High-resolution ultrasound has become increasingly useful in the evaluation of peripheral nerves. As we have gained experience, clinically helpful ultrasonographic signs have been detected. One of these is the ultrasonographic Tinel sign, in which paresthesias are produced when the transducer compresses a nerve lesion. This sign has not been previously described. We present a case in which the ultrasonographic Tinel sign helped accurately guide a nerve biopsy in a woman with mononeuritis multiplex.

OBJECTIVES: To report a case of a teenage boy (N.K.) who had a peroneal nerve palsy after an isolated repetitive stain injury from an episode of sustained repetitive kicking and to explore the emergency physician's approach to footdrop or absent dorsiflexion.
METHODS: Chart review.
RESULTS: N.K. presented to the emergency department (ED) with right footdrop, inability to dorsiflex. Results of investigations in the ED, including radiographs of his lumbar spine and knees and a magnetic resonance imaging scan of the lumbar spine, were all normal. Electromyography showed that the extensor digito

Miller Fisher syndrome (MFS) is a localized variant of Guillain-Barré syndrome (GBS), characterized by ophthalmoplegia, areflexia, and ataxia. Recent neurophysiological studies have suggested that abnormal terminal axon dysfunction occurs in some cases of Miller Fisher syndrome and Guillain-Barrd syndrome. We present a rare case report of recurrent MFS with abnormal terminal axon dysfunction. To the best of our knowledge, this is the first case report of recurrent MFS with terminal axon dysfunction that persisted up to nine months after the initial presentation of the second attack with positi

Progressive loss of motor neurons causes Amyotrophic Lateral Sclerosis. Patients complain, most often, of progressive weakness in the distal limbs. However, weakness may manifest in any body segment (bulbar, cervical, thoracic, or lumbosacral). The diagnosis of ALS is suggested by clinical examination that reveals both upper and lower motor neuron failure. Formal diagnostic criteria have been developed and validated. Nerve conduction and electromyography studies improve diagnostic sensitivity and exclude some alternate, treatable diagnoses. Likewise, conventional imaging studies and laboratory

Multifocal motor neuropathy with persistent conduction blocks was firstly reported in 1986 and outlined from the group of purely motor diseases of the peripheral nervous system. The main criterion is the presence of conduction blocks located only on the motor nerves; additionally 30 percent of patients have IgM subclass serum antibodies directed against GM1 ganglioside. The clinical picture is a multifocal, asymmetrical, neuropathy, starting and predominant in the upper limbs, occurring in males aged 50 years and more, and having a progressive course. There is no biological sign besides elevat

We present here the first known case of an unobtainable radial nerve F-wave in a case of radial nerve conduction block. This case further demonstrates that F-waves are used not only to rule out neuropathy or radiculopathy, but also to detect conduction block.

Motor conduction velocity (CV) was tested in 117 infants and children with proximal spinal muscular atrophy (SMA), aged 1-53 months, and in 99 age-adjusted healthy controls. The children were classified into SMA forms 1 and 2 according to SMA International Consortium Criteria. In all children CV was tested in four nerves and the following parameters were taken into account: distal latency, conduction velocity, amplitude of muscle response. The electromyography of muscles revealed neurogenic changes. In all cases of severe form 1 motor CV was markedly slowed, with significantly prolonged distal

A delayed syndrome of progressive weakness has been described in survivors of paralytic poliomyelitis - "Post-Polio Muscular Atrophy (PPMA)". One proposed etiology is a drop-out of motor neurons due to increased metabolic demands of an enlarged motor unit territory. We report a patient with slowly progressive lower extremity weakness 20 years after recovery from an episode of myelopathy involving the lower lumbar and sacral segments of the spinal cord. Delayed progressive amyotrophy may complicate any significant injury to anterior horn cells.

