Lipofibromatous Hamartoma of the Median Nerve - A Rare Condition in the Hand.

Lipofibromatous hamartoma (LFH) of the median nerve is a rare condition in the hand and often remains asymptomatic for a significant period. MRI imaging can reveal unique tumour characteristics; however, the definitive diagnosis is confirmed through a tissue biopsy. In this report, a 38-year-old male presented with a gradually growing mass on his right hand. Physical examination revealed a large soft tissue mass extending from the thenar area to the wrist, causing compression of the median nerve. MRI confirmed the presence of a distinct soft tissue mass on the volar side of the hand. The mass

The oxidative response in the chronic constriction injury model of neuropathic pain.

BACKGROUND: In the chronic constriction injury model of rat neuropathic pain, oxidative stress as well as antioxidants superoxide dismutase and reduced glutathione (GSH) are important determinants of neuropathological and behavioral consequences. Studies of the chronic constriction injury model observed (indirect) signs of inflammation. We, therefore, investigated the level of oxidative stress and antioxidant enzymes in skeletal muscle tissue of the rat hind paw and (jugular vein) plasma at d 7 after nerve injury. MATERIALS AND METHODS: The level of reactive oxygen and nitrogen species (RONS)

Optical stimulation of the facial nerve: a new monitoring technique?

OBJECTIVES/ HYPOTHESIS: One sequela of skull base surgery is iatrogenic damage to cranial nerves, which can be prevented if the nerve is identified. Devices that stimulate nerves with electric current assist in nerve identification. Contemporary devices have two main limitations: 1) the physical contact of the stimulating electrode and (2) the spread of the current through the tissue. In contrast to electrical stimulation, pulsed infrared optical radiation can be used to safely and selectively stimulate neural tissue and might be valuable for screening. METHODS: The gerbil facial nerve was ex

Ultrasound elastography for the evaluation of peripheral nerves: A systematic review.

Peripheral nerve disorders are commonly encountered in clinical practice. Electrodiagnostic studies remain the cornerstone of the evaluation of nerve disorders. More recently, ultrasound has played an increasing complementary role in the neuromuscular clinic. Ultrasound elastography is a technique that measures the elastic properties of tissues. Given the histological changes that occur in diseased peripheral nerves, nerve ultrasound elastography has been explored as a noninvasive way to evaluate changes in nerve tissue composition. Studies to date suggest that nerve stiffness tends to increas…

Engineering Silk Fibroin-Based Biomaterials for Neural Repair.

Neurological injuries remain a major clinical challenge due to the limited regenerative capacity of neural tissue, the persistence of inhibitory post-injury microenvironments, and the lack of biomaterials capable of simultaneously providing structural support and biological instruction. Growing evidence highlights that biomaterial-mediated modulation of the neural microenvironment is essential for effective neural regeneration and functional recovery. Silk fibroin (SF), a naturally derived protein biomaterial, has attracted growing interest in neural repair owing to its tunable mechanical prop

Juvenile mandibular chronic osteomyelitis: multimodality imaging findings.

BACKGROUND AND PURPOSE: Juvenile mandibular chronic osteomyelitis is a rare entity that predominantly affects children and adolescents, but little is known about the factors that contribute to the recurrent course and eventual resolution of this disease. Here, we describe new findings of soft tissue and mandibular nerve canal involvement. MATERIALS AND METHODS: Four patients with mandibular diffuse sclerosing osteomyelitis are presented; all were followed with CT, a few also with MRI and bone scan. We recorded imaging findings of lesion location, pattern of bone formation, presence and evolut

Lymphoid tissue in the kidney of the musk shrew, Suncus murinus.

We describe a lymphoid tissue in the kidney of the musk shrew, Suncus murinus. An anatomically well organised lymphoid tissue, resembling the mucosa-associated lymphoid tissue, is associated with the epithelium of the renal pelvis and the ureter, respectively. Lymphoid tissue distributed along the arcuate artery and arcuate vein is not structurally organised in centre and periphery. This tissue type is most prominently developed between blood vessels. Immunocytochemistry revealed S-100-immunoreactive dendritic cells in both, structurally organised and structurally non-organised lymphoid tissue

Substance P immunoreactive nerve fibres in the domestic chick ankle joint before and after acute urate arthritis.

