Migration of intraventricular neurocysticercus after ventriculostomy.

Intraventricular neurocysticerci are an uncommon occurrence and are often difficult to diagnose. They are often a cause of obstructive hydrocephalus and inflammatory reactions, which can be life threatening. Intraventricular neurocysticerci are most common in the fourth ventricle and surgical decompression of obstructive hydrocephalus is an important step in their management. However, these cysticerci are also known to migrate within the ventricular system and may cause confusion during surgery. It thus becomes very important for radiologists and neurosurgeons to be aware of this phenomenon.

[Features of rhinologic and otoneurologic symptoms and complications in intracranial tumors growing into the nose and paranasal sinuses].

The authors analyse rhinological and otoneurological symptoms in 50 patients with benign craniofacial tumors verified at surgical interventions, computed tomography, NMR-tomography. The tumors involved anterior parts of the brain stem, nasal cavity and paranasal sinuses, 34 tumors were diagnosed as meningiomas. Clinical correlations with tumor histology and topography were noted. Specific ENT preoperative, postoperative and long-term complications are described. The diagnosis and treatment of the above patients need cooperation with specialists of the allied fields: neurosurgeons, neuroroentge…

Usefulness of sagittal computed tomography in neurosurgery.

Sagittal section tomographic images can be obtained either directly or by an electronic reconstructing procedure. These two techniques are extremely useful for neurosurgeons because it is possible to obtain a better clinical and pathological evaluation of the lesion. This procedure is almost recommended in case of pituitary adenoma, cerebello-pontine angle neoplasms, orbital lesions and brain-stem lesions. The authors show the results of the experience of the University of Pavia and discuss the true indications to this neuroradiological technique.

Nordic survey on practice of neurosurgical management of craniopharyngioma in children.

PURPOSE: Management of childhood-onset craniopharyngioma (CP) may be challenging, given its rarity and complexity as well as the risks assoc … ategies, as well as varying degrees of centralization and collaboration between pediatric and adult neurosurgeons. Although some practical aspects may be unique for healthcare practised in the Nordic countries, the results of this survey and, in particular, the embedded discussion of centralization - either restricted to surgery or including complete management …

Microsurgical Management of Blister-Type Basilar Artery Apex Region Aneurysms: Companion Cases Demonstrate Technical Nuances of the Subtemporal Approach: 2-Dimensional Operative Video.

The proportion of intracranial aneurysms treated by microsurgical clip ligation has drastically decreased in the endovascular era. However, some aneurysms cannot be treated by current endovascular techniques. Therefore, trainees and young vascular neurosurgeons must develop and maintain microsurgical skills to safely treat aneurysms that require surgery. Ruptured, basilar artery apex, blister-type aneurysms are particularly treacherous and require a high degree of skill to safely manage them surgically. In this video, 2 companion cases are exhibited to demonstrate the nuances of the subtempora…

Safety and Outcome of Intracranial Pressure Monitor Placement Performed by Trauma Critical Care Surgeons.

INTRODUCTION: The Brain Trauma Foundation advises intracranial pressure monitor placement (ICPM) following traumatic brain injury (TBI) with a Glasgow Coma Scale (GCS) score ≤8 and an abnormal head computed tomographic scan (CT) finding. Prior studies demonstrated that ICPMs could be placed by non-neurosurgeons. We hypothesized that ICPM placement by trauma critical care surgeons (TCCS) would increase appropriate utilization (AU), decrease time to placement (TTP), and have equivalent complications to those placed by neurosurgeons. METHODS: We retrospectively reviewed medical records of adult …

An alternative technique for pituitary tumour removal: Sequential endoscopic-microscopic pituitary procedure (SEMPP).

