The treatment with or without prednisolone in adults aged ≥ 70 years with myasthenia gravis: a retrospective cohort study.

INTRODUCTION: This retrospective, single-center cohort study aimed to compare clinical outcomes, including time to minimal manifestations (MM)-defined as the absence of functional limitations from MG symptoms in daily activities, even with mild muscle weakness on examination-or better status, between patients aged ≥70 years with MG treated with PSL alone or with calcineurin inhibitors (CNIs), and those treated with CNI alone. METHODS: Twelve patients with acetylcholine receptor antibody (AChR-Ab)-positive MG aged ≥70 years were included (the PSL group, n = 7; the CNIs group, n = 5). Changes i

Case Report: Zilucoplan in refractory myasthenic crisis and its prodromal stages: a novel therapeutic perspective.

Myasthenic crisis (MC) represents life-threatening respiratory failure in myasthenia gravis (MG), while impending myasthenic crisis (iMC) denotes rapidly worsening weakness threatening ventilation however the condition is reversible with timely intervention. Complement inhibition has emerged as a targeted approach for acetylcholine receptor (AChR) antibody-positive MG. To date, evidence for Zilucoplan, a subcutaneous macrocyclic peptide C5 inhibitor, in acute settings remains limited. This study describes two Italian AChR-positive-MG patients experiencing severe, treatment-refractory exacerbat…

The Safety and Feasibility of Laryngeal Mask Airway Management in Nonintubated Subxiphoid-Subcostal Thoracoscopic Thymectomy: Preliminary Results.

INTRODUCTION: While intubated general anesthesia with single-lung ventilation is regarded as mandatory for conventional thoracoscopic surgery, and various minimally invasive approaches to thymectomy have been described, nonintubated thoracoscopic thymectomy remains technically challenging. This study reports the initial results of nonintubated subxiphoid-subcostal thoracoscopic thymectomy (NI-STT) under laryngeal mask airway management for patients with thymic tumor or myasthenia gravis (MG), and evaluates the feasibility and safety of the procedure. MATERIALS AND METHODS: A retrospective ana…

Research Progress in Pharmacological Targeted Therapy for Myasthenia Gravis.

Myasthenia gravis (MG) is a disorder of neuromuscular junction transmission that is mediated by autoantibodies and depends on T cell help, causing fatigable muscle weakness. Limitations of conventional first-line treatments, including long-term adverse effects and inadequate control of refractory disease, have driven targeted therapy to the forefront of MG research. This narrative review summarizes second-line and later-line targeted biological and cellular therapies, covering agents directedat B and T cells, complement inhibitors, neonatal Fc receptor (FcRn) antagonists and cytokine inhibitor…

Development and Validation of a Nomogram to Predict Minimal Symptom Expression in AChR+ gMG Patients Undergoing Efgartigimod Therapy.

BACKGROUND: Efgartigimod has emerged as an effective treatment option for acetylcholine receptor antibody-positive generalized myasthenia gravis (AChR+ gMG). This study aimed to develop and validate a predictive nomogram to evaluate the achievement of Minimal Symptom Expression (MSE) in AChR+ gMG patients following efgartigimod treatment. METHODS: We retrospectively reviewed 119 patients with AChR+ gMG who received at least one cycle of efgartigimod and had ≥ 12 weeks of follow-up. Clinical response was defined as MSE, referring to an MG-ADL score of 0 or 1 within 4 weeks of treatment initiat…

Fulminant myocarditis and sudden death associated with thymoma-related autoimmunity.

Fulminant myocarditis (FM) is a severe, rapidly progressive and life-threatening condition of myocarditis associated with infectious or autoimmune etiologies. Here, we present three patients with an invasive thymoma who had FM, which eventually led to sudden death. The most frequent clinical presentations at onset include palpitations, orthopnea, and acute worsening of myasthenia gravis (MG). Highly elevated myocardial biomarkers and positive autoantibodies against cardiac muscle, skeletal muscle, and neuromuscular junctions were detected in serum. The electrocardiograph (ECG) findings progres…

Acetylcholine Receptor Antibody Positivity, RNST Abnormality, and Thymoma Predict Generalization in Ocular Myasthenia Gravis: A South Korean Cohort Study.

