BACKGROUND: The reason for the high prevalence of mild cobalamin (vitamin B-12) deficiency in the elderly is poorly understood.
OBJECTIVE: We aimed to determine the reason for this high prevalence.
DESIGN: We examined cobalamin intake, the presence and severity of atrophic gastritis, the presence of Helicobacter pylori infection, and plasma cobalamin and methylmalonic acid (MMA) concentrations in 105 healthy, free-living, older subjects aged 74-80 y.
RESULTS: Mild cobalamin deficiency, ie, low to low-normal plasma cobalamin concentrations (< 260 pmol/L) and elevated plasma MMA concentration…

BACKGROUND: Children are susceptible to mercury toxicity, and mercury has immunomodulatory effects. Lower folate and B-12, and higher homocysteine may represent susceptibility cofactors. A large proportion of variability in rubella immune response is attributable to environmental factors.
OBJECTIVE: This study aimed to evaluate the interaction between total blood mercury (Hg) and nutritional and homocysteine status on rubella virus antibody concentrations.
DESIGN: Cross-sectional data on rubella IgG antibody concentrations, Hg, homocysteine, methylmalonic acid (MMA, an indicator of B-12 defi…

BACKGROUND: Anaemia is highly prevalent and commonly caused by nutritional deficiencies. Guidelines often include an algorithm to find the u … n B12 or folate deficiency, besides measurement of vitamin B12 or folate, some guidelines mentioned methylmalonic acid or homocysteine. Quality of evidence underlying cut-offs and parameters was variable, and laboratory aspects were underrepresented.
CONCLUSIONS: There was a lot of variation in the included diagnostic algorithms, especially for iron deficiency anae …

Cobalamin (vitamin B12 [Cbl]) is an essential cofactor for many biochemical pathways. Transcobalamin (TC) is required to internalize Cbl into the cells through membrane receptor-mediated endocytosis. Cbl is then processed in the cytoplasm and mitochondria by complementation factors leading to its active metabolites; methylcobalamin and 5-deoxyadenosyl-cobalamin. Deficiency of TC results in an elevation in methylmalonic acid and homocysteine. Patients usually present with macrocytic anemia, pancytopenia, failure to thrive, gastrointestinal symptoms, and neurological dysfunction. In this study, …

Poor micronutrient status is associated with diarrheal illness, but it is not known whether low folate and/or cobalamin status are independent risk factors for diarrhea. We measured the association between plasma folate and cobalamin and subsequent diarrheal morbidity in a prospective cohort study of 2296 children aged 6-30 mo in New Delhi, India. Plasma concentrations of folate, cobalamin, total homocysteine (tHcy), and methylmalonic acid were determined at baseline. Whether a child had diarrhea was recorded during weekly visits in a 4-mo zinc supplementation trial. Diarrhea episodes lasting …

We performed studies in 25 patients with low serum cobalamin levels who had few if any clinical or hematologic findings of cobalamin deficiency. All but three had morphologically normoblastic hematopoiesis, and 15 were not even anemic. None of those tested excreted methylmalonic acid or homocystine. Nevertheless, the dUST identified metabolic abnormalities in 18 of the 25 cases. In vitro additives were essential in the dUST. Especially noteworthy was MTHF, whose addition unmasked an otherwise undetectable dUST abnormality in four cases. Why MTHF appears to act as a "stress test" in this settin…

Vitamin B12 deficiency in early childhood is an important cause of neurodevelopmental delay and regression. Most of these cases occur in exclusively breast-fed infants of deficient mothers. Symptoms and signs of vitamin B12 deficiency appear between the ages of 2 to 12 months and include vomiting, lethargy, failure to thrive, hypotonia, and arrest or regression of developmental skills. Approximately one half of this cases exhibit abnormal movements, variously described as tremors, twitches, chorea, or myoclonus. Urinary concentrations of methylmalonic acid and homocysteine are characteristical…

Dysregulation of the mitochondrial network in terminally differentiated cells contributes to a broad spectrum of disorders. Methylmalonic acidemia (MMA) is an autosomal recessive inborn error of intermediary metabolism caused by the deficiency of methylmalonyl-CoA mutase (MMUT) - a mitochondrial enzyme that mediates the degradation of certain amino acids and lipids. The loss of MMUT activity triggers an accumulation of toxic endogenous metabolites causing severe organ dysfunctions and life-threatening complications. How MMUT deficiency instigates mitochondrial distress and tissue damage remain…

INTRODUCTION: We designed a new indicator of cobalamin deficiency, a condition that sometimes can be difficult to diagnose.
MATERIALS AND METHODS: The new indicator is a combination of the plasma concentrations of cobalamin (P-cobalamin) and methylmalonic acid (P-MMA) adjusted to an estimated glomerular filtration rate (eGFR) of 100 mL/min/1.73 m2, called P-MMA100. First, P-cobalamin and P-MMA100 are transformed to a multiplum of their median in the population (MoM-values). Second, the indicator iB12 is calculated as log10[MoM(P-cobalamin)]/[MoM(P-MMA100)] and adjusted for age and sex. We stu…

BACKGROUND: Methylmalonic acidaemia (MMA) is a genetic metabolic disorder caused by congenital defects that may result in multisystem damage. However, MMA complicated with acute myeloid leukemia (AML) is very rare.
CASE PRESENTATION: Here, we report a case of MMA with AML in a boy aged 7 years and 6 months. The boy was screened for MMA after birth and received long-term treatment with dietary control, vitamin B12, and L-carnitine. He was readmitted at the age of 7 years and 6 months with systemic bleeding spots and was diagnosed with AML by bone marrow cytology. When the diagnosis was clear, …