Several authors have reported advantages of the purely endoscopic (PE) approach over traditional purely microscopic (PM) techniques for many sinus and anterior skull base procedures. However, in pituitary surgery, the PE approach carries a number of limitations. Experienced microscopically-trained neurosurgeons are required to master the endoscopic technique which has a steep learning curve due to its one-handed nature and optically-distorted 2-dimensional view. We describe our novel technique, the sequential endoscopic and microscopic pituitary procedure (SEMPP) which does not require microsc…

Skull Base Venous Anatomy Associated with Endoscopic Skull Base Neurosurgery: A Literature Review.

An understanding of the anatomy and function of the skull base venous channels is fundamentally important for safe performance of endoscopic surgery for skull base lesions. This review focuses on 4 skull base venous channels-namely, the intercavernous sinuses, basilar plexus, osseous veins, and inferior petroclival vein-because these have been less recognized by neurosurgeons than other major venous channels such as the inferior petrosal sinus. We reviewed the literature concerning these 4 venous channels and discussed their anatomy and function. The literature review was mainly based on an el…

Single-staged resections and 3D reconstructions of the nasion, glabella, medial orbital wall, and frontal sinus and bone: Long-term outcome and review of the literature.

BACKGROUND: Aesthetic facial appearance following neurosurgical ablation of frontal fossa tumors is a primary concern for patients and neurosurgeons alike. Craniofacial reconstruction procedures have drastically evolved since the development of three-dimensional computed tomography imaging and computer-assisted programming. Traditionally, two-stage approaches for resection and reconstruction were used; however, these two-stage approaches have many complications including cerebrospinal fluid leaks, necrosis, and pneumocephalus. CASE DESCRIPTION: We present two successful cases of single-stage …

Temozolomide and pasireotide treatment for aggressive pituitary adenoma: expertise at a tertiary care center.

Aggressive pituitary adenomas (PAs) are clinically challenging for endocrinologists and neurosurgeons due to their locally invasive nature and resistance to standard treatment (surgery, medical or radiotherapy). Two pituitary-directed drugs have recently been proposed: temozolomide (TMZ) for aggressive PA, and pasireotide for ACTH-secreting PA. We describe the experience of our multidisciplinary team of endocrinologists, neurosurgeons, neuroradiologists, oncologists, otolaryngologists and pathologists with TMZ and pasireotide treatment for aggressive PAs in terms of their radiological shrinkag…

Management of lumbar spinal stenosis: a survey among Dutch spine surgeons.

BACKGROUND: Various surgical and non-surgical treatments for lumbar spinal stenosis (LSS) are widely adopted in clinical practice, but high quality randomised controlled trials to support these are often lacking, especially in terms of their relative benefit and risk compared with other treatment options. Therefore, an evaluation of agreement among clinicians regarding the indications and the choice for particular treatments seems appropriate. METHODS: One hundred and six Dutch neurosurgeons and orthopaedic spine surgeons completed a questionnaire, which evaluated treatment options for LSS an…

James L. Poppen and surgery of the "seat of the soul": a contemporary perspective.

Dr. James Leonard Poppen (1903-1978) was one of the most renowned American neurosurgeons of the 20th century. The now eponymous Poppen approach to the pineal region is still used routinely in current neurosurgical practice. He was also one of the first to describe and practice the prefrontal lobotomy and appears to be one of the surgeons in the case of Eva Peron in 1952. Poppen was born in a Dutch family on February 28, 1903, in the town of Drenthe, Michigan. Poppen described the occipital transtentorial approach to the pineal region, this operative technique to access a deep-seated area was n…

Hydrocephalic fits: forgotten but not gone.

BACKGROUND: Hydrocephalic fit is a fatal complication of posterior fossa tumors that is uncommon in modern neurosurgical practices. As such, most primary physicians and neurosurgeons lack experience with hydrocephalic fits, which makes some patients vulnerable to this fatal condition. We analyzed the patterns of hydrocephalic fits that we have observed directly in our hospital or indirectly after referral to our institution. METHODS: Eight patients who had a brain tumor and hydrocephalic fit before surgery were managed in our institution between 2001 and 2012. The clinical records and radiolo…
