OBJECTIVE: This case series documents an occupational outbreak of methemoglobinemia among five steam press operators at a rubber plant. Investigative findings identified the cause as repeated exposure (through manual handling) to an adhesive containing dinitrobenzene.
RESULTS: The workers presented with yellow-stained hands and a variety of clinical manifestations. Methemoglobinemia levels obtained in the emergency room ranged from 3.8% to 41.2%. An increased incidence of certain neoplasms occurs in immunodeficiency states. The incidence of cancer in organ transplant patients is approximately 4%. The predominant tumors are lymphomas, carcinomas of the skin and lips, carcinomas of the vulva/perineum, in situ carcinomas of the uterine cervix, and Kaposi sarcoma (KS). In June 1977 an outbreak of acute gastroenteritis affected 103 students and teachers at an elementary school in Ohio. The illness typically lasted 24 hours or less and was characterized by vomiting (86%) and cramping (70%), but more than half of the persons involved also reported having nausea, diarrhea, and headache. Data on phorias, vergences, and fixation disparities were gathered for distant and near viewing conditions from a nonclinical, young adult population. Normative values were computed from the data for analysis and comparison. Our study revealed slightly less exophoria at far and near than Morgan's study, together with large base-out vergence readings. We found proportionately more esophores with exo fixation disparity who require base-in prism to neutralize the fixation disparity than Ogle's studies found.
