Global Trends in Neonatal Sepsis Mortality and Attributable Risk Factors From 1990 to 2021.

OBJECTIVES: To evaluate the trends in mortality for early- and late-onset neonatal sepsis, as well as the attributable risk factors at global, regional and national levels from 1990 to 2021. Early-onset sepsis occurs within 0-6 days after birth, whereas, late-onset sepsis occurs between 7 and 27 days of life. METHODS: Data on deaths from early- and late-onset neonatal sepsis, as well as the proportion attributable to risk factors, was sourced from the Global Burden of Disease Study (GBD) 2021. Temporal trends in mortality rates were assessed using estimated annual percentage changes. RESULTS…

Streptococcus suis-associated neonatal meningitis and sepsis: characterization, antimicrobial resistance, and public health implications.

Neonatal meningitis and sepsis were diagnosed in a 32-day-old preterm-born infant. Streptococcus (S.) suis was isolated from cerebrospinal fluid and blood. Next-generation sequencing revealed that the strain was serotype 2 sequence type 1, and contained the ermB and tet(W) genes, which are responsible for resistance to macrolides and tetracycline, along with several pilus-associated genes and 20 virulence factors. High homology was observed with previously identified human and swine strains in the same area. S. suis meningitis and sepsis are mainly reported in adults, related to direct contact

Modeling pathogen-driven neonatal late-onset sepsis: a modification to the murine cecal slurry.

INTRODUCTION: Neonatal sepsis is a major cause of neonatal morbidity and mortality. Reliable animal models are essential to our understanding of late-onset sepsis, but notable limitations exist in the current standard murine cecal slurry model. We sought to refine the existing model by using an injection of known stock slurry ("NEC'teria") cultured from an infant who died of necrotizing enterocolitis to better mimic sepsis following the translocation of neonatal specific bacterial pathogens from the intestine into the peritoneum. METHODS: To induce sepsis, neonatal mice (P7 and P14 - P16) wer…

Early detection of neonatal sepsis and reduction of overall antibiotic exposure: Towards precision medicine.

Infections claim the lives of over half a million newborns annually and expose survivors to the risk of lifelong disability. The challenge to clinicians is to identify newborns with invasive infections rapidly, promptly initiate antimicrobial treatment, and take measures to prevent and treat organ dysfunction. Moreover, excessive antibiotic use is a global public health problem. Despite considerable research on clinical and laboratory markers of neonatal sepsis, the effective translation into clinical practice remains limited. There is no single clinical or laboratory marker, nor any combinati…

Rare case of early neonatal sepsis caused by Candida krusei successfully treated with voriconazole.

This study reports a case of multidrug resistant Candida krusei as the cause of early neonatal sepsis in a term small-for-gestational age neonate weighing 1680 g that successfully responded to voriconazole therapy. Both blood culture and urine culture of the neonate sent on day 4 and day 8 respectively showed Gram positive oval budding yeast cells on Gram staining which was confirmed as C. krusei susceptible only to voriconazole by Vitek 2 Compact (Biomérieux, France) automated system. Voriconazole was given for fourteen days leading to good clinical response with microbiological clearance of …

Involvement of IL-1β-Mediated Necroptosis in Neurodevelopment Impairment after Neonatal Sepsis in Rats.

The mechanism of long-term cognitive impairment after neonatal sepsis remains poorly understood, although long-lasting neuroinflammation has been considered the primary contributor. Necroptosis is actively involved in the inflammatory process, and in this study, we aimed to determine whether neonatal sepsis-induced long-term cognitive impairment was associated with activation of necroptosis. Rat pups on postnatal day 3 (P3) received intraperitoneal injections of lipopolysaccharide (LPS, 1 mg/kg) to induce neonatal sepsis. Intracerebroventricular injection of IL-1β-siRNA and necrostatin-1 (NEC1…

Let's Not Miss the Treatable Ones: Two Cases of Neonatal Sepsis Due to Malaria.

Congenital malaria is the direct infection of an infant with a malarial parasite from the mother either during pregnancy or at birth. Neonatal malaria occurs due to an infective mosquito bite after birth. Neonatal and congenital malaria (NCM) can occasionally present with life-threatening neonatal sepsis and rarely with neonatal jaundice. These conditions are typically managed by general pediatricians, especially in remote areas without access to specialized care. A high clinical index of suspicion is required to diagnose neonatal and congenital malaria, given that their presentation can mimic…

Clinical significance of the serum miR-455-5p expression in patients with neonatal sepsis.

