INTRODUCTION: The relationship between the optic disc size, neuroretinal rim and cup diameter is the key in identifying glaucomatous changes in optic nerve head. Like some other biological characteristics, changes of the optic disc differ among healthy populations, thus hindering detection of pathological changes.
MATERIAL AND METHODS: From the total number of 90 patients, we selected those that have optic disc diameter less than 1.30 mm, greater than 2.10 mm, as well as patients with a size of disc diameter ranging from 1.31mm to 2.09 mm. In all patients after anamnesis and standard ophthalm

Isolated and total optic disc coloboma is a markedly rare congenital anomaly of the eye. Congenital eye colobomas frequently present with eye anomalies and systemic signal findings. We present a case of isolated total optic disc coloboma without systemic involvement, which has been rarely reported in the literature.

Comparisons across C/D-based optic disk cupping categories were performed using patient-clustered generalized estimating equations (GEE) to account for inter-eye correlation. Multivariable GEE linear regression was used to identify factors independently associated with cup-to-disk ratio ( …

Glaucoma is the leading cause of irreversible blindness worldwide. Often asymptomatic for years, this disease can progress significantly before patients become aware of the loss of visual function. Critical examination of the optic nerve through ophthalmoscopy or using fundus images is a crucial component of glaucoma detection before the onset of vision loss. The vertical cup-to-disc ratio (VCDR) is a key structural indicator for glaucoma, as thinning of the superior and inferior neuroretinal rim is a hallmark of the disease. However, manual assessment of fundus images is both time-consuming a

Non-arteritic anterior ischaemic optic neuropathy (NAION) is typically characterised by non-pallid optic disc oedema and a contralateral small optic nerve head. Typically, the optic cup is absent or small with a cup-to-disc ratio of less than 0.3 termed 'disc at risk'. While systemic and ocular risk factors are identified as risk factors of NAION, the exact pathogenesis of NAION is not fully understood. We report a case of NAION with pallid optic disc oedema and the absence of a disc at risk in a patient with multiple vascular risk factors, suggesting an underlying severe atherosclerotic patho

Optical coherence tomography (OCT) is an in vivo imaging modality that provides non-invasive, high resolution and fast cross-sectional images of the optic nerve head, retina and choroid. OCT angiography (OCTA) is an emerging tool. It is a non-invasive, dye-free imaging approach of visualising the microvasculature of the retina and choroid by employing motion contrast imaging for blood flow detection and is gradually receiving attention for its potential roles in various neuro-ophthalmic and retinal conditions. We will review the clinical utility of the OCT in the management of various common n

BACKGROUND: Optical coherence tomography (OCT)-obtained retinal nerve fiber layer (RNFL) thickness measurements allow for qualitative and quantitative assessment of RNFL thinning in glaucoma. Errors in data acquisition or in software analysis may result in artifacts and erroneous RNFL thickness measurements that may lead to an inaccurate clinical interpretation.
PURPOSE: This video describes tips to recognize artifacts in RNFL thickness measurement in various OCT printouts.
SYNOPSIS: The video presents a series of OCT of optic nerve head printouts with artifacts and useful teaching points to

OBJECTIVES: To investigate the relationship between the shape of the optic nerve head (ONH) margin detected by optical coherence tomography (OCT) and the clinical characteristics of glaucomatous eyes with papillomacular retinoschisis (PMRS).
METHODS: The medical record of patients with a PMRS in a glaucomatous eye were reviewed. The eyes were placed into two groups determined by the shape of the ONH margin in the OCT images; eyes with an externally oblique ONH margin (Group 1) and eyes with an internally oblique ONH margin (Group 2). We compared the clinical characteristics of the PMRS of the

Although the visual system extends through the brain, most vision loss originates from defects in the eye. Its central element is the neural retina, which senses light, processes visual signals, and transmits them to the rest of the brain through the optic nerve (ON). Surrounding the retina are numerous other structures, conventionally divided into anterior and posterior segments. Here, we used high-throughput single-nucleus RNA sequencing (snRNA-seq) to classify and characterize cells in six extraretinal components of the posterior segment: ON, optic nerve head (ONH), peripheral sclera, perip

PURPOSE: To report a case of ophthalmomyiasis interna with optic nerve invasion that was treated with oral ivermectin and prednisone.
METHODS: Case report with retrospective review of medical records and multimodal imaging studies.
RESULTS: A Christmas tree farmer in his sixties presented with expanding multicolored, spiraling photopsias in the left eye. The visual acuity measured 20/50, and examination and imaging findings showed subretinal tracks consistent with ophthalmomyiasis interna. After several weeks of spontaneous improvement, the visual acuity decreased to 20/150 and fundus examin

Unlike the usual peripheral nerve, the optic nerve accompanies a thick "dural sheath," a thin "sheath of pia mater" (SPM), and multiple "sep … rve. These connective tissue configurations were the same between adult and fetal specimens. At the optic disk, the dural sheath and SPM merged with the sclera, whereas the septa appeared to end at the lamina cribrosa. However, in fetuses without lamina cribrosa, the septa extend into the nerve fiber layer of the retina. The SPM and septa showed strong elast …

PURPOSE: To determine the association of the multifocal electroretinographic (mfERG) response amplitude with the volumes of the inner, postreceptor, and photoreceptor retinal layers in the region stimulated by each mfERG element.
METHODS: Sixteen healthy, young adult control subjects were studied. Each of the 103 hexagonal elements of the standard, scaled mfERG were aligned, where possible, with patches of retina imaged using optical coherence tomography. Stimuli falling on the fovea and on the optic nerve head were excluded. Linear mixed-effects modeling was then used to derive estimated coe

PURPOSE: To develop a three-dimensional (3D) deep learning algorithm to detect glaucoma using spectral-domain optical coherence tomography (SD-OCT) optic nerve head (ONH) cube scans and validate its performance on ethnically diverse real-world datasets and on cropped ONH scans.
METHODS: In total, 2461 Cirrus SD-OCT ONH scans of 1012 eyes were obtained from the Glaucoma Clinic Imaging Database at the Byers Eye Institute, Stanford University, from March 2010 to December 2017. A 3D deep neural network was trained and tested on this unique raw OCT cube dataset to identify a multimodal definition

PURPOSE: To report a case of unilateral disc swelling as the only presentation of enterovirus meningitis in a 43-year-old male who presented with reduction in visual quality.
METHODS: Vision was 20/20 unaided in both eyes and the only ocular finding was left optic disc edema accompanied by left relative afferent pupillary defect. Visual field examination revealed a generalized reduction in sensitivity, a paracentral scotoma and a superior and inferior arcuate defect. Apart from a marginally elevated white blood cell count, the evaluation for common diseases associated with unilateral disc swe

Deep learning (DL)-based retinal image quality assessment (RIQA) algorithms have been gaining popularity, as a solution to reduce the frequency of diagnostically unusable images. Most existing RIQA tools target retinal conditions, with a dearth of studies looking into RIQA models for optic nerve head (ONH) disorders. The recent success of DL systems in detecting ONH abnormalities on color fundus images prompts the development of tailored RIQA algorithms for these specific conditions. In this review, we discuss recent progress in DL-based RIQA models in general and the need for RIQA models tail
