This article addresses the accepted indications for using ultrasound alone or in conjunction with other imaging techniques in solving problems of breast diagnosis and guiding patient management. The indications include evaluation of palpable and mammographic masses; guidance of interventional procedures; evaluating problems with implants; radiation treatment planning; and as the initial imaging technique in young,pregnant, and lactating patients. Evolving indications and controversial applications for breast ultrasound are also discussed.

Thirty two patients aged between 2 and 19 years underwent laparoscopy for 32 impalpable testes. There were no complications associated with laparoscopy and the procedure clarified the condition in all patients. The purpose of this paper is to estimate the value of laparoscopy in boys with impalpable testes.

An underappreciated characteristic of prostate cancer screening is that it may detect some prostate cancers solely by serendipity or chance. Serendipity, previously described in the detection of colonic neoplasms, could affect prostate cancer detection when a screening test result is abnormal for reasons other than the presence of prostate cancer, but prostate cancer is coincidentally detected during the subsequent evaluation of the abnormal screening result. Despite the fact that cryptorchidism is a common congenital anomaly, our understanding of this problem is hampered by confusion and uncertainty regarding diagnosis and treatment. A major factor producing this confusion is the lack of a common terminology that describes to the reader (or listener) exactly what the author means by a given term. Herein is proposed a classification system that, if adopted and utilized, could ameliorate this problem.

We evaluated 68 prepubertal boys with 84 impalpable testes who were operated upon without using any other diagnostic maneuvers. Of the testes 18 (22%) were absent (anorchism or 'vanished') and 38 (45%) could be placed in a scrotal position with standard orchiopexy. A staged, Fowler-Stephens or microvascular procedure was required for 28 testes (33%), involving orchiectomy in 2 cases, and succeeded in a scrotal position for another 24 testes. A series of 161 boys with 178 impalpable testes is reviewed. Extended inguinal exploration was the main form of investigation and treatment. Hormone injections and studies were performed in bilateral cases. Sonography was unhelpful. Laparoscopy was not used.
