Optic neuritis complicating measles, mumps, and rubella vaccination.

A 6-year-old boy developed bilateral optic neuritis with decreasing visual acuity 18 days after administration of live attenuated trivalent measles, mumps, and rubella vaccine. The patient was treated with oral corticosteroids. The optic neuritis resolved within several weeks and normal vision returned. An afferent pupillary defect persistent in the more severely involved eye for 14 months following vaccination.

The pattern evoked responses (VER) in optic neuritis.

Five patients presenting clinically accepted signs of optic neuritis served as subjects for a study on EP's (evoked potentials); the patients were tested during the acute phase, the chronic phase and at a stage when no impairment was clinically observed. At the acute phase of optic neuritis the VER was massively altered; in the chronic phase delayed latencies depending upon the impairment of visual acuity and visual fields could be recorded. At the recovery stage, while visual acuity and visual field showed no deficit, not only delayed latencies but also reduction in the amplitude at all stimu…

Symptoms, signs and early course of optic neuritis.

The study was an anlysis of the early course of optic neuritis based on the case histories of 185 patients, 57% of whom were females and 43% males. More than half of the patients suffered from multiple sclerosis. In 28% of the patients the etiology remained unknown. The most common initial symptom was acute decrease in visual acuity, but in 25% the onset was subacute or slow. Pain occured in 62% and preceded decrease in visual acuity in 16% of the cases. The initial attack was unilateral in 70% and bilateral in 30% of the patients. On admission, in 64% of the involved eyes, visual acuity was p…

Unilateral optic disc drusen mis-diagnosed as optic neuritis: Diagnostic and therapeutic implications.

Objective: To present a case of unilateral optic disc drusen, initially mis-diagnosed as optic neuritis, which led to chronic systemic administration of steroids and the development of hypercortisolism. Methods: A 22-year-old female was referred because of the lack of improvement of the manifestations of optic neuritis despite the chronic use of systemic steroids. Presence of unilateral optic disc edema was initially observed, associated with ipsilateral scotomata and increased ipsilateral latency time in visually evoked potentials (VEP). Results: A CT scan, A and B ultrasonography and autoflu…

Malignant optic glioma masked by suspected optic neuritis and central retinal vein occlusion.

Malignant optic glioma presents a clinical and diagnostic challenge, as early imaging findings overlap with other more common causes of optic nerve enhancement and enlargement, potentially leading to delay in diagnosis. This rare diagnosis carries an extremely poor prognosis, with death usually occurring within 1 year. We present a case of malignant optic glioma that was initially diagnosed as optic neuritis and central retinal vein occlusion, and we emphasize the importance of serial imaging and definitive biopsy to promote early diagnosis and treatment of this entity.

Initial Pattern of Optic Nerve Enhancement in Korean Patients with Unilateral Optic Neuritis.

PURPOSE: The purpose of this study was to demonstrate whether the pattern of optic nerve enhancement in magnetic resonance imaging (MRI) can help to differentiate between idiopathic optic neuritis (ON), neuromyelitis optica (NMO), and multiple sclerosis (MS) in unilateral ON. METHODS: An MRI of the brain and orbits was obtained in patients with acute unilateral ON. Patients with ON were divided into three groups: NMO, MS, and idiopathic ON. The length and location of the abnormal optic nerve enhancement were compared for ON eyes with and without NMO or MS. The correlation between the pattern …

Noninvasive assessments of optic nerve neurodegeneration in transgenic mice with isolated optic neuritis.

PURPOSE: To determine if phosphorylated neurofilament heavy chain (pNF-H) released into the bloodstream and the pattern ERG are noninvasive indicators of neurodegeneration in experimental optic neuritis. METHODS: Serum from Myelin oligodendrocyte glycoprotein (MOG)-specific T cell receptor-positive (TCR+) transgenic mice that develop isolated optic neuritis usually without any other characteristic lesions of inflammation or demyelination in the spinal cord and littermates negative for the transgene were assayed for the presence of serum phosphorylated neurofilament H (pNF-H). In vivo measurem…

Optic nerve diffusion changes and atrophy jointly predict visual dysfunction after optic neuritis.

Recently, there has been strong interest in the development of imaging techniques to quantify axonal and myelin pathology in patients with multiple sclerosis (MS). Optic neuritis, a condition characterised by inflammatory demyelination of the optic nerve, is one of the commonest sites of MS relapse, and exhibits similar pathological alterations to MS lesions elsewhere in the central nervous system (CNS). Unlike other regions of the CNS, however, the function of the optic nerve can be accurately assessed using clinical measures, as well as electrophysiological techniques such as visual evoked p…

[Leber hereditary optic neuropathy and optic neuritis: similarities and differences of clinical characteristics].

OBJECTIVE: To compare the clinical characteristics of Leber hereditary optic neuropathy (LHON) with optic neuritis (ON). METHODS: The information of clinical manifestation and laboratory tests in 55 patients with LHON and 48 patients with ON were reviewed and analyzed. RESULTS: Compared with ON, LHON was more commonly found in male patients (46/55 vs 21/48) and onset at younger age (median 16 years vs 30 years). There were more LHON than ON patients with the decrease of visual acuity lasted more than two weeks (42/49 vs 10/48), severe damage in visual acuity whereas little pain sensation in …

Visual field defects in optic neuritis and anterior ischemic optic neuropathy: distinctive features.

BACKGROUND: We analyzed the value of visual-field defects in the differential diagnosis of optic neuritis (ON) and non-arteritic anterior ischemic optic neuropathy (AION). METHODS: Ninety-nine consecutive patients with acute-onset optic neuropathy formed the basis for this study. Compressive and vasculitic neuropathies were excluded. Eighty-six patients fulfilled the criteria for either ON (50 patients): < or = 35 years, normal disk, recovery of visual function, or AION (36 patients): > or = 60 years, swelling of the disk, no recovery of visual function. Without knowledge of other clinical da…