Substance P immunoreactive (SP-IR) nerve fibres were identified using fluorescence immunohistochemistry and the length of the labelled individual nerve fibres was measured using confocal microscopy of whole mounts of the lateral wall of the ankle joint in 2-week-old domestic chicks. In the normal ankle there was an extensive network of SP-IR fibres in the synovial and subsynovial tissue of the joint capsule. Fours hours after injection of sodium urate into the joint space the joint was inflamed and there was a significant reduction in the labelled nerve fibres. These results demonstrate experi

Extrusion of a NeuroTube: A Case Report.

BACKGROUND: Peripheral nerve injury is a common result of trauma. In cases of nerve gap, treatment may involve placement of a nerve conduit. This case involves a polyglycolic acid nerve conduit tube that was extruded through soft tissue. To our knowledge, this reactive process has only been previously documented in one article. This complication is not commonly known among hand surgeons, hence our interest in documenting it. CASE REPORT: We present the case of a 33-year-old male who injured his right nondominant thumb in a workplace saw accident. His complex wound involved the radial digital

Nerve fiber planimetry in acute and chronic nerve lesions and in nerve lesions in continuity.

The level of resection of damaged nerve tissue in acute and chronic nerve lesions was determined on the basis of the vascular structure, the consistency of the nerve during palpation, the amount of interfascicular connective tissue, and the mushroom formation of the fascicles. Intraoperative electrophysiologic recordings were performed on the cut nerve ends to determine the function of the axons. Postoperative planimetric analyses of cross sections made through the resected nerve stumps were performed to measure axonal and endoneural tube diameters and to correlate these results with the clini…

Multi-modality neurophysiological monitoring during surgery for adult tethered cord syndrome.

During complex microneurosurgery performed in patients with tethered cord syndrome, the conus medullaris and the roots that innervate the lower limbs, bladder and bowel are potentially exposed to damage. The aim of multimodality intraoperative monitoring (IOM) is to reduce the risk of inadvertent injury of neural tissue. We simultaneously record tibial nerve somatosensory evoked potentials (SSEPs) from the scalp and free run electromyography (EMG) of limb muscles supplied by L2 to S2 roots, anal and urethral sphincters. We also identify critical neural structures in the operative field, includ

Lack of Mucosal Cholinergic Innervation Is Associated With Increased Risk of Enterocolitis in Hirschsprung's Disease.

BACKGROUND & AIMS: Hirschsprung's disease (HSCR) is a congenital intestinal motility disorder defined by the absence of enteric neuronal cells (ganglia) in the distal gut. The development of HSCR-associated enterocolitis remains a life-threatening complication. Absence of enteric ganglia implicates innervation of acetylcholine-secreting (cholinergic) nerve fibers. Cholinergic signals have been reported to control excessive inflammation, but the impact on HSCR-associated enterocolitis is unknown. METHODS: We enrolled 44 HSCR patients in a prospective multicenter study and grouped them accordin

Ultrastructure of perineurial cells during peripheral nerve regeneration. Electron microscopical investigations on the so-called amputation neuroma.

The ultrastructural morphology of perineurial cells was examined after transsection of the sciatic nerve of rats. The material prepared for … ution of a diffusion barrier the perineurial cells may exert a stabilizing function in regenerating nerve tissue comparable to myofibroblasts in granulation tissue, and may be engaged in remodeling of connective tissue fibers. The relations of perineurial cells to other cells of peripheral nerves under the condition of regeneration are discussed.

[Neurovascular autoregulation of the regeneration of nerves in the upper extremities].

Functional features of tissue microcirculation in injury and regeneration of nerves were revealed from analysis of 440 polarograms and capillarograms recorded in 30 healthy individuals and 70 patients with sequelae of injury to the median and ulnar nerves. The initial regeneration of the nerve trophic function proper increases the level of microcirculation and oxygenation of the tissues and thus prepares them for the subsequent reinnervation and is conducive to the trophics of the penetrating axons. "Neurovascular autoregulation" of nerve regeneration takes place. A microcirculatory approach t

Anatomy of the neural fibers at the superior mesenteric artery-a cadaver study.

PURPOSE: Most surgeons perform right-sided semicircular clearance of the superior mesenteric artery (SMA) nerve plexus for pancreatic head carcinoma, presuming a linear course of the SMA nerve fibers. The hypothesis was that the SMA nerve plexus fibers follow a non-linear course, and the goal of the present study was to assess the neural fibers distribution along the SMA. METHODS: The course of neural fibers along the retropancreatic and suprapancreatic SMA was assessed in 7 cadavers. RESULTS: In the retropancreatic course of the vessel, the main nerve cords branch and form a large number of
