Localized neurological necrotizing vasculitides. Three cases with isolated mononeuritis multiplex.

Localized vasculitic neuropathies are increasingly reported. We describe 3 cases of peripheral neuropathy with necrotizing vasculitis confined to nerves and muscles without systemic involvement. These neuropathies were severe and relapsing, in contrast to a usually benign prognosis. Our cases appear to be isolated vasculitic neuropathies, with vasculitis strictly limited to the peripheral neuromuscular system without nonspecific clinical and/or biological systemic involvement.

Atraumatic intraneural heterotopic ossification of the common peroneal nerve: illustrative case.

BACKGROUND: Intraneural heterotopic ossification, or neuritis ossificans (NO), is characterized by nonneoplastic calcifications of extra-ske … ons. It emphasizes the need for a high index of suspicion in patients with unexplained, progressive mononeuropathies. When conventional imaging and electrodiagnostic studies are inconclusive, early referral for advanced imaging and specialist evaluation may be critical. Delayed recognition can result in significant neurological impairment. https://thejns.org/doi/1 …

Peripheral nerve biopsy in pure neural leprosy: a 26-year experience in Brazil.

Brazil ranks second globally in leprosy burden, with approximately 25 000 cases reported in 2022. This study aimed to analyse the 26-year ex … ensory disorders (85.7%), localized paraesthesia (70.3%) and muscle weakness (59.4%), with multiple mononeuropathies frequently observed on electroneuromyography. The most biopsied nerves were the ulnar cutaneous branch (50.6%), sural (37.2%) and superficial peroneal (6.8%). Nerve biopsy proved valuable for confirming leprosy and distinguishing differential diagno …

Dextrose-based Lyftogt perineural injection therapy on facial muscle strength in a Bell's palsy patient - a case report.

Bell's palsy, a unilateral peripheral facial nerve palsy, is one of the most common acute mononeuropathies. Chronic cases persisting beyond one year present significant challenges to conventional treatment methods. Standard treatment includes steroids and antiviral medications in the acute phase, and physical therapy, various supplements such as vitamin B12 and folic acid supplementation, and injections for chronic therapy. This case report describes the effect of a unique treatment, dextrose-based Lyftogt perineural injection therapy (LPIT), in a 22-year-old Filipino male with left-sided peri…

Report of an Isolated L5 Radiculopathy Caused by an L2-3 Disc Herniation and Review of the Literature.

Intervertebral disc herniation is a common cause of radiculopathy. Disc herniations occurring in the lumbar spine typically affect the nerve … drop and remained symptom-free. Although rare, disc herniations may cause isolated, remote, painful mononeuropathies not related to the direct level of nerve root compression and should be considered along with other etiologies of peripheral neuropathies.

Atypical diabetic neuropathies.

Diabetes is one of the most common conditions in the world and is associated with a broad range of adverse effects in multiple organ systems … sculitis that does not seem to respond to immunomodulatory therapy. By contrast, monophasic cranial mononeuropathies are caused by non-inflammatory microvascular ischemia and present acutely followed by slow improvement. Recent Advances in the Understanding and Management of Carpal Tunnel Syndrome: a Comprehensive Review.

PURPOSE OF REVIEW: Carpal tunnel syndrome (CTS) is an entrapment neuropathy that involves the compression of the median nerve at the wrist and is considered the most common of all focal entrapment mononeuropathies. CTS makes up 90% of all entrapment neuropathies diagnosed in the USA and affects millions of Americans. RECENT FINDINGS: Age and gender likely play a role in the development of CTS, but additional studies may further elucidate these associations. Of known associated risk factors, diabetes mellitus seems to have the greatest association with CTS. One of the most commonly reported sy…

Differential Diagnosis of Churg-Strauss Syndrome with Tingling Sensation and Weakness of the Lower Extremities.

INTRODUCTION: Churg-Strauss syndrome (CSS; or eosinophilic granulomatosis with polyangiitis) is an extremely rare autoimmune condition chara … gic disease and the possibility of malignancy. However, a nerve conduction study suggested multiple mononeuropathies. In addition, his laboratory findings showed peripheral eosinophilia and positivity for myeloperoxidase anti-neutrophil cytoplasmic autoantibody. Therefore, CSS was strongly suspected. CONCLUSIONS: Churg-Strauss syndrome is considered to be a condi …

Compression of Root Level in a Patient with Hereditary Neuropathy with Liability to Pressure Palsy Diagnosed by Magnetic Resonance Imaging.

Hereditary neuropathy with liability to pressure palsies (HNPP) is characterized by acute, painless, and recurrent mononeuropathies that are secondary to compression or minor trauma. This case is the first to report an intraspinal compression of the radicular nerve by schwannoma in a patient with HNPP. A 66-year-old woman developed left foot drop and paresthesia of the lateral aspects of left distal lower leg. An electromyography showed left L5 radiculopathy and severe peripheral polyneuropathy. A lumbosacral magnetic resonance imaging revealed a radicular nerve in the intradural and extramedu…

Hereditary neuropathy with liability to pressure palsy: a recurrent and bilateral foot drop case report.

Hereditary neuropathy with liability to pressure palsy is characterized by acute, painless, recurrent mononeuropathies secondary to minor trauma or compression. A 16-year-old boy had the first episode of right foot drop after minor motorcycle accident. Electromyography revealed conduction block and slowing velocity conduction of the right deep peroneal nerve at the fibular head. After motor rehabilitation, he fully recovered. Six months later he had the second episode of foot drop in the opposite site after prolonged squatting position. Electromyography revealed sensorimotor polyneuropathy of …

Predicting recovery after fibular nerve injury: which electrodiagnostic features are most useful?

OBJECTIVE: Although it is important to have strong predictors of outcome in peripheral mononeuropathies so that appropriate management can be instituted early, little is known about the prognostic value of electrodiagnostic results for these lesions. Our objective in this study was to evaluate the prognostic value of electrodiagnostic studies in fibular neuropathy. DESIGN: In this retrospective study, 39 of 138 subjects with fibular neuropathy met the inclusion criteria. Electrodiagnostic results at the time of testing were evaluated for their value in predicting outcome. Good outcome was def…

Neurologic complications after surgery for obesity.

Bariatric surgical procedures are increasingly common. In this review, we characterize the neurologic complications of such procedures, including their mechanisms, frequency, and prognosis. Literature review yielded 50 case reports of 96 patients with neurologic symptoms after bariatric procedures. The most common presentations were peripheral neuropathy in 60 (62%) and encephalopathy in 30 (31%). Among the 60 patients with peripheral neuropathy, 40 (67%) had a polyneuropathy and 18 (30%) had mononeuropathies, which included 17 (94%) with meralgia paresthetica and 1 with foot drop. Neurologic …
