The purpose of this study was to investigate the influence of the lateral wedge on asymmetrical weight bearing and posture in hemiplegic gait. The subjects were 2-female and 4-male moderate stroke patients, average age 54 +/- 6 yr. The time from onset was 127 +/- 69.7 days. A 7 mm-thick wedged insole was applied to the affected side for a week. Before and after that, the vertical ground reaction force, single supporting period and side movement of the trunk were recorded during walking. As a result, we found that the vertical ground reaction ratio and single supporting period ratio of the affe

PURPOSE: In this study our aim was to evaluate the effectiveness of Ankle-Foot Orthoses (AFOs) on gait function in patients with spastic cerebral palsy for whom orthoses were indicated to control dynamic equines deformity.
METHOD: Twenty-four spastic cerebral palsied patients with dynamic equines deformity were included in the study. Videotape recordings were performed to each patient on the same day with barefoot and AFOs. Temporal distance factors including velocity, cadence, stride length, stride width and Clinical Gait Assessment Score (CGAS) were compared across two conditions.
RESULTS:

Writer's cramp is a focal, task-specific dystonia of the hand and wrist. It primarily affects people who do a significant amount of writing, and causes difficulties in writing. We present five cases with writer's cramp who showed improvement in their writing ability with an applied hand orthosis.

The purpose of this study was to determine the effects of the ankle-foot orthosis (AFO) on gait patterns in patients with low-level myelomeningocele and to identify any abnormal gait patterns that may lead to future knee instability and pain. A total of 28 children (26 L4-level sides, 18 L5-level sides, and 10 S1-2-level sides) underwent a three-dimensional gait analysis when ambulating barefoot and with AFOs. Results show significant improvements in sagittal plane function with reductions in excessive ankle dorsiflexion, increases in peak plantar flexor moment, and reductions in crouch and kn

The treatment of rheumatic disease requires an interdisciplinary approach in which the orthopedic surgeon is also involved. His contribution includes both conservative and operative procedures. Early synovectomy applied to joints and tendons is considered a preventive measure. Arthroscopic synovectomy belongs in the hands of an experienced operator. Joints that are not directly involved in weight-bearing can be treated by means of arthroplastic procedures, provided the cooperation of the patient is ensured. Arthrodesis is used preferentially to treat articular destruction of the ankle joint, t

STUDY DESIGN: This report is an account of three patients with scoliosis and polyostotic fibrous dysplasia involving the spine. The perioperative course of two patients who underwent posterior spinal arthrodesis is described.
OBJECTIVES: The objective of the study was to show that extensive fibrous dysplasia of the spine and scoliosis can be treated by standard methods of posterior spinal arthrodesis with good results.
SUMMARY OF BACKGROUND DATA: Scoliosis developed in three patients, and two patients underwent spinal arthrodesis. No report that discusses the results of this type of operatio

A case of a rare genetic disorder with typical lower extremity and gait alterations as the chief concern was presented. A description of the characteristic lower extremity and gait manifestations of this syndrome has been developed. The major and minor diagnostic criteria, which have been published to identify this syndrome, were reviewed with respect to this case. The practitioner who has the opportunity to evaluate patients for gait and lower extremity complaints should be aware of these correlations to aid in timely diagnosis of this condition. Management of compensations from these lower e

Providing CP Patients with functional clubfoot with orthopaedic shoe modifications has not a good evaluation in the literature. An own concept of fitting this kind of spastic foot deformity is demonstrated. It is based on stabilizing shoe modifications combined with spasticity inhibiting posture of the foot in the shoe. The results based on a follow-up examination of nearly five years can be considered positive.

An excessive prominence of the bursal projection in the posterosuperior aspect of the calcaneous constitutes Haglund's deformity. Swelling in this area constitutes Haglund's disease and is associated with retrocalcaneal bursitis. Rigid and prominent heel counters with high heels impinge on the soft tissues overlying the prominence and give rise to symptoms of pain and swelling. Cavo varus deformities exacerbate this problem. The bursal projection can be demonstrated radiologically by a superior calcaneal angle of more than 75 degrees, a combination of calcaneal inclination and a posterior calc

The diagnosis and treatment of lateral foot pain is often difficult. The differential diagnoses are numerous, including osseous abnormalities, sprains, peripheral mononeuropathies, and radiculopathy. An often overlooked cause of lateral foot pain is sinus tarsi syndrome. From a review of the literature, it appears that even when the diagnosis of sinus tarsi syndrome is made, the biomechanics of the foot and ankle are not considered. The following is a case study of a patient with sinus tarsi syndrome treated successfully through biomechanical evaluation, and fabrication and use of custom-molde

Splints, exercise, traction, and compression garments are commonly accepted methods to minimize disabling scar formation. Although burn rehabilitation treatment has improved over the past 10 years, there is still no overnight cure for scars and contracture. The extent and depth of the burn injury, emotional strength and patience of the burn victim, and support systems available play an important role in scar treatment. Scar contracture is a frustrating complication for the recovering patient and burn team. Surgical reconstruction to correct functional impairment is often needed before wound ma

In the relatively recent past, the treatment of congenital dislocation of the hip was carried out according to Lorenz or to some other similar method. Many poor late results and too many deformities of the proximal part of the femur were observed, following a modification of the orientation of the femoral neck or avascular necrosis of the head and its growth plate. The full abduction position was responsible for these complications. Since 1960, a method has been developed based on the observation of the normal physiologic fetal position of the thighs close to the trunk, and on the fact that in

Modification of a hand splint for a musician who had suffered an incomplete disruption of the left ring finger extensor tendon is described. This modification enabled the patient to receive the benefit of conservative treatment of this disruption while continuing his employment. The modification can be performed easily with the standard stack splint; the success of this method is described.

A pneumatic walking training aid for amputees is described. It was developed by the late Biomechanical Research and Development Unit, Roehampton, from an original design by Professor Little in Australia. The walking aid has been used successfully on the majority of below-knee and through-knee patients rehabilitated in the Roehampton Walking Training School since 1976. There have been no complications that could be attributed to the use of the pneumatic walking aid.

The authors report their experience in the treatment of acquired ulcerating and mutilating trophic lesions of the lower limbs. Ligature of the dorsalis pedis artery and of its collaterals did not give good results, according to the pathogenic concept of the authors, in which lesions of the nervous system predominate over vascular alterations. More important lesions (such as osteo-arthritis of the first and fifth metatarsals) require immobilization of the axis of the wounded toe, control of any superinfection and use of a plastic removable limb appliance.

Weight-bearing orthoses have been disigned with the primary goal of reducing or eliminating weight bearing in skeletal structures. In evaluating various designs and adjustments of these orthoses, we found that the training of the subject in the use of his orthosis was one of the most significant factors in controlling weight bearing. Subjects fitted with instrumented ischial weight-bearing and patellar-tendon-bearing orthoses walked over a force plate which measured forces imparted to the floor. The force data from the orthoses and force plate were computer-processed with single frame, multipl