Background: Ocular myasthenia gravis (OMG) can remain restricted to ocular symptoms or progress to generalized myasthenia gravis (GMG). However, the rate of generalization and its predictors may vary across populations, and data on South Korean patients remain inadequate. This study investigated the conversion rate from OMG to GMG and identified clinical, serologic, electrophysiologic, and thymic predictors of this generalization in a South Korean cohort. Methods: This retrospective cohort study included 89 patients who initially presented with OMG and were followed up for at least 2 years. Th…

Lipidomic Profiling Reveals Distinct Molecular Signatures Across Clinical Subtypes of Myasthenia Gravis.

BACKGROUND/OBJECTIVES: Myasthenia gravis (MG) is an immune-mediated neuromuscular disorder for which antibody-based assays have limited sensitivity, particularly in double-seronegative MG (dsNMG), highlighting the need for complementary biomarkers. Given their roles in immune regulation, membrane integrity, and metabolic stress responses, lipids represent promising candidates for biomarker discovery. METHODS: We designed a prospective case-control study and systematically stratified 68 patients with myasthenia gravis (MG) according to clinical classification and autoantibody status. Using LC-…

Rapid Onset of Clinical Response to Eculizumab in a Single-Center Cohort of Older Patients with Refractory Generalized Myasthenia Gravis.

BACKGROUND: Current treatments for myasthenia gravis (MG), while improving outcomes, are often associated with adverse effects, and a proportion of patients remain refractory to standard therapies. Eculizumab, a humanized monoclonal antibody targeting complement protein C5, is approved for refractory anti-acetylcholine receptor antibody-positive generalized MG (AChR-Ab+ gMG). Aim of the present study is to explore the rapidity of onset of eculizumab efficacy in a single-center cohort of MG patients. METHODS: This retrospective, observational, single-center study evaluated the real-world effec…

Association of Baseline Depressive Tendency with Post-Counseling Clinical Status in Patients with Thymoma-Associated Myasthenia Gravis.

Background: Depressive symptoms are highly prevalent among patients with thymoma-associated myasthenia gravis (MG) and are associated with poorer clinical status and quality of life. However, their relationship with psychological counseling outcomes remains insufficiently investigated. Objectives: This study aimed to examine the association between baseline depressive tendency, baseline clinical status, and post-counseling clinical outcomes in patients with thymoma-associated MG. Methods: This single-group pre-post exploratory pilot study enrolled 17 patients with thymoma-associated MG who rec…

Diaphragmatic ultrasound could unmask subclinical respiratory dysfunction in non-severe myasthenia gravis: a cross-sectional analysis.

While myasthenic crisis is well-characterized, subclinical respiratory muscle dysfunction in non-severe disease (MGFA Class I-III) remains under-recognized. To evaluate estimated diaphragmatic contractility using ultrasound in non-severe MG, and to determine its association with concurrent pulmonary function and generalized disease severity. 50 patients with confirmed non-severe MG (MGFA Class I-III) underwent concurrent evaluation using B-mode diaphragmatic ultrasound, spirometry and validated clinical scales (QMG, MG-ADL). Diaphragmatic thickness was measured at end-expiration (Tmin) and max…

Single-cell transcriptomic analysis reveals systemic immune dysregulation and identifies PLEK as a candidate hub gene associated with CD8+ T-cell activation in myasthenia gravis.

Myasthenia gravis (MG) is an autoimmune disorder characterized by antibody-mediated neuromuscular dysfunction, but the systemic immune landscape and underlying mechanisms remain unclear. Here we analyzed single-cell RNA-seq data from peripheral blood mononuclear cells of MG patients and healthy controls to characterize immune cell composition, transcriptional programmes and intercellular communication. Among 206,472 high-quality cells, we observed significant immune dysregulation in MG, including expansion of innate populations, reduction of naïve T cells, and increases in memory T cells and i…