STUDY DESIGN: Nerve conduction velocity was studied in the dog cauda equina subjected to chronic double-level compression.
OBJECTIVES: To analyze the effects of chronic double-level cauda equina compression.
SUMMARY OF BACKGROUND DATA: Double-level cauda equina compression produces more symptoms in patients and more changes in acute experimental set-ups than does single-level compression. However, there have been no controlled, experimental studies on chronic double-level compression.
METHODS: A total of 20 dogs were anesthetized. Two balloons were placed under the lamina of the seventh lum

Diaphragm pacing is an established mode of ventilation for patients with upper motor neuron injury and preserved phrenic nerve function. Careful patient evaluation with regard to phrenic nerve function, motivation, and adequate psychosocial support is paramount for successful pacing. In properly selected individuals, full-time continuous bilateral pacing for several years has been demonstrated with advantages of increased independence and productivity, fewer tracheal tube complications, and improved phonation. Ongoing research in the field of diaphragm pacing includes refinements in electrode

All disorders of neuromuscular transmission (NMT) may cause ventilatory failure, albeit rarely. Respiratory muscle weakness is occasionally the presenting feature of myasthenia gravis (MG), the Lambert-Eaton myasthenic syndrome (LEMS), hypermagnesemia and botulism. Chronic MG, congenital myasthenic syndromes and LEMS may be acutely exacerbated by various intercurrent conditions and by drugs which interfere with NMT. Finally, in the ICU, difficulty in weaning from the ventilator may be caused by prolonged use of neuromuscular blocking agents. Electrophysiological studies of NMT disorders in the

The purpose of this study was to investigate gait-pattern changes after complete tibial nerve lesion in the cat, and to observe whether nerve repair could reverse some of the changes. In six cats, a 5-cm segment of the tibial nerve was transected. The nerve gap was then repaired with nerve autograft in three animals and was unrepaired in three as controls. The walking patterns of the cats were videotaped, and the hip, knee, ankle, and metatarsophalangeal joint angles were measured at the beginnings of the F, E1, E2, and E3 phases of the step cycle. Two weeks after surgery, abnormal gait patter

STUDY DESIGN: Nerve conduction velocity was studied in the dog cauda equina subjected to chronic and, 1 week later, additional acute compression.
OBJECTIVES: To model the situation of spinal stenosis by first inducing a controlled, chronic compression injury and then to add compression at various pressure levels to the already compressed cauda equina.
SUMMARY OF BACKGROUND DATA: Previous models on nerve root and cauda equina compression have analyzed the effects of compression only on noninjured nerves. Clinically, the nerves that are compressed in spinal stenosis already are exposed to chro

Ulnar nerve entrapment at the wrist (UNW) is uncommon and often difficult to localize electrophysiologically. The difference between the motor latencies to the median-innervated second lumbrical (2L) and ulnar-innervated palmar interosseous (Pl) (Diff 2L-Pl) has been shown to be of localizing value in patients with median neuropathy at the wrist. In the last year, we evaluated 2 patients with clinically definite ulnar neuropathy at the wrist. We performed motor studies to the 2L-Pl on the 2 patients and 12 disease controls with ulnar neuropathy at the elbow as follows: Using the same electrode

The incidence of carpal tunnel syndrome is increased during pregnancy. The common conventional therapeutic approach is conservative, as symptoms usually abate after delivery. We describe our experience with 65 hands (50 patients), who were treated initially by a conservative approach and later, when required, surgically. We found that all patients who (i) had either started having CTS symptoms during the first two trimesters or had previous history of CTS symptoms; and (ii) had both a positive Phalen test within less than 30 seconds and abnormal two point discrimination at the finger tips ( >

In multifocal motor neuropathy (MMN), little is known of the long-term effect of human immunoglobulin (HIG) infusions as a maintenance therapy. We report a patient who improved after an initial HIG infusion of 2 g/kg and continued to improve both clinically and electrophysiologically during maintenance HIG treatment consisting of one infusion of 0.4 g/kg every week. Our findings suggest that maintenance HIG therapy may lead to further improvement of MMN, and that more frequent HIG infusions at a lower dosage may be more effective.

A 9 year old boy had chronic progressive motor-sensory neuropathy that started in early infancy. He had enlarged nerves and pes cavus deformity. Motor conduction studies showed very dispersed, polyphasic compound muscle action potentials with conduction velocities around 2 m/s. A sural nerve biopsy showed severe loss of myelinated fibres. Two months of treatment with corticosteroids restored muscle power. During this time the enlarged nerves became normal and electrophysiological recovery was achieved. Chronically acquired neuropathy in infancy is strikingly similar to genetically determined n
