-Life Decisions.

In the future, policies surrounding end-of-life decisions will be faced with the question of whether competent people in a completely locked-in state should be enabled to make end-of-life decisions via brain-computer interfaces (BCI). This article raises ethical issues with acting through BCIs in the context of these decisions, specifically self-administration requirements within assisted suicide policies. We argue that enabling patients to end their life even once they have entered completely locked-in state might, paradoxically, prolong and uphold their quality of life.

Revisiting the Role of Health Care Proxies in Reproductive Choice.

In an effort to protect patients' reproductive rights, many states prohibit health care proxies from serving as surrogate decision makers for pregnancy termination in patients who lack capacity. We explore the case of a 24-year-old developmentally delayed woman with intractable seizures and complex psychosocial needs who was found to be pregnant. Her older sister was her health care proxy and declared that an abortion would be in her best interest, medically and socially; the patient herself lacked capacity to make this decision. Legally, her sister's judgment alone was insufficient to move fo

Factors that predict murder defendants' competence to stand trial.

One hundred sixty-three homicide case files from The Center for Forensic Psychiatry in Ypsilanti, Michigan were examined for evidence of factors associated with the outcomes of Competency to Stand Trial (CST) evaluations. Of the socio-demographic, legal, and clinical factors investigated, only three were significant. Defendants with lower IQs were more likely to be found incompetent to stand trial, and those with more property crime arrests were more likely to be found competent to stand trial. Additionally, defendants who were found incompetent to stand trial were more likely to be accused of

Lack of Capacity is not an 'off Switch' for Rights and Freedoms: Wye Valley Nhs Trust V Mr B (By His Litigation Friend, the Official Solicitor) [2015] Ewcop 60.

Wye Valley NHS Trust v Mr B [2015] EWCOP 60 illustrates the extent to which the wishes, feelings, beliefs, and values strongly expressed by a person who lacks decision-making capacity are to be considered in determining his best interests. Whilst not going as far as a supported decision-making model, as endorsed by the UN Convention on the Rights of Persons with Disabilities, the case exemplifies the participative ethos of the Mental Capacity Act 2005 and the requirement that the person lacking capacity should participate as fully as possible in any decision affecting him.

Futility in Chronic Anorexia Nervosa: A Concept Whose Time Has Not Yet Come.

Comparatively little scholarly attention has been given to the question of futility in chronic psychiatric disorders, with the exception of a small body of work on so-called end-stage anorexia nervosa (AN). A review of this literature provides the background for a critical examination of whether the concept of futility has any clinically meaningful, ethically justifiable, and legally defensible application to AN. In this article, the arguments for and against futility judgments in AN are analyzed with special emphasis on determinations of capacity in this serious mental illness. Parallels betw

Anorexia nervosa: the diagnosis. A postmodern ethics contribution to the bioethics debate on involuntary treatment for anorexia nervosa.

This paper argues that there is a relationship between understandings of anorexia nervosa (AN) and how the ethical issues associated with involuntary treatment for AN are identified, framed, and addressed. By positioning AN as a construct/discourse (hereinafter "AN: the diagnosis") several ethical issues are revealed. Firstly, "AN: the diagnosis" influences how the autonomy and competence of persons diagnosed with AN are understood by decision-makers in the treatment environment. Secondly, "AN: the diagnosis" impacts on how treatment and treatment efficacy are defined and the ethical justifiab

Measures of everyday competence in older adults with cognitive impairment: a systematic review.

BACKGROUND: The issue of safety of the cognitively impaired elderly people living alone has been continuously raised. Traditional psychometric measures of cognitive abilities may not adequately reflect older adults' functioning in a real everyday context. OBJECTIVES: To conduct a systematic review on instruments available for evaluating the everyday problem-solving or everyday competence of the elderly with cognitive impairment and to critically review the measurement properties of the identified instruments. METHODS: We searched the databases such as Cinahl, Medline, PsycINFO, AARP Ageline,

What to do when a competent ICU patient does not want to live anymore but is dependent on life-sustaining treatment? Experience from The Netherlands.

If patients on the intensive care unit (ICU) are awake and life-sustaining treatment is suspended because of the patients' request, because of recovering from the disease, or because independence from organ function supportive or replacement therapy outside the ICU can no longer be achieved, these patients can suffer before they inevitably die. In The Netherlands, two scenarios are possible for these patients: (1) deep palliative (terminal) sedation through ongoing administration of barbiturates or benzodiazepines before withdrawal of treatment, or (2) deliberate termination of life (euthanasi

The RCPSC oral examination: patient perceptions and impact on participating psychiatric patients.

OBJECTIVE: To investigate patient perceptions and the impact of participating in the Royal College of Physicians and Surgeons of Canada (RCPSC) long case oral examination. METHOD: Immediately following the RCPSC exam, participating psychiatric patients completed a Participant Impact Questionnaire. We combined results for 2002 and 2003 from both the western and eastern sites. RESULTS: Overall, there was a 93% response rate (132/142): 98% (65/66) for the western site and 88% (67/76) for the eastern site. Of the respondents, 94% said they would participate in the RCPSC oral exam again, if requi
