The vascular anatomy of the dorsal aspect of the hand is variable. Nevertheless the presence of the first and the second dorsal metacarpal artery (DMA) is constant. DMA3 and 4 are more variable. Metacarpal and metatarsal fractures in 11 patients were treated 'closed' with a specially designed External Skeletal Fixation (ESF) frame with a walking bar and traction applied to the digits. PURPOSE: To determine factors that affect healing index and consolidation time following lengthening of congenitally short metacarpals (brachymetacarpia).
METHODS: 18 metacarpals of 8 young women (mean age, 18.6 years) were lengthened using the callus distraction technique. The results, including healing index and consolidation time, were analysed and compared with those in the literature.

Hands are involved in over 70% of all serious burns. This study comprised 80 hands in 41 patients with burns of the dorsum of the hand. Eighty percent of the hands in our study had deep partial-thickness and full-thickness burns that had to be grafted. AIM: This study was undertaken to analyse the importance of the Metacarpal Index (MCI) in making the diagnosis of Marfan syndrome.
PATIENTS AND METHODS: The characteristics of 42 patients with Marfan syndrome as defined by strict criteria were analysed. Fifty-one consecutive accident and emergency patients comprised the control group for the MCI measurements.

A 5-year retrospective review of 83 patients with 98 metacarpal fractures was performed. Fractures of the thumb metacarpal were excluded. Ninety-six closed metacarpal fractures were reduced closed and fixed with flexible intramedullary fixation, using multiple 0.8-mm prebent rods. Two open metacarpal fractures were also fixed with this technique. Fractures amenable to flexible intramedullary fixation include short oblique and transverse fractures. A complex hand injury with scaphotrapezial dislocation and dislocation of the trapezoid, third, fourth, and fifth carpometacarpal joints occurred when a motorcyclist was involved in a road accident. The extent of the injury was initially overlooked. Combined injuries of the carpus and carpometacarpal joints can easily be missed at radiographic examination. The true lateral radiograph of the wrist is necessary in the diagnosis of complex wrist injuries.

Longitudinal lacerations involving the finger commissures and adjacent intermetacarpal clefts (including the adjacent tendons, the intrinsic muscles, the deep transverse metacarpal ligament, adjacent arteries, nerves, bones and joints), present multiple problems not seen in the unusual hand lacerations. Since January 1979 we have treated 14 patients at the 1st University Clinic of Traumatology in Vienna with the MFE described by Jaquet. The MFE was used mainly in severe open fractures of fingers and metacarpal bones. All 14 patients were examined for follow-up after a mean interval of 20 months. This method of treatment warrants minimal strain on the soft tissues while it ensures stability and optimal care of the wounds.