Sepsis is a systemic inflammatory response caused by infection and is a major cause of neonatal death. This study explored the miR-455-5p in neonatal sepsis, and further investigated the diagnostic and prognostic value of miR-455-5p in neonatal sepsis (NS). The levels of serum miR-455-5p in 88 healthy controls and 90 NS patients were examined by quantitative real-time polymerase chain reaction (qRT-PCR). Pearson correlation coefficient was used to evaluate the correlation between miR-455-5p and clinical features. Receiver operating characteristic (ROC) curve analysis was performed for the diag…

First report of neonatal early-onset sepsis caused by multi-drug-resistant Raoultella ornithinolytica.

Raoultella ornithinolytica is increasingly being isolated as a causative organism in human infections. Most of the infections caused by R. ornithinolytica are hospital acquired and occur in patients who are immunocompromised, had invasive procedures or have indwelling catheters. This is a first report of early onset neonatal sepsis caused by multi-drug-resistant R. ornithinolytica. The infection was not very severe and was characterised by generalized flushing of the skin. Patient made complete recovery once appropriate antibiotics were started.

Evaluation of IL-6, CRP and hs-CRP as Early Markers of Neonatal Sepsis.

INTRODUCTION: Bacterial sepsis is a life threatening crisis with high mortality and morbidity in neonates. Due to non-specific clinical presentation, diagnosis of sepsis is still a challenge. It can be diagnosed by blood culture but it is time consuming. So, a reliable marker is needed for the diagnosis of neonatal sepsis so that early treatment can be initiated. Various cytokines, chemokines, acute phase reactants, cell surface markers and interferons have been evaluated to find out the effective marker for early diagnosis of neonatal sepsis. In this study, levels of IL-6, CRP and hs-CRP have…

Glucose-6-phosphate dehydrogenase deficiency (G6PD) as a risk factor of male neonatal sepsis.

Introduction.Neonatal sepsis is a disease process, which represents the systemic response of bacteria entering the bloodstream during the first 28 days of life. The prevalence of sepsis is higher in male infants than in females, but the exact cause is unknown. Glucose-6-phosphate dehydrogenase (G6PD) is an enzyme in the pentose phosphate pathway, which leads to the production of NADPH. NADPH is required for the respiratory burst reaction in white blood cells (WBCs) to destroy microorganisms. The purpose of this study was to evaluate the prevalence of G6PD deficiency in neonates with sepsis. Ma…

Adaptation to vancomycin pressure of multiresistant Staphylococcus capitis NRCS-A involved in neonatal sepsis.

OBJECTIVES: The Staphylococcus capitis clone NRCS-A has recently been described as a frequent cause of late-onset sepsis (LOS) in pre-term neonates worldwide. Representatives of this clone exhibit non-susceptibility to vancomycin, the first-line agent used in LOS. Cases of prolonged S. capitis LOS despite vancomycin treatment have been reported. We investigated whether NRCS-A strains exhibit faster adaptation to vancomycin pressure as compared with other staphylococci. METHODS: Strains of S. capitis NRCS-A, S. capitis non-NRCS-A and Staphylococcus epidermidis (n = 2 each, all with vancomycin

TRIF-dependent innate immune activation is critical for survival to neonatal gram-negative sepsis.

Current evidence suggests that neonatal immunity is functionally distinct from adults. Although TLR signaling through the adaptor protein, MyD88, has been shown to be critical for survival to sepsis in adults, little is known about the role of MyD88 or TRIF in neonatal sepsis. We demonstrate that TRIF(-/-) but not MyD88(-/-) neonates are highly susceptible to Escherichia coli peritonitis and bacteremia. This was associated with decreased innate immune recruitment and function. Importantly, we found that the reverse was true in adults that MyD88(-/-) but not TRIF(-/-) or wild-type adults are su…

Role of polymerase chain reaction as an early diagnostic tool for neonatal bacterial sepsis.

BACKGROUND: Neonatal bacterial sepsis is a challenging disease that needs to be detected early. As it is a life-threatening condition, the use of an approach that could be more rapid than standard culture and identification techniques for detection of neonatal sepsis would be highly desirable. OBJECTIVE: The aim of this work was to assess the effectiveness of the PCR technique compared with blood culture for the early detection of bacterial sepsis. PARTICIPANTS AND METHODS: This study included 50 neonates with suspected sepsis. A blood sample was collected and divided into two parts: one par…
