Comparison of visually evoked peak systolic and end diastolic blood flow velocity using a control system approach.

Transcranial Doppler sonography measures blood flow velocity in basal cerebral vessels with high accuracy. For quantification, time averaged mean blood flow velocities are used most because the peak systolic and end diastolic blood flow velocities mark the velocity extremes of one heart cycle. It is known, from hemodynamic measurements of the neurovascular coupling mechanism, that the end diastolic velocity is more sensitive for change in hemodynamics than the peak systolic velocity. Thus, we used a recently introduced control system approach to compare both indices for their use in functional

Decreased effective connectivity in the visuomotor system after alcohol consumption.

Functional magnetic resonance imaging (fMRI) allows observing cerebral activity not only in separated cortical regions but also in functionally coupled cortical networks. Although moderate doses of ethanol slowdown the neurovascular coupling, the functions of the primary sensorimotor and the visual system remain intact. Yet little is known about how more complex interactions between cortical regions are affected even at moderate doses of alcohol. Therefore the method of psychophysiological interaction (PPI) was applied to analyze ethanol-induced effects on the effective connectivity in the vis

Coupling of cerebral blood flow and oxygen consumption during physiological activation and deactivation measured with fMRI.

The physiological basis of the blood oxygenation level dependent (BOLD) signal and its dependence on baseline cerebral blood flow (CBF) were investigated by comparing responses to a visual stimulus after physiological changes of the baseline. Eight human subjects were imaged with 3 and 4 T MRI scanners, and both BOLD signal and CBF were simultaneously measured. Subjects viewed a flickering radial checkerboard in a block design experiment, alternating between eyes open or closed during the off periods. Compared to a baseline state with eyes open in a darkened room, substantial deactivation (ave

Fatty acid receptor Gpr40 mediates neuromicrovascular degeneration induced by transarachidonic acids in rodents.

OBJECTIVE: Nitro-oxidative stress exerts a significant role in the genesis of hypoxic-ischemic (HI) brain injury. We previously reported that the ω-6 long chain fatty acids, transarachidonic acids (TAAs), which are nitrative stress-induced nonenzymatically generated arachidonic acid derivatives, trigger selective microvascular endothelial cell death in neonatal neural tissue. The primary molecular target of TAAs remains unidentified. GPR40 is a G protein-coupled receptor activated by long chain fatty acids, including ω-6; it is highly expressed in brain, but its functions in this tissue are la

The blood-brain tumor barrier as a spatial immune checkpoint in glioblastoma: leukocyte entry, perivascular retention, and therapeutic implications.

Glioblastoma (GBM) combines heterogeneous vascular dysfunction with a predominantly myeloid immune landscape. This Mini Review focuses on a specific problem: vascular permeability is not equivalent to productive leukocyte access. Entry requires endothelial capture, chemokine-guided arrest, transendothelial migration, release from perivascular sites, intratumoral dispersion, target recognition, and sustained effector function. Human tissue and spatial studies show regionally distinct neurovascular and myeloid states, whereas much of the causal evidence for trafficking and retention remains prec

Epigenetic encoding of metabolic memory in diabetic retinopathy: From molecular imprinting to neurovascular network reprogramming.

Diabetic retinopathy (DR) is increasingly recognized not merely as a microvascular complication, but as a chronic neurodegenerative disorder of the central nervous system characterized by progressive neurovascular unit (NVU) dysregulation. In many individuals, DR continues to progress despite subsequent glycaemic normalization, a phenomenon known as metabolic memory. Emerging evidence indicates that transient hyperglycaemic stress is converted into durable transcriptional programs through epigenetic encoding mechanisms. In this Review, we propose a hierarchical framework in which early metabol

A 3D-1D-0D multiscale model of the neuro-glial-vascular unit for synaptic and vascular dynamics in the dorsal vagal complex.

Cerebral blood flow regulation is critical for brain function, and its disruption is implicated in various neurological disorders. Many existing models do not fully capture the complex, multiscale interactions among neuronal activity, astrocytic signaling, and vascular dynamics, especially in key brainstem regions. In this work, we present a 3D-1D-0D multiscale computational framework for modeling the neuro-glial-vascular unit (NGVU) in the dorsal vagal complex (DVC). Our approach integrates a quadripartite synapse model, which captures the dynamic interactions among excitatory and inhibitory

Treatment Options for Fear of Blushing.

PURPOSE OF REVIEW: To review mechanisms of blushing and fear of blushing from physiological, neuropharmacological and psychological viewpoints, and to evaluate current forms of treatment for blushing-related fear. RECENT FINDINGS: Blushing appears to be driven primarily by sympathetic adrenomedullary and neural vasodilator discharge, possibly in association with secondary neurovascular inflammation. Psychological risk factors for fear of blushing include social anxiety, coupled with heightened self-focused attention and inflated beliefs about the likelihood and social costs of blushing. In ad

Structural causes of ischemic and hemorrhagic stroke in children: moyamoya and arteriovenous malformations.

PURPOSE OF REVIEW: Moyamoya and arteriovenous malformations represent, respectively, significant sources of ischemic and hemorrhagic stroke in children after the first year of life. Although rarely encountered in routine pediatric practice, the potential severe morbidity of these entities, coupled with the typical acuity of their presentation, merit ongoing awareness of current relevant diagnostic and therapeutic strategies. RECENT FINDINGS: Mutations in RNF213, ACTA2, and GUCY are implicated in moyamoya. Several common pediatric conditions - trisomy 21, sickle-cell disease, and neurofibromat

Visualization of the intracisternal angioarchitecture at the posterior fossa by use of image fusion.

OBJECTIVE: Magnetic resonance (MR) images most clearly visualize intracranial tissues but have some limitations in terms of detailed analysis of the intracisternal vasculature. To compensate for these shortcomings, an image fusion of three-dimensional digital subtraction angiography (DSA) and MR images, DSA-MR fusion, has been developed. The goal of this study was to evaluate the usefulness of DSA-MR fusion for the visualization of the intracisternal arteries and veins at the posterior fossa. METHODS: Ten consecutive patients (five with neurovascular compression syndrome and five with brain t

Fractures of experimentally traumatized embalmed versus unembalmed cadaver legs.

Intact legs from six geriatric cadavers were fractured in a self-controlled study aimed at documenting the effects of embalming on both the soft and hard tissues of cadaver specimens subjected to biomechanical impact research. Upon bequeathal, one leg was removed and frozen while the other remained with the cadaver for embalming. The embalmed legs were amputated later and pre-test radiographs were made. For testing, a rod was inserted in the upright leg such that simulated upper body mass could be applied. A 50 kg cart propelled by a pneumatic accelerator to 7.5 m/s struck the anterior leg mid

Comparison of BOLD cerebrovascular reactivity mapping and DSC MR perfusion imaging for prediction of neurovascular uncoupling potential in brain tumors.

The coupling mechanism between neuronal firing and cerebrovascular dilatation can be significantly compromised in cerebral diseases, making it difficult to identify eloquent cortical areas near or within resectable lesions by using Blood Oxygen Level Dependent (BOLD) fMRI. Several metabolic and vascular factors have been considered to account for this lesion-induced neurovascular uncoupling (NVU), but no imaging gold standard exists currently for the detection of NVU. However, it is critical in clinical fMRI studies to evaluate the risk of NVU because the presence of NVU may result in false ne

Lactate dynamics in Parkinson's disease: striatal circuit vulnerability, biomarker potential, and therapeutic windows.

Lactate is a mobile carbon substrate, extracellular signal, and candidate mediator of metabolism-to-gene coupling. These properties make lactate relevant to Parkinson's disease (PD), but current evidence does not support a simple protective or toxic interpretation. In this Review, we synthesize evidence that lactate dynamics may influence PD-relevant striatal biology through substrate provision, redox-sensitive NMDAR-ERK/MAPK-CREB signaling, HCAR1-mediated receptor signaling, and the emerging lactylation-linked modification of inflammatory or transcriptional states. We explicitly distinguish e

New horizons: disrupted brain energy metabolism as a driver of delirium.

Delirium is a highly prevalent neuropsychiatric syndrome characterised by acute inattention, altered arousal and impaired cognition. Cerebral energy insufficiency is hypothesised to drive delirium and both hypoglycaemia and hypoxia can directly precipitate functional deficits and EEG slowing. Here we review the evidence that disrupted energy metabolism may play a causative role in delirium across multiple settings. Neuromonitoring methods including near infrared resonance spectroscopy and Transcranial Doppler suggest an association between altered cerebral perfusion and delirium, albeit with a

PCSK9 inhibition attenuates alcohol-induced cardiovascular dysfunction and links hepatic lipid accumulation to impaired myocardial contractile reserve.

Excessive alcohol consumption accelerates cardiovascular aging by promoting oxidative stress, inflammation, lipid dysregulation, fibrotic remodeling, and loss of ventricular-vascular reserve. PCSK9, a key regulator of cholesterol metabolism, has emerged as a mediator of age-related cardiovascular dysfunction and alcohol-associated liver and neurovascular injury. We investigated whether PCSK9 inhibition protects against alcohol-induced cardiovascular dysfunction and associated cardiac-hepatic injury in rats. Male Sprague-Dawley rats were assigned to pair-fed control or 35% ethanol liquid diet g

Impact of exercise on cognitive efficiency in older adults with subjective memory complaints: a randomized clinical trial.

INTRODUCTION: Cerebrovascular dysfunction may contribute to cognitive impairment in older adults. Exercise is recommended for preventing cerebrovascular health and cognition; however, it remains unclear whether cognitive changes are related to changes in cerebrovascular function. Thus, this study aimed to examine whether exercise-induced changes in cognition and subjective memory complaints are associated with changes in cerebral blood velocity. METHODS: This study is a randomized controlled trial that included 72 sedentary older adults with memory complaints (age = 62 ± 7, 19 [26%♂]). Partic

ABCC9/SUR2 has a Complex Expression Pattern in Human Brain Gliovascular Unit Cells, Including Astrocytes.

The ABCC9 gene and its cognate protein SUR2 play important roles in neurovascular coupling and are implicated in hippocampal sclerosis of aging (HS-Aging). However, prior studies have not focused on human brain SUR2 expression or SUR2 in glial cells. Here we analyzed cell type-specific ABCC9/SUR2 expression patterns, and correlation with known genetic risk variants, using multiple data sets and a novel antiserum. Existing single-nucleus RNA sequencing data sets and new spatial transcriptomics data indicated that SUR2 transcripts were expressed primarily in human brain pericytes, astrocytes, sm

Transcranial direct current stimulation to improve cognitive function in patients with vascular cognitive impairment: a literature review.

Vascular pathology represents the second most common cause of cognitive impairment after Alzheimer's disease and contributes substantially to mixed dementia involving both vascular and neurodegenerative mechanisms. Vascular cognitive impairment (VCI) emerges as a term encompassing cognitive impairments resulting from stroke, cerebral small vessel disease (CSVD), or other forms of vascular pathology. While numerous studies have demonstrated the beneficial effects of transcranial direct current stimulation (tDCS) on mild cognitive impairment and dementia, evidence specifically addressing its eff
